ABSTRACT

Fibrosis, a common cause and serious outcome of organ failure that can affect any organ, is responsible for up to 45% of all deaths in various clinical settings. Both preclinical models and clinical trials investigating various organ systems have shown that fibrosis is a highly dynamic process. Although many studies have sought to gain understanding of the mechanism of fibrosis progression, their findings have been mixed. In recent years, increasing evidence indicates that neutrophil extracellular traps (NETs) are involved in many inflammatory and autoimmune disorders and participate in the regulation of fibrotic processes in various organs and systems. In this review, we summarize the current understanding of the role of NETs in fibrosis development and progression and their possibility as therapeutic targets.

ABSTRACT

Glutathione synthetase deficiency (GSSD) is an autosomal-recessive metabolic disorder caused by glutathione synthetase (GSS) gene mutations. No more than 90 cases of GSSD have been reported worldwide; thus, the spectrum of GSS mutations and the genotype-phenotype association remain unclear. Here, we present a severely affected infant carrying a compound heterozygous GSS variation, c.491G > A, and a novel variant of c.1343_1348delTACTTC. We also summarize the clinical manifestations, treatment protocol, prognosis, and genetic characteristics of previously reported GSSD cases in China. In this case study, our patient presented with tachypnea, jaundice, intractable metabolic acidosis, and hemolytic anemia. Urinary-organic acid analysis revealed elevated 5-oxoproline levels. Further, this patient showed improved outcomes owing to early diagnosis and the timely administration of vitamins C and E. Therefore, our study indicates that in clinical cases of unexplained hemolytic anemia and metabolic acidosis, GSSD should be considered. Additionally, genetic testing and antioxidant application might help identify GSSD and improve the prognosis.

ABSTRACT

An optimized QuEChERS method for the simultaneous extraction of 26 antibiotics and 19 anthelmintics in whole cow milk was established, followed by UHPLC-MS/MS analysis. Briefly, 20 mL acetonitrile with 1 g disodium hydrogen citrate, 2 g sodium citrate, 4 g anhydrous MgSO4, and 1 g sodium chloride were added to 10 g milk for target chemical extraction, followed by 50 mg anhydrous MgSO4 for purification. Satisfactory recoveries were obtained using the modified QuEChERS method, with recoveries of the antibiotics ranging from 79.7 to 117.2%, with the exception of norfloxacin, which was at 53.4%, while those for anthelmintics were in the range of 73.1-105.1%. The optimized QuEChERS method presented good precision, with relative standard deviations ranging from 7.2 to 18.6% for both antibiotics and anthelmintics. The method was successfully applied to analyze the antibiotics and anthelmintics in 56 whole cow milk samples from China. Briefly, the detection frequencies and concentrations of most of the antibiotics and anthelmintics were low in the whole cow milk samples, with concentrations ranging from below LOD to 4296.8 ng/kg. Fenbendazole, febantel, enrofloxacin, levofloxacin, sulfadiazine, and sulfamethoxazole were the predominant drug residues in the whole cow milk samples. Spatial distribution was found for those antibiotics and anthelmintics with detection frequency higher than 50%, especially for the antibiotics, indicating regional differences in drug application. Based on the current study, exposure to antibiotics and anthelmintics through whole cow milk consumption are lower than the acceptable daily intake values suggested by the China Institute of Veterinary Drug Control. However, long-term exposure to low doses of antibiotics and anthelmintics still needs attention and merits further study.

