ABSTRACT

How do military alliances affect public support for defending targets of aggression? We studied this question by fielding an experiment on 14,000 voters in 13 member countries of the North Atlantic Treaty Organization (NATO). Our experiment involved a hypothetical scenario in which Russia attacked a target country. We randomly varied the identity of the target (Bosnia, Finland, Georgia, or Sweden), and whether the target was a member of NATO at the time of the attack. We found that voters in every member country were far more willing to use military force to defend each target when the target was in NATO, than when the target was outside the alliance. The expansion of NATO could, therefore, transform European security by altering the likelihood and scale of future wars. We also uncovered important heterogeneity across targets: the benefits of joining NATO were considerably larger for Bosnia and Georgia than for Finland and Sweden, since most voters in NATO countries would defend Finland and Sweden even if they remained outside the alliance. Finally, the effect of NATO was much stronger among voters who perceived NATO as valuable for their own country. Rhetorical attacks on NATO could, therefore, undermine the alliance by eroding the public's willingness to defend other members, whereas rhetoric highlighting the benefits of NATO could bolster defense and deterrence. These findings advance knowledge about the effects of alliances, while also informing policy debates about the value and size of NATO.

ABSTRACT

Electrocardiographic (ECG) artifacts may resemble ventricular tachycardia (VT), leading to inappropriate therapies. Despite extensive training, electrophysiologists have still been shown to misinterpret artifacts. The literature is scant regarding the intraoperative identification by anesthesia providers of ECG artifacts resembling VT. We present two cases of the intraoperative occurrence of ECG artifacts resembling VT. The first case involved a patient undergoing extremity surgery after receiving a peripheral nerve block. The patient was treated with a lipid emulsion for a presumptive local anesthetic systemic toxicity diagnosis. The second case was a patient with an implantable cardiac defibrillator (ICD) with suspended anti-tachycardia functionality due to the location of the surgery in the region of the ICD generator. The second case's ECG was identified as an artifact, and no treatment was initiated. Misinterpretation of intraoperative ECG artifacts continues to lead clinicians to institute unnecessary therapies. Our first case occurred in the context of a peripheral nerve block leading to the misdiagnosis of local anesthetic toxicity. The second case occurred during the physical manipulation of the patient during liposuction.

ABSTRACT

Patients with amyotrophic lateral sclerosis (ALS) who undergo lower extremity joint arthroplasty are rarely encountered. Patients with ALS are at an increased risk for perioperative anesthetic complications. Anesthetic techniques, regional or general, present different risks to patients with ALS. The historical concern of worsening pre-existing neurologic symptoms with regional anesthesia is being re-examined in light of emerging evidence supporting its use in patients with ALS. Here, we present the successful perioperative management of a patient with severe bulbar ALS undergoing total knee arthroplasty. Despite his advanced bulbar symptoms, he was independently ambulatory with severe knee pain related to osteoarthritis. During multidisciplinary planning with the patient and his wife, it became clear that his primary perioperative concern was avoiding intubation, prolonged ventilation, and tracheostomy placement. With this in mind, we planned for a neuraxial anesthetic without intraoperative sedation, a postoperative adductor canal peripheral nerve block, and multimodal non-opioid analgesia. There were no perioperative complications. At the six-week follow-up, he experienced improved ambulation and showed no signs of worsened ALS symptoms.

ABSTRACT

OBJECTIVE: To evaluate which rapid blood administration technique causes the least iatrogenic hemolysis in canine packed red blood cells (pRBCs) as determined by plasma free hemoglobin (fHb) and percent hemolysis (% hemolysis). DESIGN: Prospective in vitro randomized study. SETTING: Private referral center. ANIMALS: None. INTERVENTIONS: Thirteen units of canine pRBCs were divided equally into 5 aliquots, resulting in 65 trials. The aliquots of each unit were subjected to the following administration techniques: gravity-driven (control), an infusion pump at maximal rate, application of a pressure bag, manual compression, and syringe bolus. Plasma fHb and % hemolysis were recorded before and after each trial. Rate of administration (mL/s) was calculated for each method. MEASUREMENTS AND MAIN RESULTS: Compared to the control, there were no significant increases in % hemolysis or plasma fHb noted among any of the trial methods. The manual compression and syringe bolus methods resulted in the fastest transfusion rates, whereas the infusion pump was not faster than the gravity-driven method. Despite a storage time of ≤14 days, 15% of pRBC units had unsuitable (>0.8%) hemolysis before even being subjected to the trials. CONCLUSIONS: Commonly used rapid infusion techniques in small animal transfusion medicine do not cause significant iatrogenic hemolysis of canine pRBCs in vitro, although a significant risk is present in stored blood. This suggests that if an expedited transfusion is needed, any method described in this study could be considered, although stored pRBCs should be tested for unsuitable levels of hemolysis prior to transfusion.

ABSTRACT

Extant research consistently links youth externalizing problems and later maladaptive outcomes, and these behaviors are particularly detrimental given their relative stability across development. Although an array of risk and protective factors for externalizing problems have been identified, few studies have examined factors reflecting the multiple social-ecological levels that influence child development and used them to predict longitudinal trajectories of externalizing problems. The current study examined externalizing behavior trajectories in a sample of 1,094 at-risk youth (539 boys, 555 girls) from the Longitudinal Studies in Child Abuse and Neglect multisite longitudinal study of child maltreatment. Normed Child Behavior Checklist externalizing scores were used to estimate group trajectories via growth-based trajectory modeling at ages 10, 12, 14, and 16 using the SAS PROC TRAJ procedure. Model fit was assessed using the Bayes information criterion and the Akaike information criterion statistics. Analyses revealed optimal fit for five distinct behavioral trajectories: low stable, mid-increasing, mid-decreasing, medium high, and high stable. Multinomial logistic regressions revealed that a combination of risk and protective factors at individual, family, school, and neighborhood levels contribute to distinct trajectories of externalizing problems over time. Predictors of low and decreasing trajectories can inform interventions aimed at addressing externalizing problems among high-risk adolescents.

ABSTRACT

Continuous brachial plexus nerve block catheters are commonly inserted for postoperative analgesia after upper extremity surgery. Modifications of the insertion technique have been described to improve the safety of placing an infraclavicular brachial plexus catheter. Rarely, these catheters may become damaged or entrapped, complicating their removal. We describe a case of infraclavicular brachial plexus catheter entrapment related to differences in arm positioning during catheter placement and removal. Written authorization to obtain, use, and disclose information and images was obtained from the patient.

ABSTRACT

This case-series describes the 6 human infections with Onchocerca lupi, a parasite known to infect cats and dogs, that have been identified in the United States since 2013. Unlike cases reported outside the country, the American patients have not had subconjunctival nodules but have manifested more invasive disease (eg, spinal, orbital, and subdermal nodules). Diagnosis remains challenging in the absence of a serologic test. Treatment should be guided by what is done for Onchocerca volvulus as there are no data for O. lupi. Available evidence suggests that there may be transmission in southwestern United States, but the risk of transmission to humans is not known. Research is needed to better define the burden of disease in the United States and develop appropriately-targeted prevention strategies.
