ABSTRACT

The article describes the models of surgical care designed and tested at Valle University Hospital in Cali, Colombia, for the provision of more coverage of patients without detriment to the quality of their care. The program was based on an analysis of preliminary studies done on operations performed in the department of Valle del Cauca. The surgical problems were analyzed by the systems approach, and taking account of the physical, educational and administrative resources and the evaluation of those components. Finally, prototype models have been designed that have been successfully used to demonstrate the feasibility of increasing the coverage of patients by 80% and attaining 90% satisfaction of the patients themselves. Later, one of the models was put into practice at San Cristóbal, Venezuela, with similar results.

ABSTRACT

El articulo describe los modelos de atencion quirurgica que se disenaron y pusieron a prueba en el Hospital Universitario del Valle, Cali (Colombia), con el objeto de aumentar la cobertura de pacientes sin que disminuyera la calidad de la atencion que se les presta. El programa se implanto con base en el analisis de los estudios preliminares realizados al respecto de las operaciones que se realizaban en el Valle del Cauca. Los problemas quirurgicos se analizaron utilizando el enfoque de sistemas y se tuvieron en cuenta los recursos fisicos, educativos, administrativos y la evaluacion de esos componentes. Se diseno por fin modelos prototipo con los que fue posible demostrar la factibilidad de aumentar la cobertura de los pacientes en un 80% y lograr un 90% de satisfaccion de los propios pacientes

The article concerns a program launched by a multidisciplinary group at del Valle University, at Cali, Colombia, to improve surgical services and extend their coverage, and to improve the care of the patients. On the basis of a study of the operations most frequently performed in Valle del Cauca department, a project has been designed for the training of surgeons in the University's area of influence. The project has a care component as well. It is now being implemented in a hospital of the region, where the new system makes it possible to conduct two operations at the same time in the same operating room, instruct patients and the families on post-operative care in the home, and evaluate the results of the arrangement. It is hoped that this exercise will make it possible to attend to more patients requiring surgery without need of more resources than those already available.

El articulo se refiere a un programa puesto en marcha por un grupo multidisciplinario de la Universidad del Valle (Cali, Colombia) para mejorar los servicios quirurgicos, ampliar su cobertura y prestar mejor atencion a los pacientes. Con base en un estudio realizado sobre las operaciones mas frecuentes en el Valle del Cauca, se diseno un proyecto para la capacitacion del cirujano en el area de influencia de la Universidad. El proyecto tiene tambien un componente de asistencia.Actualmente se esta llevando a cabo en un hospital de la region en el que es posible con el nuevo sistema realizar dos operaciones simultaneamente en un solo quirofano, instruir a pacientes y familias para realizar la atencion en el hogar posteriormiente a la operacion y evaluar los resultados alcanzados con estas experiencias.Se espera que con este programa sera posible aumentar la cobertura de los enfermos que requieren intervenciones quirurgicas sin necesidad de emplear mas recursos que los ya disponibles

ABSTRACT

This paper presents a cost analysis for herniorrhaphies as they are currently performed, as compared with a new plan. The current system utilizes hospital postoperative care. The new plan involves a different utilization of hospital facilities, personnel, drugs, supplies, and postoperative home care. Costs are considerably reduced under this new proposal. Projections reveal that implementation of the new plan with the same level of present expenditures could eliminate 75 percent of the present backlog of hernias over a 10 year time period. Although the cost and disease estimates derive from an experimental study conducted at the Universidad del Valle, Cali, Colombia, this model can help meet population health care needs in any country or geographic region by basing the system of services and facilities on epidemiologically derived disease prevalence and incidence rates.
