ABSTRACT

The current status of first aid in the Russian Federation is covered. The necessity and the basic principles o first aid in the country are formulated, the need for the creation of which is due to the involvement of first aid to victims wide range of people, including and without health education. The necessity of developing and adopting the number of regulations to administrate various aspects of first aid, changes in existing regulations, as well as the establishment and functioning of the interagency coordinating council on first aid. The principles of operation of the system of training of participants in first aid, equipping them with the means to administer first aid are suggested.

ABSTRACT

The 4-years experience of medical supporting of a wide-ranging contra-terrorist action (2000-2003) has conclusively demonstrated an exact correlation between intensity of battle actions and scales of sanitarian losses, between character of battle actions and structure of sanitarian losses, between steps, passed by wounded and results of treatment. In offensive operations dominated single gunshot wounds. In these conditions was marked a high frequency of chest case, of hurts of venter and hip joint. In conditions, when territory is already occupied and the opponent began a "landmine" war,dominate fragment and multiple wounds. In the same phase of war frequency of conjoined and combinated hurts rise, the same as hurts of legs. In conditions of battle actions in structure of viscerogenic pathology the first place is occupied by diseases of respiratory system, of blood circulatory system and urogenital system.

ABSTRACT

Medical troop of single-purpose (MTSP) and it's capabilities are examined in different variants of it's using in conditions of providing of immense antiterrorist acts. Setting tasks, opportune and corresponding operational environment, let use the MTSP all-out in 3 variants: classifying-evacuation hospital, field mobile hospital and medical batch for reinforcement Separated Medical Battalion. High effectiveness of the MTSP's activity is reached in consequence of it's reinforcement by qualified personnel from MMCH of Internal Troops of MIA, of wide using of aerotransport with medical equipment and using of reanimation brigade on the board, of uninterrupted providing by the blood and it's constituents. Locomotivity and efficiency of work were provided by the presence of modern well-equipped modules on wheels.

ABSTRACT

Results of treatment of 71 patients with gun-shot wounds of the pancreas were analyzed. Features and structure of gun-shot abdominal penetrating wounds with injury of the pancreas, potential of up-to-date diagnostic methods were studied. Surgical policy in combined injuries of hollow organs and pancreas was developed. Variants and sequence of surgeries were determined depending on location and types of pancreatic injuries. Proposed treatment and diagnostic policy permitted to reduce rate of purulent and septic complications by 11.1% and lethality by 5.3%.

ABSTRACT

Based on experience of treatment of 177 wounded with gunshot combined wounds of the pelvis, rational surgical policy in these wounds is substantiated. It is necessary to maintain sequence of each stage of the operation bearing in mind trajectory of shell in organs and tissues of the abdomen and pelvis, simultaneous wounds of organs which are significantly different by specific anatomic structure and physical role.

ABSTRACT

The experience of treatment of 73 wounded with gunshot injuries of the rectum is reviewed. Classification of gunshot injuries of the rectum is proposed which permits to choose rational surgical policy depending on the location of the rectal injury is proposed. Terms of closing colostomas in remote periods after operation were determined. The authors believe that because of the complicated anatomo-topographic characteristics of the rectum and frequency of the combined injuries it is advisable to perform operations bor gunshot wounds of the rectum as an urgent specialized aid.