ABSTRACT

This paper numerically studies the characteristics of flow field around a high-rise building and the cross-contamination when the building surface is heated by the solar radiation. Firstly, the normalized concentration Kc is used to evaluate the dispersion characteristics under different source locations without surface temperature rise. Under iso-thermal condition, the near-wall pollutant dispersion features revealed by the predicted results are similar to our previous wind tunnel experiment. Then, the effect of wall surface temperature rise on the cross-contamination and the flow fields is evaluated based on the near-wall concentration distributions and the wake zone vortex core positions, respectively. When the building surface temperature rises, the location of vortex core obviously changes comparing with that under iso-thermal condition. The correction formula for the vortex core location with the leeward wall surface temperature rise below 15 K is developed. The windward wall surface temperature rise brings more serious pollutant accumulation. The near-wall concentrations increase with the rise of temperature when the pollutant is released from the bottom and middle of leeward wall surface, while the top-release scenario exhibited a contrary tendency. For the three interval ranges of generally recognized Richardson number Ri (Ri < 0.1; 0.1 < Ri < 10; Ri > 10), these results indicate that when Ri is less than 0.1, the effect of wall surface temperature rise on near-wall flow and cross-contamination of small-scale model cannot be ignored.

ABSTRACT

Angiotensin I-Converting Enzyme (ACE, CD143) Gene plays a crucial role in the pathology of carcinomas in many cancers including colorectal cancer (CRC). However, the methylation of ACE was rarely reported. In this study, our purpose was to investigate the methylation status of ACE and explored its prognostic value in CRC. The expression of ACE was detected by quantitative real-time polymerase chain reaction (qRT-PCR) analysis while the methylation status of ACE was measured via methylation-specific polymerase chain reaction (MSP). The result demonstrated that ACE expression was up-regulated in tumour tissues and HT-29 cells compared with the controls. ACE was also confirmed to be hypomethylated in CRC. Next, we evaluated the influence of ACE hypomethylation on cell growth. It was proved to be a favourable factor for the cell proliferation, cell colony forming, but an inhibitor for the cell apoptosis of CRC cells according to MST assay, colony forming assay and flow cytometry assay. ACE hypomethylation was also considered to be related to the prognosis of CRC through Cox regression analysis. Taken together, the over-expression of ACE was regulated by its hypomethylation and the ACE hypomethylation might be an independent prognostic indicator in CRC.

ABSTRACT

Based on frequently used internal partitions in offices, the effects of pollutant source characteristics and an internal partition on airflow and contaminant distribution under different ventilation modes are studied in this paper. The indoor flow field measurement is implemented in a 1:1 single environmental chamber under different ventilation patterns, and then the numerical model is established. The numerical method is verified and analyzed by comparing the measured and simulated results. According to the verification results, the numerical simulation is introduced to study the influence of different supply and return air mixes and pollutant source distributions on the flow field and diffusion performance with an internal partition. The indoor flow field and concentration distribution under different conditions are compared, and the discharge efficiency under different working conditions is analyzed. The results indicate that internal partitions have a greater influence on the down-supply up-return ventilation mode than the floor-supply up-return and top-supply down-return ventilation mode. Furthermore, if the room is zoned, the effect of source position is larger under the down-supply up-return ventilation mode than under the other two modes.

