ABSTRACT

The article presents results of treatment of 53 patients with acute thrombosis of profound veins of various localizations by means of a new home thrombolytic drug, physiologic activator plasminogen--renokinase. High effectiveness of thrombolytic therapy with good rehabilitation or marked clinical improvement was obtained in 79% of the patients. Renokinase is areactogenic and may be used many times.

ABSTRACT

Electron microscopy of the venous valves in varicose disease shows that dystrophic changes develop on the external and internal surfaces of the valve cusps. Endotheliocytes on the external surface are located longitudinally in relation to valvular axis and on the internal surface they are perpendicular to it. Fragments of the elastic membrane, collagen fibres and smooth muscle cells are seen under the endothelial lining. Endothelium actively regulates proliferative processes of smooth cells secreting the so-called "growth factor of endothelial origin". The development of venous valve pathological changes in varicose disease is accompanied by endothelial damage, disturbance of morphofunctional state of smooth muscle cells and intercellular substance of the vascular wall.

ABSTRACT

The author describes his experience with studying disturbances of the venous blood flow in the lower extremity veins and extravasal correction of the valves in 1500 operations on profound veins, their free transplantation, reconstructive operations and analysis of literature. Three types of the profound vein valve function were established.

ABSTRACT

Under observation there were 60 patients operated upon for varicose dilatation of the lower extremity veins. It was established that the reconstruction of the walls of the altered vessels had a substantial influence on processes of the parietal microthromboformation and facilitated local alteration of hemodynamics and elevation of the aggregation properties of thrombocytes.

ABSTRACT

The method of occlusive plethysmography giving an exact quantitative assessment of resistance of the venous bed was used for the examination of 91 patients with different degrees of alterations of the venous outflow before and after surgery. An analysis of the data obtained has shown the efficiency of the method as allowing to supplement and sometimes to substitute invasive methods of examination especially for the assessment of the venous outflow in profound veins and of effects of the surgical treatment.

ABSTRACT

The article presents an experience with the operative treatment of 476 patients with recurrent dilatation of superficial veins of lower extremities. True and false variants of the recurrences are discussed as well as problems of their diagnosis and treatment. Stress is made on high specific weight (87%) of patients with the development of recurrences due to incomplete examinations and technical defects during the first operation. In 13% of the patients recurrences were caused by progress of the varicose disease and non-eliminated hypertension in profound veins due to their ectasia and incompetence of the valves.

ABSTRACT

The work analyzes the experience of 210 operations for extravasal correction of incompetent valves of profound veins with framework elastic spirals made of lavsan and tantalum in the complex treatment of the varicose disease of lower extremities. The technique of the operation for extravasal correction of incompetent valves with the framework spirals is described. A complete elimination of pathologic reflux of blood through the corrected valve was observed in 56 patients. The framework spirals of lavsan and tantalum enabled avoiding thromboses of veins and scarry deformity of valves.

ABSTRACT

An analysis of late results of surgery for thrombophlebitic affection is reported. 55 patients were examined in different terms (up to 7 years after reconstructive operative procedures). Favourable and satisfactory issues were gained in 44 of them Control phlebograms proved the patency of anastomosed and free grafted veins. The following operations are recommended for a wider clinical application: croos shunting, the femoral vein plasty with a free venous autograft, the construction of anastomoses between v. saphena magna and profound veins of the extremity. The need for reconstructive procedures that would be associated with the removal of pathologically changed subcutaneous veins and ligation of insufficient communicant veins of the leg and foot is emphasized.
