ABSTRACT
Se presentan los datos clínicos, radiológicos, tomográficos y espirométricos de un paciente de 49 años de edad, fumador inveterado, que asistió a la consulta del cuerpo de guardia por presentar fiebre de 38,5ºC, anorexia, pérdida de peso y tos. Se realizaron exámenes en los que se incluyeron el hemograma con diferencial, eritrosedimentación, esputo BAAR, esputo bacteriológico y citológico, hemocultivo, rayos X de tórax vista posteroanterior, TAC de pulmón simple y prueba funcional respiratoria. El diagnóstico inicial fue neumonía adquirida en la comunidad, bulla enfisematosa infectada y enfermedad pulmonar obstructiva crónica. La evolución clínica y radiológica del paciente bajo tratamiento con antibióticos confirmó la sospecha diagnóstica (AU)
It is presented clinical, radiological, tomographic, and spirometric data of a 49-year-old inveterate smoker, who attended the consultation of the on-duty service for presenting fever of 38,5°C, anorexia, weight loss and coughing. It was performed tests in which it was included hemogram with differential, erythrocyte sedimentation rate, sputum AFB, bacteriological and cytological sputum, blood culture, chest X-ray, single-lung CAT and pulmonary function test. The initial diagnosis was community-acquired pneumonia, infected emphysematous bulla and chronic obstructive pulmonary disease. The clinical and radiological evolution of the patient on antibiotics confirmed the suspected diagnosis (AU)
