ABSTRACT

Resumen La regulación normativa y la jurisprudencia referente a la formalización de la práctica de la prostitución ha tenido avances significativos en el Estado Colombiano. Sin embargo, persisten una serie de dificultades que hace que el ejercicio de esta actividad se vea frustrada por falta de garantías en el ejercicio de los derechos, haciendo que los personas que ejercen esta actividad económica, se vean inmersas en la informalidad que históricamente ha caracterizado esta labor.


Abstract The normative regulation and jurisprudence regarding the formalization of the practice of prostitution has had significant advances in the Colombian State. However, there are still a series of difficulties, which frustrate the exercise of this activity due to the lack of guarantees in the exercise of rights, by causing people, who exercise this economic activity, to be immersed in the informality, which has historically characterized this work.

ABSTRACT

This paper presented a new approach to the calculation of quality-adjusted life years (QALY) based on multi-criteria decision-making (MCDM) methods and using the EQ-5D-5L questionnaire. The health status utility values were calculated through a hybrid methodology. We combined the analytic hierarchy process (AHP), the AHP with a D-number extended fuzzy preference relation (D-AHP), the fuzzy analytic hierarchy process (F-AHP), and the technique for order preference by similarity to the ideal solution (TOPSIS) to obtain individual and aggregated utility values. The preference data were elicited using a sample of individuals from a Colombian university. In all tested methods, the ordinal preferences were consistent, and the weights were compared using the Euclidean distance criterion (EDC). We identified F-AHP-TOPSIS as the optimal method; its benefits were associated with modeling the response options of the EQ-5D in linguistic terms, it gave the best approximation to the initial preferences according to EDC, and it could be used as an alternative to the known prioritization method. This hybrid methodology was particularly useful in certain medical decisions concerned with understanding how a specific person values his or her current health or possible health outcomes from different interventions in small population samples and studies carried out in low- and middle-low-income countries.

ABSTRACT

This document briefly describes the health conditions of the Colombian population and, in more detail, the characteristics of the Colombian health system. The description of the system includes its structure and coverage; financing sources; expenditure in health; physical material and human resources available; monitoring and evaluation procedures; and mechanisms through which the population participates in the evaluation of the system. Salient among the most recent innovations implemented in the Colombian health system are the modification of the Compulsory Health Plan and the capitation payment unit, the vertical integration of the health promotion enterprises and the institutions in charge of the provision of services and the mobilization of additional resources to meet the objectives of universal coverage and the homologation of health benefits among health regimes.

ABSTRACT

En este trabajo se presenta una breve descripción de las condiciones de salud de Colombia y una descripción detallada del sistema colombiano de salud. Esta última incluye una descripción de su estructura y cobertura, sus fuentes de financiamiento, el gasto en salud, los recursos con los que cuenta, quién vigila y evalúa al sector salud y qué herramientas de participación tienen los usuarios. Dentro de las innovaciones más recientes del sistema se incluyen las modificaciones al Plan Obligatorio de Salud y a los montos de la unidad de pago por capitación, la integración vertical entre empresas promotoras de salud y las instituciones prestadoras de servicios, así como el establecimiento de nuevas fuentes de recursos para lograr la universalidad e igualar los planes de beneficios entre los distintos regímenes.

ABSTRACT

El acceso a los servicios de salud está determinado por factores como la capacidad de pago, la distancia al sitio de atención, el estado de salud y la cultura. En ese contexto, el seguro facilita el acceso al remover las barreras financieras que impiden la búsqueda y utilización de servicios. El régimen subsidiado de salud colombiano (ley 100 de 1993) está inspirado en este propósito y cubre con un seguro de salud a la población de escasos recursos. De acuerdo con el esquema propuesto por Aday y Andersen de 1974 se estudian los resultados que ofrece el régimen en el departamento de Antioquia, en cuanto a condiciones de acceso y sostenibilidad financiera. Se encuentra que si bien la afiliación a la seguridad social muestra facilidades en el acceso, aún se presentan problemas referidos especialmente a pagos de bolsillo, distancia y organización de la oferta de servicios; además, existen dificultades para ampliar cobertura y sostener e1 plan de beneficios
