ABSTRACT

Features of regulation of gastric acid production in 82 patients with favorable and unfavorable results of duodenal ulcers (DU) surgical treatment were studied. Current radioimmunoassays were used for assessment of hormonal and acid production function of operated stomach. It is demonstrated that an increase in gastrointestinal and pancreatic hormones secretion in postvagotomic syndromes depends on postoperative disorders degree and terms after gastric surgery that may be regarded as hyperactivity of regulatory systems. It is concluded that primary state of acid production and its regulators may be criteria of outcomes assessment of DU surgical treatment. Timely and targeted correction of these hormonal disorders in the postoperative period is the measure of prophylaxis of many postoperative complications.

ABSTRACT

The in vitro study of the influence of photodynamic action (with the use of photosensitizer "Photosens", laser and non-laser irradiation) on the strains of the main representatives purulent wound microflora was carried out. Staphylococcus aureus, S. epidermidis, Pseudomonas aeruginosa, Escherichia coli, Proteus mirabilis cultures have been isolated from wound secretions and identified. The photosensitizer was shown to produce no bactericidal effect by itself. Irradiation with laser and non-laser light sources induced a sharp decrease in the number of viable cells in the cultures under study. The irradiation of the photosensitizer led to its activation, manifested in bactericidal action. The results thus obtained confirm good prospect of using photodynamic therapy for the treatment of purulent wounds.

ABSTRACT

The work is based on the analysis of the results of operative interventions for different esophagus and gastric diseases in 263 patients during up to 40 year observation. In 222 patients cardia has been removed, and in 41 patient operations were performed for it's destruction. Postoperative reflux-esophagitis has been registered in 68 patients. The authors concluded that the most frequent complications of postoperative reflux-esophagitis are found to be scarry esophageal stricture (in 25 of 68 patients) and ulcers of esophagogastric anastomosis (in 8 of 68). That is why the surgical prophylaxis of esophagitis should be performed during the primary operation.

ABSTRACT

Selective proximal vagotomy (SPV) was performed in 658 patients. Recurrent ulcer was revealed in 6.2% of them. A classification of recurrent peptic ulcer is proposed. An operation was carried out for a second time in 65 patients. Correct techniques of SPV, excision of the ulcer, and stomach draining operations with consideration for the functional condition of the duodenum during the first operation are conducive to marked decrease in recurrences of the ulcerative process.

ABSTRACT

On the basis of complex examination of 83 patients with the ulcer disease including ionomanometry 25 of them were found to have hiatal hernia. Specific features of the clinical picture of this combination are described. The detection of hiatal hernia in patients with the ulcer disease allows the correction of the valve mechanism of the esophago-gastric transition to be simultaneously fulfilled during the operation.
