ABSTRACT

The emergent IMS (Internet Protocol Multimedia Subsystem) technology appears to improve the current communication technologies. Its characteristics, such as Quality of Service (QoS), make it an advantageous system for innovative applications. Providing integrated services to users is one of the main reasons for the existence of IMS. Operators provide the technology as an open source, to be able to use services developed by researchers. Combining and integrating them, users will receive completely new services. Our proposal of use for IMS is the development of a telemedicine platform, designed to support not only remote biological signal monitoring, but value-added services for diagnosis and medical care, both of these working in real time.

ABSTRACT

Objetivo: Hay pocos estudios sobre las manifestaciones osteomusculares tras un trasplante alogénico de progenitores hematopoyéticos (TPH-ALO). El objetivo del presente estudio es investigar los síntomas osteomusculares observados tras la realización de un TPH-ALO como tratamiento de distintas enfermedades hematológicas y no hematológicas. Material y métodos: Estudio observacional retrospectivo, realizado mediante la revisión de historias clínicas. Resultados: Desde el año 1996 hasta el 2000 se identificó a 123 pacientes sometidos a un TPH-ALO, 34 niños (5 meses-14 años) y 81 adultos (15-55 años). La distribución por sexos fue la siguiente: 48 mujeres (39%) y 75 varones (61%). De estos pacientes, 43 (34,90%) presentaron enfermedad del injerto contra el huésped (EICH) aguda, 33 de 106 (31,1%) presentaron EICH crónica y 50 de los pacientes (40,65%) murieron durante el seguimiento. Catorce (13,2%) presentaron manifestaciones osteomusculares: 6 (5,6%) necrosis aséptica, 5 (4,7%) esclerodermia, uno artralgias y bronquiolitis obliterante, uno poliartralgias y otro monoartritis de rodilla. Conclusiones: Es difícil atribuir los síntomas osteomusculares tras un TPH-ALO a una sola causa. Las manifestaciones inflamatorias y fibrosantes pueden ocurrir debido al proceso inflamatorio de la EICH crónica o por los tratamientos administrados durante el procedimiento(AU)

Musculoskeletal manifestations were found in 14 patients (13.2%): aseptic necrosis in six (5.6%), scleroderma in five (4.7%), arthralgia and bronchiolitis obliterans in one, polyarthralgia in one, and knee monoarthritis in one. Conclusions: Musculoskeletal manifestations after AHSCT cannot be attributed to a single cause. Inflammatory and fibrosing manifestations can be due to the inflammatory process of cGVHD or to the treatments administered during transplantation(AU)

ABSTRACT

Antecedentes. Hemos analizado de forma retrospectiva 36 pacientes diagnosticados de oftalmopatía de Graves tratados con radioterapia. Métodos. Hemos tratado 36 pacientes, 30 habían sido tratados previamente con glucocorticoides. El tratamiento se ha realizado en un acelerador lineal de electrones de 8 Mv con dos campos bilaterales y una dosis total de 20 Gy, fraccionada en 10 sesiones de 2 Gy. Los parámetros medidos han sido la proptosis, la afección de partes blandas, la diplopía y la agudeza visual. Resultados. La afección de partes blandas mejoró en un 66 por ciento, la proptosis en un 55 por ciento, la diplopía en un 35 por ciento y la agudeza visual en un 20 por ciento (p = 0,06). Conclusiones. La radioterapia orbitaria puede mejorar la mayoría de los signos y síntomas de la oftalmopatía de Graves (AU)

ABSTRACT

The umbilicus was preserved in 32 gastroschisis repairs. A modified technique was performed in the last eight infants, which provided an improved cosmetic appearance. The procedure is suitable for both primary and staged repairs.

ABSTRACT

Seventeen infants with a very low birth weight (VLBW) and spontaneous, non-necrotizing enterocolitis (NEC), intestinal perforations are presented; 14 of them were seen in the past 3 years. A comparison with 16 surgically treated NEC infants (< 1,000 g) is provided. At our institution, the yearly survival of VLBW infants increased from 54% to 90% over the past 6 years. All 17 non-NEC patients were operated on, and 15 (88.2%) survived. Ileal perforations were observed frequently. Initial enterostomies were followed by reanastomosis at an average age of 3 months. A 22.6-month follow-up was attained for all survivors. Non-NEC intestinal perforations in tiny neonates are increasing and constitute a challenging but treatable group. The improving survival rate of VLBW infants will probably be accompanied by a variety of complications.

ABSTRACT

A newborn with a large omphalocele is presented. A new technique of umbilical preservation is introduced. The cosmetic result was excellent, and the method is suitable for both primary and staged omphalocele repairs.

ABSTRACT

One hundred eight consecutive bilateral inguinal herniorrhaphies were performed in female infants and children over the past 56 months. A technique that uses a single suprapubic incision is described. The average age was 29.8 months. The operative time was decreased by 5.6 minutes, and the final results were excellent.

ABSTRACT

Catheter decompression of the bilioenteric conduit was routinely carried out coincident with stomal closure in 15 consecutive patients with biliary atresia after Kasai hepatic portoenterostomy operation. Postoperatively the catheter was systematically, gradually occluded over a 3- to 8-week period and then withdrawn. Complications were relatively minor and early postoperative cholangitis was avoided.