ABSTRACT

BACKGROUND: Cardiovascular disease (CVD) remains an important cause of mortality and morbidity, and high levels of blood cholesterol are thought to be the major modifiable risk factors for CVD. The use of statins is the preferred treatment strategy for the prevention of CVD, but some people at high-risk for CVD are intolerant to statin therapy or unable to achieve their treatment goals with the maximal recommended doses of statin. Ezetimibe is a selective cholesterol absorption inhibitor, whether it has a positive effect on CVD events remains uncertain. Results from clinical studies are inconsistent and a thorough evaluation of its efficacy and safety for the prevention of CVD and mortality is necessary. OBJECTIVES: To assess the efficacy and safety of ezetimibe for the prevention of CVD and all-cause mortality. SEARCH METHODS: We searched the CENTRAL, MEDLINE, Embase and Web of Science on 27 June 2018, and two clinical trial registry platforms on 11 July 2018. We checked reference lists from primary studies and review articles for additional studies. No language restrictions were applied. SELECTION CRITERIA: We included randomised controlled trials (RCTs) that compared ezetimibe versus placebo or ezetimibe plus other lipid-modifying drugs versus other lipid-modifying drugs alone in adults, with or without CVD, and which had a follow-up of at least 12 months. DATA COLLECTION AND ANALYSIS: Two review authors independently selected studies for inclusion, extracted data, assessed risk of bias and contacted trialists to obtain missing data. We performed statistical analyses according to the Cochrane Handbook for Systematic Reviews of Interventions and used the GRADE to assess the quality of evidence. MAIN RESULTS: We included 26 RCTs randomising 23,499 participants. All included studies assessed effects of ezetimibe plus other lipid-modifying drugs compared with other lipid-modifying drugs alone or plus placebo. Our findings were driven by the largest study (IMPROVE-IT), which had weights ranging from 41.5% to 98.4% in the different meta-analyses.Ezetimibe with statins probably reduces the risk of major adverse cardiovascular events compared with statins alone (risk ratio (RR) 0.94, 95% confidence interval (CI) 0.90 to 0.98; a decrease from 284/1000 to 267/1000, 95% CI 256 to 278; 21,727 participants; 10 studies; moderate-quality evidence). Trials reporting all-cause mortality used ezetimibe with statin or fenofibrate and found they have little or no effect on this outcome (RR 0.98, 95% CI 0.91 to 1.05; 21,222 participants; 8 studies; high-quality evidence). Adding ezetimibe to statins probably reduces the risk of non-fatal myocardial infarction (MI) (RR 0.88, 95% CI 0.81 to 0.95; a decrease from 105/1000 to 92/1000, 95% CI 85 to 100; 21,145 participants; 6 studies; moderate-quality evidence) and non-fatal stroke (RR 0.83, 95% CI 0.71 to 0.97; a decrease 32/1000 to 27/1000, 95% CI 23 to 31; 21,205 participants; 6 studies; moderate-quality evidence). Trials reporting cardiovascular mortality added ezetimibe to statin or fenofibrate, probably having little or no effect on this outcome (RR 1.00, 95% CI 0.89 to 1.12; 19457 participants; 6 studies; moderate-quality evidence). The need for coronary revascularisation might be reduced by adding ezetimibe to statin (RR 0.94, 95% CI 0.89 to 0.99; a decrease from 196/1000 to 184/1000, 95% 175 to 194; 21,323 participants; 7 studies); however, no difference in coronary revascularisation rate was observed when a sensitivity analysis was limited to studies with a low risk of bias.In terms of safety, adding ezetimibe to statins may make little or no difference in the risk of hepatopathy (RR 1.14, 95% CI 0.96 to 1.35; 20,687 participants; 4 studies; low-quality evidence). It is uncertain whether ezetimibe increase or decrease the risk of myopathy (RR 1.31, 95% CI 0.72 to 2.38; 20,581 participants; 3 studies; very low-quality evidence) and rhabdomyolysis, given the wide CIs and low event rate. Little or no difference in the risk of cancer, gallbladder-related disease and discontinuation due to adverse events were observed between treatment groups. For serum lipids, adding ezetimibe to statin or fenofibrate might further reduce the low-density lipoprotein cholesterol (LDL-C), total cholesterol and triglyceride levels and likely increase the high-density lipoprotein cholesterol levels; however, substantial heterogeneity was detected in most analyses.None of the included studies reported on health-related quality of life. AUTHORS' CONCLUSIONS: Moderate- to high-quality evidence suggests that ezetimibe has modest beneficial effects on the risk of CVD endpoints, primarily driven by a reduction in non-fatal MI and non-fatal stroke, but it has little or no effect on clinical fatal endpoints. The cardiovascular benefit of ezetimibe might involve the reduction of LDL-C, total cholesterol and triglycerides. There is insufficient evidence to determine whether ezetimibe increases the risk of adverse events due to the low and very low quality of the evidence. The evidence for beneficial effects was mainly obtained from individuals with established atherosclerotic cardiovascular disease (ASCVD, predominantly with acute coronary syndrome) administered ezetimibe plus statins. However, there is limited evidence regarding the role of ezetimibe in primary prevention and the effects of ezetimibe monotherapy in the prevention of CVD, and these topics thus requires further investigation.

