ABSTRACT

Primary lymphoma originating from the lacrimal drainage system is a rare disease. Such lymphomas are mostly B-cell in origin and present nonspecific symptoms. The treatment of malignant lymphoma of the lacrimal drainage system is slightly different. We present the case of a 71-year-old woman with a painless mass below the medial canthus. Computed tomography (CT) scan of the orbit revealed a mass invading the right lacrimal sac. An incision biopsy was obtained, and the pathologic findings suggested a diagnosis of primary diffuse large B-cell lymphoma of the lacrimal sac. The patient was treated with chemotherapy and intrathecal methotrexate. After completing eight cycles of chemotherapy, the patient was followed up by a CT scan, which revealed nearly total resolution of an ill-defined enhancing mass. At the time of this case report writing, the patient is in complete remission at six months with no other complications.

ABSTRACT

BACKGROUND: Intranasal corticosteroid (INCS) is one of the most effective treatments for allergic rhinitis (AR). To achieve optimum efficiency while avoiding adverse effects (AEs), patients must comply with and follow the drug administration instructions. OBJECTIVE: To evaluate INCS administration techniques and steps and the association between inaccurate drug administration and AEs. METHODS: A descriptive cross-sectional study was performed in patients diagnosed with AR who had used an INCS for more than 1 month at the HRH Princess Maha Chakri Sirindhorn Medical Center, Nakhon Nayok, Thailand between September 2020 and August 2021. Patient information was collected, evaluate the accuracy of the application techniques and the steps they followed for INCS administration and the occurrence of any associated AEs. RESULTS: In total, 150 subjects diagnosed with AR met the criteria. Only 6 patients (4%) correctly performed all 12 steps of INCS administration, while 44 patients (29.33%) correctly performed the 5 recommended essential steps. AEs were found in 23 patients (15.33%), with not pointing the tip slightly outward away from the septum significantly associated with a 3.6 times higher risk of AEs in patients (odds ratio, 3.6; 95% confidence interval, 1.3-9.4; p = 0.012). CONCLUSION: Investigations into INCS use in patients with AR revealed that only 4% of patients correctly performed all the administration techniques and steps, while 30% of patients at least followed the 5 recommended essential steps perfectly. Not pointing the tip slightly outward away from the septum was found to result in the most AEs. TRIAL REGISTRATION: Thai Clinical Trials Registry: TCTR-20210718003.

ABSTRACT

INTRODUCTION: The World Health Organization has issued guidelines suggesting exclusive breastfeeding for at least six months for all infants. However, one of the main factors interfering with successful breastfeeding is ankyloglossia. Frenotomy may help improve breastfeeding, but evidence on the long-term effects on breastfeeding is lacking. Therefore, this study was conducted to evaluate the effect of frenotomy on breastfeeding and identify factors associated with receiving exclusive breastfeeding for six months. METHODS: This was a cross-sectional descriptive study conducted using retrospective chart review. Infants diagnosed with anterior ankyloglossia who underwent frenotomy at HRH Princess Maha Chakri Sirindhorn Medical Center between April 1, 2013 and March 31, 2019 were included. Data on both the mothers and the infants were collected. The length and types of feeding at 2, 4, 6, and 12 months were recorded. RESULTS: In total, 526 infants were included in the study; 322 (61.22%) were male. Their median age on the day of surgery was 2 days (minimum 1 day, maximum 30 days). In total, 7.03% did not receive breastfeeding, whereas 17.96% and 75.01% had received breastfeeding for fewer than and more than six months, respectively. The prevalence of infants exclusively breastfed decreased over time from 81.37% to 55.70%, 33.46%, and 4.38% at 2, 4, 6, and 12 months, respectively. The study did not show any independent factors of neonatal and maternal characteristics associated with being exclusively breastfed for six months. CONCLUSION: Among infants with anterior ankyloglossia who received frenotomy, 33.46% were exclusively breastfed for six months. The study did not show any independent factors of neonatal and maternal characteristics associated with being exclusively breastfed for six months.

