ABSTRACT

Resumen La coledocolitiasis secundaria es una entidad de alta prevalencia que involucra desafíos de diagnóstico y tratamiento; asimismo, genera un elevado uso de recursos y costo económico. Existen múltiples vacíos de conocimiento con respecto a los modelos de predicción clínica para el diagnóstico, estratificación en grupos de riesgo y manejo de pacientes con coledocolitiasis secundaria. Los principales vacíos recaen sobre su rendimiento diagnóstico, variables incluidas y umbrales de riesgo, así como sobre su costo-efectividad para el uso de recursos no invasivos e invasivos, y aplicación en grupos poblacionales especiales. Este artículo ahonda estos vacíos de conocimiento y propone una agenda que puede orientar la investigación futura.


Abstract Secondary choledocholithiasis is a disease of high prevalence that involves diagnostic and treatment challenges; it implies a high use of resources and economic costs. There are significant knowledge gaps related to clinical prediction models, risk group classification, and patient treatment in secondary choledocholithiasis cases. Those gaps include diagnostic performance, variables, and risk thresholds, as well as cost-effectiveness for the use of non-invasive and invasive resources, and their application in special population groups. This study analyzes those knowledge gaps and outlines a guideline that could lead future research.

ABSTRACT

Background Ankle fractures are one of the main causes of consultation with emergency services around the world, with an incidence of 187 / 100,000 inhabitants per year. The most common is type B in the Weber-Dannis classification, with a long-term evolution towards osteoarthritis of 14%. Although the diagnosis of distal fibular fracture on an ankle radiograph is very simple, its usefulness has probably been overestimated to provide the necessary insights to decide the appropriate treatment method for the fracture. The objective of the study is to evaluate the diagnostic utility of ankle radiographs and their inter-observer agreement for a treatment decision making in Weber B fractures. Methods A prospective, randomized, double-blind, multicenter trial using three-country surveys of orthopedic surgeons was conducted; orthopedic surgeons and foot and ankle surgeons, were exposed to four Weber B ankle fractures x-ray series where surgery was actually required as the only treatment option. Unilateral and comparative projections, where presented in different time and order to each surgeon, measuring the variability of surgical decisions. Results Fifty-one orthopedic surgeons (72.37%), 24 foot and ankle surgeons (15.79%), 4 surgeons of other orthopedic subspecialties (2.63%) and 14 orthopedic surgery residents (9.21%) were surveyed. An inter-observer agreement of 70.83% (p <0.05) was found in the comparative projections series and 55.31% (p <0.05) in the unilateral ones. When performing linear regression between the training degree, years of experience and the provenance country of the surgeons, no causality was found (power of ß <95%). Nevertheless, 100% of the respondents diagnosed the fracture of the distal fibula as a Weber B fracture. Discussion The unilateral and comparative radiographic series are not recommended for making an adequate surgical decision in Weber B ankle fractures, given their very low inter-observer agreement. For decision making in Weber B ankle fractures it is necessary to take other complementary examinations such as Computed Axial Tomography.

Aunque el diagnostico de la fractura del peroné distal en una radiografía de tobillo es muy sencillo, probablemente se ha sobrestimado su utilidad para proporcionar elementos de juicio necesarios para decidir el método de tratamiento adecuado para la fractura. El objetivo del estudio es evaluar la utilidad diagnóstica de las radiografías de tobillo y su concordancia interobservador para la toma de decisiones de tratamiento en fracturas de tobillo Weber B. Materiales y Métodos Se realizó un experimento, prospectivo, aleatorizado, doble ciego, multicéntrico, mediante la aplicación de encuestas en tres países a Ortopedistas, residentes de ortopedia y Cirujanos de pie y Tobillo, en las cuales se les presentaban cuatro casos de fracturas de tobillo Webber B de características quirúrgicas en proyecciones unilaterales y comparativas, midiendo la variabilidad de las decisiones quirúrgicas. Resultados Se recolectaron 152 encuestas que respondieron 110 ortopedistas (72.37%), 24 cirujanos de pie y tobillo (15.79%), 4 cirujanos de otras subespecialidades ortopédicas (2.63%) y 14 residentes de ortopedia (9.21%). Se encontró una concordancia interobservador en las proyecciones comparativas del 70.83% (p<0.05) y en las unilaterales del 55.31%, (p<0.05), al realizar regresión lineal entre la respuesta el grado de entrenamiento, los años de experiencia y el país de procedencia no se encontró causalidad (poder de ?? <95%). El 100% de los encuestados diagnosticó la fractura del peroné distal. Discusión La serie radiográfica unilateral y comparativa no se recomiendan para la toma de una adecuada decisión quirúrgica en las fracturas de tobillo Weber B, dada su muy baja concordancia inter-observador. Para la toma de decisiones en fracturas de tobillo Weber B es necesaria la toma de otros exámenes complementarios como la Tomografía axial Computarizada.

