ABSTRACT
Results of the use of classic Indian and petalous frontal flaps for surgical treatment of 52 patients of the age from 11 till 58 years with congenital and acquired eyelid defects were presented. In all cases eyelid form and function it was possible to restore, reduce the number of operations and achieve maximal esthetic rehabilitation in the terms to 1 year from the patient addressing to clinic and in patients with congenital oblique face clefts - before achieving the age of 14-16 years. Frontal flap use on medial, paramedical and lateral pedicles considerably widened reconstructive blepharoplastics possibilities and increased efficacy of treatment of the patients with congenital and acquired limited total and subtotal eyelid defects.

The Expert Commission of the Institute of Plastic Surgery and Cosmetology has undertaken comparative analysis of complaints over poor quality of the surgical treatment for the correction of nasal deformities from the patients operated during the periods of 2001-2003 and 2004-2006. The analysis has demonstrated that the frequency of undesirable outcomes of rhinoplastic interventions remains rather high. The majority of the aesthetically unacceptable postoperative deformations are usually due to technical errors during surgery. It is concluded that systematic analysis of the causes leading to professional mistakes and unfavourable outcomes of the surgical treatment is needed to collect data necessary for the development of measures for their prevention.

The results of the application of argon coagulator "Forse-40-GSU-System" in reconstruction of abdominal wall for ventral hernias are presented. The use of this coagulator provides decrease of intraoperative blood loss volume, an achievement of good results during the preparation of a skin flap for autodermoplasty.

J. Raven's (1992) classification was used in the treatment of patients with multiple fractures of the face associated with contusions, brain concussions, and injuries to the skull vault and base. According to this classification, all injuries of the cerebral and facial skull are classified as types I, II, and subtype Ia. Use of this classification became possible due to introduction of computer-aided tomography. A clinical example of a possible surgical correction of the orbit in a patient with type II fracture is presented.

The techniques of correction of the nose tip and upper lip deformations after the bilateral clefts plastic analysed, their shortcomings were related to the insufficient restitution of the nasal passages lining. The author has put forward a cheilorhinoplastic technique providing the more complete usage of the tissue overgrowth from the upper lip in transposition of the "split" flap consisting of the nasal membranous septum and cicatrized tissues of the lip. The technical realization of the method provided a basis for planning the reconstructive cheilorhinoplasty in any degree of deformation. Reconstructive cheilorhinoplasty is performed with due consideration of the local upper lip tissue availability using the local plastic techniques including the Abbe flap transposition.

The author describes a method of reparative operation intended for bringing down the prolabium as a skin-muscle flap on two feeding vascularized pedicles with simultaneous plasty of the defect of the lining of the intermaxilla with a flap of the mucosa from the prolabium. The posterior wall of the upper lip and entrance to the mouth were formed by shifting the lateral fragments of the lip to the center. The red border defect was also eliminated at the expense of the same tissues. In order to avoid complications it is necessary not to make thinner the skin of the nose septum which was used for the elongation of prolabium and to suture the edges of the orbicular muscle of the mouth with tension. The method was used in 438 patients and gave good results.

The deformity of the upper lip of a congenital and acquired character is often accompanied by an alteration of the Cupid arch contours. The author describes the method based upon creation of duplicating of the orbicular muscle of the mouth which allows to make a more raised and protrusive Cupid arch. Good results were obtained in 18 patients, their follow-up observation being 4 years.

The author describes the method of operation for the elimination of the recurrent flat wing and tip of the nose after reconstructive chilorhinoplasty in unilateral cleft lips. The method of chilorhinoplasty was used in more than 500 patients. The symmetry of the nose tip was achieved in 85% of the cases which shows the prospects of introduction of the method into wide clinical practice.

The author proposes a classification of deformities of the upper lip and nose after plasty of congenital clefts. Preliminary results of the treatment of 250 patients with the deformities of the upper lip and nose after plasty of the uni- and bilateral clefts have confirmed the positive estimation of the classification and prospects of possible use of the principles of planning reconstructive cheilorhinoplasty in clinical practice.
