ABSTRACT

Reports of SARS-CoV-2 reinfections are on the rise. This study focused on reinfections in patients with confirmed COVID-19 in the Czech Republic. Between 1 March 2020 and 9 November 2020, 362 084 cases with the onset of symptoms before 31 October 2020 were reported. Overall, 28 cases of symptomatic SARS-CoV-2 reinfections were identified, 11 in males and 17 in females, age range 25-80 years, median age 46 years. The interval between the first and second episodes of the disease ranged from 101 to 231 days, and the median interval was 201.5 days. During both symptomatic episodes, all patients have been tested by RT-PCR. Altogether 26 patients (92.9%) have been tested negative after recovery from the first episode of COVID-19. Symptomatic reinfections occurred in nearly 0.2% of all patients at risk. Most patients with reinfection had mild symptoms in both episodes, and only three episodes were moderate to severe. Thus, reinfections may have been underdiagnosed. In summary, COVID-19 reinfections are possible and not exceptional.

ABSTRACT

BACKGROUND: The monitoring of the late effects of childhood cancer treatments was established approximately in the 1970s. With an increasing number of children cancer survivors the identification of the short-term or late effects becomes more detailed. The psychosocial and cognitive problems are of the most frequent sequelae of the cancer treatment and their prevalence is nearly 20% in survivors of childhood cancer. These problems can have an adverse impact on further professional career or private life in the childhood cancer survivors. The most threatened group of patients are children treated for brain tumors and acute lymphoblastic leukemia. DESIGN: The object of this paper is to review the present information in the area of the neuropsychological sequelae in the childhood cancer survivors. CONCLUSIONS: Identification of the specific cognitive problems in childhood cancer survivors can have the profound impact on improvement of the support delivered to the children and adolescents by their families, in the school and further career. Tailored interventions can have the positive impact on the quality of life of this subgroup of children. Multidisciplinary approach including routine psychological screening is necessary for addressed follow-up care concerning this vulnerable at-risk population.

ABSTRACT

The authors present a case report of a patient, in whom after a head injury the monolateral blindness occurred. Because of autoimmune thrombocytopeny the patient was treated with long-term corticosteroids. The clinical findings corresponded with the orbital apex syndrome. According to the results of the CT and MRI examinations, the sphenoidotomy was indicated, and the histological findings verified fragments of paranasal sinuses' aspergiloma. During the next course of the disease, despite antimycotic therapy, the progression of the aspergiloma in to the anterior cranial fossa occurred. Invasive sino-orbital aspergilosis, after the penetration of the infectious agent across the wall of the sinus, may cause the orbital apex syndrome with paralysis of all three cranial nerves innervating the extraocular muscles, sensoric defect in the area of the ophthalmic nerve and the involvement of the optic nerve.
