ABSTRACT

Mucormycosis is a fungal infection caused by a rare pathogen most commonly affecting immunocompromised hosts. Successful treatment of pulmonary mucormycosis requires rapid diagnosis, reversal of predisposing factors, aggressive surgical excision, and antifungal therapy. We present a case of pulmonary mucormycosis affecting a young neutropenic male requiring a pneumonectomy.

ABSTRACT

Primary myoepithelial carcinoma of the lung is a rare neoplasm with only 8 cases reported in the English literature to date. Myoepithelial carcinomas of the lung are thought to arise from submucosal bronchial glands and have morphologic features similar to their salivary gland counterparts. The pathologic features and immunohistochemical profile of this tumor have not yet been summarized in the literature. Our objective is to review the clinicopathologic features and immunohistochemistry of these tumors.

ABSTRACT

BACKGROUND: Insulin-like growth factor 1 (IGF-I), IGF binding proteins (IGFBP) 1 to 7, and C-peptide have been postulated to predict survival in non-small cell lung cancer (NSCLC). Studying serum levels in NSCLC patients treated with surgical resection may provide information on the aggressiveness of tumors and be predictive of disease recurrence. METHODS: Immunobead assays were used to measure pretreatment serum levels of IGF-I, IGFBP1 to IGFBP7, and C-peptide in 100 NSCLC patients. Of these, 59 had no metastatic progression (T1 to T4 N0 M0), whereas 41 had positive lymph nodes (T1 to T4 N1 to N3 M0). Data were analyzed using the Mann-Whitney two-sided rank sum test or Kaplan-Meier curves. RESULTS: Low serum IGFBP5 levels correlated strongly with a positive nodal status (p < 0.001) and any incidence of disease recurrence (p = 0.003). Low serum levels of IGFBP5 also predicted poor recurrence-free survivals in the overall cohort (p ≤ 0.001) and in patients with no nodal metastases (p = 0.027). Conversely, a high serum level of IGFBP7 correlated with positive nodal status (p = 0.008), but was not prognostic for recurrence-free survival. No significant correlations were found for IGFBP5 or IGFBP7 for sex, age, race, smoking history, tumor histology, or fasting state. CONCLUSIONS: IGFBP5 and IGFBP7 had value as biomarkers for identifying NSCLC progression and patient outcome.

BACKGROUND: Orthogonal and parallel plate constructs are used for fixation of intra-articular distal humerus fractures but optimal plate configuration remains controversial. The purpose of this study was to compare the biomechanical properties of orthogonal versus parallel plate constructs in a cadaver distal humerus fracture model. MATERIAL AND METHODS: An intra-articular distal humerus fracture with a metaphyseal defect was created in 14 matched pairs of cadaver elbows. Paired specimens were fixed with either orthogonal or parallel plates from a single elbow plating system using nonlocking screws. Using a novel testing protocol, loading was applied to the forearm and was transmitted to the distal humerus through intact collateral ligaments, olecranon, and radial head. Seven matched pairs were tested under varus loading and seven under axial/sagittal loading. Each specimen underwent cyclic loading first, followed by loading to failure. RESULTS: Parallel plate constructs had significantly higher stiffness than orthogonal ones during cyclic varus loading (P = .002). Screw loosening occurred in all posterior plates of orthogonal constructs but in no plates of parallel constructs (P = .001). Parallel constructs had significantly higher ultimate torque in varus loading to failure (20.7 vs 15.9 Nm, P = .008), and higher ultimate load in axial/sagittal loading to failure (1287.8 vs 800.0 N, P = .03). DISCUSSION: Parallel plating of intra-articular distal humerus fractures with a metaphyseal defect demonstrates superior biomechanical properties compared to orthogonal plating, and may be preferable for fixation of these fractures.

ABSTRACT

INTRODUCTION: Primary leiomyoma of the lung is a rare benign tumor that usually presents as a solitary lesion predominantly in young females. Fewer than 100 cases have been reported. Common symptoms include fever, chronic cough, hemoptysis, chest pain, shortness of breath, and pneumonias. CASE PRESENTATION: A 34-year-old, non-smoker female who presented with recurrent pneumonias. She was found to have a primary leiomyoma of the right middle lobe. This was treated by right middle lobectomy. 6-month follow up showed patient doing well without evidence of residual disease on computerized tomography. CONCLUSION: Primary pulmonary leiomyoma is a rare tumor distinct from benign metastasizing leiomyoma. Histologic features include absence of mitotic count, low cellularity, lack of cytologic atypia and pleomorphism. Treatment is by conservative surgical resection and carries a favorable prognosis.

ABSTRACT

New antibiotics have been developed targeting resistant microorganisms; however, limited information is available on their use in implant-related chronic osteomyelitis. We evaluated the infection control rate of linezolid in treating these challenging infections and delineate indications for its use. We retrospectively reviewed 22 consecutive adult patients with chronic implant-related osteomyelitis, treated with linezolid in addition to surgical débridement and implant removal. Osteomyelitis was associated with fracture fixation implants (n = 18) or arthroplasty implants (n = 4). Methicillin-resistant Staphylococcus aureus (MRSA) was the most common pathogen (10 of 22 patients). Fourteen patients had one or more comorbidity, including intravenous drug abuse in four patients. Indications for linezolid use included preference for oral administration in 13 patients, presence of vancomycin-resistant enterococcus (VRE) in five patients, and development of allergic reactions to vancomycin in four patients. Fourteen patients were followed for a minimum of 6 months (mean, 22 months; range, 6-34 months) with no recurrence of infection. Linezolid is a reasonable alternative for treating chronic implant-related osteomyelitis. Our treatment protocol, including linezolid, achieved control of infection in all patients despite the challenging nature of these infections due to patient comorbidities and resistant organisms.

ABSTRACT

TrkB receptors mediate the effects of BDNF on striatal medium spiny neurons and mesencephalic dopamine neurons. The effect of partial BDNF gene deletion on locomotor activity and the gene expression of these neurons was evaluated at 3, 12, and 24 months of age in BDNF heterozygous (BDNF(LacZ/neo+)) and wildtype mice. BDNF(LacZ/neo+) mice displayed less spontaneous horizontal activity than wildtypes at 3 and 24 months of age. Whereas striatal preproenkephalin and preprodynorphin mRNA and mesencephalic tyrosine hydroxylase mRNA levels were significantly lower at all ages in BDNF(LacZ/neo+) mice, GAD67 mRNA was only lower at 24 months. In contrast, BDNF(LacZ/neo+) mice expressed more trkB mRNA in the striatum at 3 months and less at 24 months of age than wildtypes. Total striatal cell number in the two genotypes was not different at 12 months of age, whereas Golgi staining revealed that the spine density on distal dendrites of medium spiny neurons was less in BDNF(LacZ/neo+) mice than in wildtypes at 24 months of age. These data indicate that endogenous BDNF is required to maintain the normal phenotype and functioning of striatal projection neurons and mesencephalic dopamine neurons and that exaggerated dysfunction of these neurons and a concomitant decline in locomotor behavior occurs during aging.
