Fallopian tube tumour presenting as supraclavicular lymph node metastasis.

A 72-year-old woman noted a lump in her left supraclavicular fossa with no other symptoms or other signs on physical exam. A cervical biopsy indicated metastatic carcinoma. On the diagnostic workup: thoracic-abdominal-pelvic CT revealed augmented lymph nodes (LNs) in the retroperitoneum; Positron Emission Tomography-CT showed uptake in the LNs described and in two small areas in the pelvis; blood tests showed elevated CA125 and CA72.4. Another biopsy, considering a para-aortic LN, proved to be metastasis of a high-grade serous carcinoma (HGSC). Gynaecological exam and pelvic imaging were innocent. Diagnostic laparoscopy, including hysterectomy and bilateral adnexectomy, was performed and anatomopathological examination confirmed a HGSC in the fallopian tube (FT), in a tiny focus of 1.5 mm. The patient continued treatment with adjuvant chemotherapy. Literature review indicates that supraclavicular LN as first manifestation of FT carcinoma is not usual, and widespread lymphadenopathies with no macroscopic pelvic disease at diagnosis are even rarer.

The topic of this article is the relationship between the terms discourse and psychosis in Lacanian theory. Contrary to the current affirmation that in the Jacques Lacan theory, psychosis and discourse are excluding terms, the author explores excerpts of Lacan's legacy, demonstrating that the bases on which the commentators hinge their work to sustain this affirmation are not so evident.

Este artigo aborda as condições epistemológicas do discurso sobre o inconsciente utilizando autores da epistemologia e da psicanálise. Indicamos a emergência e a trajetória do discurso científico para discutirmos as relações entre a psicanálise e a ciência. Perguntamonos se a episteme que Descartes inaugura pode ser a mesma em que se insere a psicanálise, se esta estaria subordinada às coordenadas da ciência moderna. Concluímos sustentando que as condições epistemológicas do discurso sobre o inconsciente devem ser consideradas outras que a da ciência moderna, reconhecidas como pertencentes a um modo de produção de conhecimento que se distingue do da ciência moderna por incluir o real em seu campo operatório, e esse modo de produção é o que caracteriza a ciência contemporânea(AU)

In order to address the epistemological conditions of unconscious discourse, we consider authors of the referred fields. We begin using the history of the emergency and evolution of the scientific discourse and later move to discussing the relationship between psychoanalysis and science. If Descartes's episteme could be said to be that of psychoanalysis it means that the later could be thought in the coordinates of modern science. We concluded the work maintaining the idea that the epistemological conditions of the discourse on the unconscious should be considered others than those of modern science; i.e. recognized as belonging to anew mode of production of knowledge, as it includes the Real in its field of operation. This mode of production is what characterizes contemporary science(AU)

O Laço social próprio à psicose

A teoria que fundamenta essa tese é a psicanalítica. O tema abordado é a especificidade da psicose. Nela, o autor percorre os textos de Freud, Lacan e comentadores que contemplam o tema, buscando evidenciar o que consiste a especificidade desse quadro clínico.