ABSTRACT

Resumen La Asociación Colombiana de Osteoporosis y Metabolismo Mineral se reunió a principios de 2017 para actualizar el Consenso Colombiano de Osteoporosis, elaborado por primera vez en 2005, un paso que se consideró necesario en vista del subdiagnóstico de esta enfermedad, el impacto esperado del envejecimiento poblacional y los cambios en el tratamiento farmacológico que ha habido desde entonces. Se seleccionó un equipo técnico con especialistas de múltiples áreas y amplia trayectoria, repartidos en 4 grupos de trabajo: definición y epidemiología, diagnóstico, tratamiento farmacológico y medidas no farmacológicas. Luego de una revisión de la literatura científica, en reuniones de trabajo se generaron las definiciones y recomendaciones que se resumen en este documento.


Abstract The Colombian Osteoporosis and Mineral Metabolism Association met in early 2017 to update the Colombian Consensus on Osteoporosis. This was first issued in 2005, and is seen as a necessary step in view of the underdiagnosed status of this disease, and the expected impact of population ageing. A technical team was formed with specialists with long experience across multiple disciplines, who were assigned to four working groups: definitions and epidemiology, diagnosis, pharmacological treatment, and non-pharmacological treatment. After a scientific literature review and a series of meetings, the definitions and recommendations are summarised in this article.

ABSTRACT

INTRODUCTION: Colorectal cancer (CRC) incidence is rapidly increasing. It has been demonstrated that it can be prevented and cured when the diagnosis is made in early stages. OBJECTIVE: For this reason it is necessary to apply a screening program in asymptomatic patients. METHOD: Since 2003, we conducted a CRC screening plan called "Mes del Colon" at Clínica Alemana Santiago. A press and local diffusion campaign was designed. Open to the community CRC talks were scheduled. An ad hoc database was designed. An informed consent was available. Patients older than 50 years and high risk patients were included. Total colonoscopy and a medical interview after the procedure were included in the plan with favorable economic conditions. RESULTS: Since 2003, 1158 patients were included The 1.8% of them were excluded because of incomplete data or because they did not meet the inclusion criteria. The 54% of patients were women. Mean age was 58.4 years old and mean body mass index 25.5 kg/m2. Polipoid lesions were seen in 45% of the patients. Six (1%) of them were adenocarcinomas, 291 (57%) adenomas (98% tubular adenomas), 189 (37%) hyperplastic polyps and 25 (5%) miscellaneous lesions. In this series, the necessary number to investigate for 1 adenoma was 3.9. CONCLUSIONS: CRC prevention campaigns are needed due to the continuous increase of the incidence in our country. The detection of precursor or early lesions that are longstanding before becoming advanced cancer allows its treatment avoiding progression.

