ABSTRACT

We present a selected group of patients (18) who underwent excision of various malignant skin lesions in the leg, and had the defect resurfaced with V-Y advancement flaps. The mobility and reliability of this type of flap was enhanced by raising it as a fasciocutaneous flap based on one or two leg perforators. As patients were mobilised as soon as they recovered from the operation, there was minimal postoperative morbidity. This type of flap has the added advantage of leaving no significant donor defect and therefore better cosmesis.

ABSTRACT

The potential benefits of harvesting the rectus abdominis muscle endoscopically are great. To date no simple, reliable method has been developed for harvest based on the inferior epigastric artery and transferred to the living patient. We have devised a simplified endoscopically assisted method of harvest on a cadaveric model that adds speed and the advantage of a grossly intact sheath to other published methods. It has been used successfully in a clinical situation. Two-portal access and triplanar dissection are the key points in this method.

ABSTRACT

We present our experience with the use of the posterior arm fasciocutaneous flap in the reconstruction of 22 axillae in 17 patients. These patients all suffered from long-standing, severe hidradenitis suppurativa and this flap was used to reconstruct the axilla after surgical excision of the whole hair-bearing area of the axilla and the underlying axillary disease. In most cases, excision included all of the axillary contents up to the axillary vessels but excluded the apex of the axilla. Nineteen axillae were reconstructed with a pedicled flap. Three axillae were reconstructed more recently with an island flap. All reconstructions were achieved without flap complications.
