ABSTRACT

We report on a 58-year-old man, in whom a clinical suspicion of orchitis of the right testicle was treated with antibiotics. The instored antibiotics failed to improve the clinical condition of orchitis. Previous medical history revealed a gastric lymphoma for which gastrectomy was performed, followed by combination chemotherapy. Therefore testicular biopsies were performed. These biopsies confirmed the diagnosis of testicular localisation of lymphoma. A combination chemotherapy, in association with alpha-Interferon was instaured. One year after instauration of combination therapy, the patient is in complete remission. This unusual localisation of lymphoma is discussed in regard of other cases described in literature.

ABSTRACT

6 patients presenting with uric acid renal obstruction were managed by alkalization of urine using intravenous perfusion of 0.16 (1/6) M sodium lactate. Despite obstructive renal failure in 3 patients, rapid relief of obstruction was obtained and surgery was not necessary in any patient. Lysis of uric acid stones by urine alkalization is well documented [1, 4-12]. Intravenous infusion of 0.16 (1/6) M sodium lactate provides rapid urinary alkalization and is effective in the relief of obstruction by dissolution of uric acid calculi.

ABSTRACT

Carcinoid tumors are rare and mainly found in the G.I. tractus but also in the tracheobronchial tree and ovaries. The testicular localization, primary or metastatic, is rare. The authors report a primary carcinoid tumor of the testis in a 35 years old male. The tumor characteristics are well enough known from the literature. Primary carcinoid tumors of the testis appear to be of a low grade malignancy. Local therapy suffices for care of the patient as long as, on microscopic examination, it is not an expression of a teratocarcinoma. The diagnosis is made by the discovery of a tumor since the carcinoid syndrome is seldom in the clinical picture (only 2 cases in the literature, one of whom with generalized metastasis).

ABSTRACT

The authors report 7 cases of isolated injuries of the gallbladder, in closed abdominal trauma. The literature indicates the frequency to lie between one and five percent. The postprandial period and fasting alcohol levels, responsible for the distention of the gallbladder, are mostly the cause of such lesions. Four groups of injuries are described: 1) laceration 2) avulsion 3) simple contusion 4) traumatic cholecystitis Early diagnosis of such injuries is difficult as abdominal signs are poor and little specific; the authors note the value of peritoneal puncture and lavage. The treatment of gallbladder trauma is always surgical, cholecystectomy giving the best results; raphy and cholecystostomy are second choice treatments. The prognosis is usually favorable, even if surgery is delayed, and depends essentially on the nature of associated extra-abdominal injuries.