ABSTRACT

BACKGROUND: Cholecystectomy is a common surgical procedure. In the open cholecystectomy area, antibiotic prophylaxis showed beneficial effects, but it is not known if its benefits and harms are similar in laparoscopic cholecystectomy. Some clinical trials suggest that antibiotic prophylaxis may not be necessary in laparoscopic cholecystectomy. OBJECTIVES: To assess the beneficial and harmful effects of antibiotic prophylaxis versus placebo or no prophylaxis for patients undergoing elective laparoscopic cholecystectomy. SEARCH STRATEGY: We searched the The Cochrane Hepato-Biliary Group Controlled Trials Register, the Cochrane Central Register of Controlled Trials (CENTRAL) in The Cochrane Library (Issue 3, 2010), MEDLINE (1985 to August 2010), EMBASE (1985 to August 2010), SCI-EXPANDED (1985 to August 2010), LILACS (1988 to August 2010) as well as reference lists of relevant articles. SELECTION CRITERIA: Randomised clinical trials comparing antibiotic prophylaxis versus placebo or no prophylaxis in patients undergoing elective laparoscopic cholecystectomy. DATA COLLECTION AND ANALYSIS: Our outcome measures were all-cause mortality, surgical site infections, extra-abdominal infections, adverse events, and quality of life. All outcome measures were confined to within hospitalisation or 30 days after discharge. We summarised the outcome measures by reporting odds ratios and 95% confidence intervals (CI), using both the fixed-effect and the random-effects models. MAIN RESULTS: We included eleven randomised clinical trials with 1664 participants who were mostly at low anaesthetic risk, low frequency of co-morbidities, low risk of conversion to open surgery, and low risk of infectious complications. None of the trials had low risk of bias. We found no statistically significant differences between antibiotic prophylaxis and no prophylaxis in the proportion of surgical site infections (odds ratio (OR) 0.87, 95% CI 0.49 to 1.54) or extra-abdominal infections (OR 0.77, 95% CI 0.41 to 1.46). Heterogeneity was not statistically significant. AUTHORS' CONCLUSIONS: This systematic review shows that there is not sufficient evidence to support or refute the use of antibiotic prophylaxis to reduce surgical site infection and global infections in patients with low risk of anaesthetic complications, co-morbidities, conversion to open surgery, and infectious complications, and undergoing elective laparoscopic cholecystectomy. Larger randomised clinical trials with intention-to-treat analysis and patients also at high risk of conversion to open surgery are needed.

ABSTRACT

Introducción: El cierre de la pared abdominal es un procedimiento frecuente en la práctica de los cirujanos generales. El tipo de cierre y el material de sutura en la mayoría de los casos se realizan y escogen en forma empírica. El presente estudio valora la mejor evidencia disponible sobre la técnica para el cierre de la fascia abdominal y el tipo de sutura. Métodos: Se realizó una búsqueda sistemática de la literatura y se evaluaron los artículos de más alta calidad (revisiones sistemáticas). Los resultados se analizaron particularmente. Resultados: La mejor técnica de sutura de la fascia abdominal es aquella con puntos continuos con material de absorción lenta (polidioxanona) o no absorbible (polipropileno), los cuales tienen índices de hernia incisional menores que las suturas de absorción rápida. Conclusiones: Es recomendable utilizar sutura de material de absorción lenta o no absorbible para el cierre de la pared abdominal.

OBJECTIVE: To assess the effectiveness of antibiotic prophylaxis in mesh hernioplasty. BACKGROUND: Antibiotic prophylaxis use in mesh inguinal hernioplasty is controversial. Available evidence is nonconclusive because of the low number of clinical trials assessing its effectiveness. Some trials have a small sample size that could overestimate or underestimate the real effectiveness of this intervention. Meta-analysis is a good method to improve these methodological flaws. METHODS: Meta-analysis intended to measure the benefits of antibiotic prophylaxis on surgical site infection rate in adult patients scheduled for mesh inguinal hernioplasty. Six randomized clinical trials were found. Quality was assessed using Cochrane Collaboration criteria. RESULTS: A total of 2507 patients were analyzed. Surgical site infection frequency was 1.38% in the antibiotic group versus 2.89% in the control group (odds ratio = 0.48; 95% confidence interval, 0.27-0.85). There was no statistical heterogeneity. Sensitivity analysis by quality did not show differences in overall results. CONCLUSION: Antibiotic prophylaxis use in patients submitted to mesh inguinal hernioplasty decreased the rate of surgical site infection by almost 50%.

BACKGROUND: Use of antibiotics in patients with isolated chest trauma is controversial. Available studies offer contradictory results because of small sample sizes. However, information provided by recent randomized controlled trials (RCT) included in a systematic review and meta-analysis could help solve the controversy. We performed a systematic review using high-quality information related to the use of antibiotics in patients with a chest tube. METHODS: We developed a systematic review to evaluate the effectiveness of prophylactic antibiotics in chest-trauma patients. Studies included were class I RCT comparing prophylactic antibiotics versus placebo in patients with isolated chest trauma. Main outcomes were posttraumatic empyema and pneumonia. RESULTS: Five Class I studies were selected. There were statistically significant differences regarding the frequency of posttraumatic empyema (RR 0.19) and pneumonia (RR 0.44) in favor of the use of prophylactic antibiotics when compared with placebo. CONCLUSIONS: The use of prophylactic antibiotics in patients with chest trauma decreases the incidence of posttraumatic empyema and pneumonia.