ABSTRACT

To compare radiofrequency and electrocauterization in reducing the degree of postoperative pain in pediatric patients undergoing tonsillectomy. A double-blind, parallel-group, randomized controlled trial. Pediatric patients with indications for tonsillectomy were randomized to receive either tonsillectomy with radiofrequency or electrocautery. Also recorded were intraoperative blood loss, duration of the surgery, length of hospital stays, and complications from the surgery. Forty pediatric patients participated in the study; 20 were randomized to each group. The participants' ages ranged from 3 to 14 years old, and 65% were male. The overall postoperative pain scale score during the early postoperative period was lower in the radiofrequency group compared to the electrocautery group (mean difference - 1.37 (95%CI - 1.72, - 0.12); p < 0.001). Also, the mean pain scale scores at 6 h, 1, and 2 days were significantly lower in the radiofrequency group. Intraoperative blood loss and length of hospital stay in the radiofrequency group were also significantly lower than in the electrocautery group, whereas the duration of the surgery and complication rate were not different between the two groups. Pediatric tonsillectomy using radiofrequency ablation could reduce the severity of early postoperative pain, intraoperative blood loss, and length of hospital stay compared to tonsillectomy with electrocautery. Therefore, tonsillectomy with radiofrequency may be considered a safe and effective operative option. Trial registration Thai Clinical Trials Registry (TCTR-20210303007), 03-March-2021.

ABSTRACT

To evaluate the positive pathological findings, patient satisfaction, pain, and complications of transnasal esophagoscopy (TNE) in laryngopharyngeal reflux (LPR) patients at HRH Princess Maha Chakri Sirindhorn Medical Center (MSMC), Thailand. A descriptive cross-sectional study. Forty adult LPR patients who had failed 3-months proton-pump inhibitor therapy were recruited to undergo TNE at Otolaryngology department, MSMC from January 1, 2019 to December 31, 2019. Data including demographics, Reflux Finding Score, Reflux Symptom Index, Eating Assessment Tool (EAT-10) scores, and endoscopic findings were recorded and compared between patients who had positive pathological findings and those who had negative findings. Sixteen patients (40%) had positive findings, which led to the changes in management of their conditions. Most positive findings included multiple esophageal and gastric ulcers (4/16), gastric ulcer (3/16), esophagitis (2/16), and esophageal candidiasis (2/16). There was no statistically significant difference in the baseline data between patients in the TNE positive and the TNE negative groups. The average patient satisfaction on VAS was 8.60 ± 1.43. Most patients were in the mild pain score group (18/40). Complications were not encountered. TNE is an efficient tool for evaluating LPR in Thai patients. The procedure was proven to be safe, well tolerated, and highly satisfied.

ABSTRACT

PURPOSE: Recently, there are many new surgical methods of endoscopic thyroidectomy, which have advantages in cosmetic results while do not increase the complication rates. This study was conducted to evaluate and compare both intra- and post-operative results and quality of life between TOETVA and endoscopic thyroidectomy axillo-breast approach (ETABA). MATERIALS AND METHODS: A non-randomized, patient preference clinical trial was performed in Otorhinolaryngology, Head and Neck surgery department, HRH Princess Maha Chakri Sirindhorn Medical center. Patients who decided to go for endoscopic thyroidectomy were allocated into two groups. Demographics data, operative time, intra-operative blood loss, post-operative pain score, length of hospital stay, complications were collected. Overall satisfaction was evaluated by visual analogue scale and quality of life was evaluated by SF-36 questionnaires. RESULTS: 22 patients were totally included and equally allocated in 2 groups (11 patients per group). All of the operations were lobectomy. There was no significant difference in demographic data. The results showed that pain score at 3 days post-operation and average pain score were significantly higher in TOETVA group. Average pain score was 3.67 ± 1.11 in ETABA group and 5.15 ± 1.35 in TOETVA (p = 0.011); however, the operative time, intra-operative blood loss, length of hospital stay, complications and overall satisfaction were not significantly different. Different change in the quality of life was not detected in both groups. CONCLUSION: Transoral endoscopic thyroidectomy vestibular approach and ETABA both have comparable results and do not worsen the quality of life.
