ABSTRACT

The study of 500 blood donors, of 23 prostitutes, 22 confirmed AIDS cases and 19 ARC patients in Abidjan showed that HIV-2 prevalence in blood donors (3.4%) was somewhat higher than that of HIV-1 (2.4%). Furthermore HIV-2 alone was found associated with 14% of AIDS cases and 21% of ARC, while HIV-1 was associated with 32% of the cases. Antibodies to HIV-1 and HIV-2 were found in 54% of AIDS and ARC cases, as well as in 48% of the prostitutes sera. These preliminary results suggest that HIV-2 is pathogenic, being causally involved in some AIDS cases and does not protect from HIV-1 pathogenic potential.

ABSTRACT

The authors report 16 cases of cytomegalovirus (CMV) disease in previously healthy adults. Constant features included pyrexia lasting 3 to 8 weeks and mononucleosis occurring 2-3 weeks after the onset of fever. Moderate hepatomegaly without jaundice, splenomegaly and morbilliform or petechial rush were observed in 30 to 50 p. 100 of cases. None had pharyngitis. Mild increase in serum transaminase activity (2 to 5 N) was present in 13 of the 16 patients, but increased alkaline phosphatase activity was observed in only 3 of them. Liver biopsy was obtained in 10 patients. Liver lesions were characterised by the association of intra lobular granuloma, abundant mononuclear cells in the sinusoids and hepatic peri-venous inflammation but hepatocellular necrosis was not prominent. Typical intracellular inclusions were not seen, either in hepatocytes or in cells of biliary ducts. The diagnosis was ascertained by positive viremia and or viruria and presence of IgM antibodies. The outcome was favourable although clinical and biological signs lasted for about 8 weeks. The authors conclude that adults with chronic pyrexia, no pharyngitis and sub-clinical hepatitis with mild increases in transaminase activity and histologic mononucleosis hepatitis showing mononuclear infiltrates and granuloma formation are likely to have CMV disease.

ABSTRACT

Fever was a constant feature in 28 subjects with spontaneous mononucleosis due to cytomegalovirus and lasted 15 days or more in almost all cases (26/28). Blood mononucleosis and biochemical hepatic disorders were virtually always present but appeared later, so that the fever was initially thought to be "isolated". Splenomegaly was found in 39.3 p. cent of the cases. Skin rashes developed in 5 patients who had received ampicillin. Non-specific immunological abnormalities were detected in 8 of the 13 patients in whom they were looked for. The diagnosis was established by serological tests, which showed either a significant rise in, or (in patients seen at a late stage of the disease) high, sustained antibodies titers. The virus was found in blood in seven patients among the fourteen in whom it was looked for. We believe that these findings in otherwise healthy subjects could be termed "primary infection". The condition invariably followed a favourable course in the absence of any treatment. Contrary to what is observed in infectious mononucleosis, there was no sore throat and the Paul-Bunnel Davidsohn test always gave negative results.