OBJECTIVE: To examine and quantify the potential dose-response relationship between green tea intake and the risk of gastric cancer. DESIGN: We searched PubMed, EMBASE, Web of Science, CBM, CNKI and VIP up to December 2015 without language restrictions. SETTING: A systematic review and dose-response meta-analysis of observational studies. SUBJECTS: Five cohort studies and eight case-control studies. RESULTS: Compared with the lowest level of green tea intake, the pooled relative risk (95 % CI) of gastric cancer was 1·05 (0·90, 1·21, I 2=20·3 %) for the cohort studies and the pooled OR (95 % CI) was 0·84 (0·74, 0·95, I 2=48·3 %) for the case-control studies. The pooled relative risk of gastric cancer was 0·79 (0·63, 0·97, I 2=63·8 %) for intake of 6 cups green tea/d, 0·59 (0·42, 0·82, I 2=1·0 %) for 25 years of green tea intake and 7·60 (1·67, 34·60, I 2=86·5 %) for drinking very hot green tea. CONCLUSIONS: Drinking green tea has a certain preventive effect on reducing the risk of gastric cancer, particularly for long-term and high-dose consumption. Drinking too high-temperature green tea may increase the risk of gastric cancer, but it is still unclear whether high-temperature green tea is a risk factor for gastric cancer. Further studies should be performed to obtain more detailed results, including other gastric cancer risk factors such as smoking and alcohol consumption and the dose of the effective components in green tea, to provide more reliable evidence-based medical references for the relationship between green tea and gastric cancer.

ABSTRACT

The lasing spectrum of dye-doped chiral nematic liquid crystal under electric filed was investigated. Two kinds of electrodes were designed to apply transverse electric field to positive liquid crystal cell and longitudinal electric field to negative liquid crystal cell. A 532 nm Nd∶YAG pulsed solid-state laser was used to pump the cell. When transverse electric field is applied to positive liquid crystal device, multi-wavelength laser output is obtained in the range of 630~660 nm. When longitudinal electric field is applied to negative liquid crystal cell, 18.5 nm tunable output lasing is obtained. The output characteristic of cell was analyzed from the texture of device and the photonic band gap. In the positive liquid crystal cell, the competition of electric moment with twisting moment causes the flow of LC molecule, and the flow of LC molecule leads to a floating photonic band gap. For this reason, not only at the edge of photonic band gap but in the photonic band gap can produce lasing. For negative liquid crystal cell, the pitch shrinks with the increase of electric field. The photonic band gap shows blue-shift with the decrease of pitch. and Lasing wavelength is blue shifted from 681.0 to 662.5 nm and the lasing at the edge of photonic band gap. Negative liquid crystal cell has better stability under electric field effect.

BACKGROUND: Although a substantial number of studies focus on the teaching and application of medical statistics in China, few studies comprehensively evaluate the recognition of and demand for medical statistics. In addition, the results of these various studies differ and are insufficiently comprehensive and systematic. OBJECTIVES: This investigation aimed to evaluate the general cognition of and demand for medical statistics by undergraduates, graduates, and medical staff in China. METHODS: We performed a comprehensive database search related to the cognition of and demand for medical statistics from January 2007 to July 2014 and conducted a meta-analysis of non-controlled studies with sub-group analysis for undergraduates, graduates, and medical staff. RESULTS: There are substantial differences with respect to the cognition of theory in medical statistics among undergraduates (73.5%), graduates (60.7%), and medical staff (39.6%). The demand for theory in medical statistics is high among graduates (94.6%), undergraduates (86.1%), and medical staff (88.3%). Regarding specific statistical methods, the cognition of basic statistical methods is higher than of advanced statistical methods. The demand for certain advanced statistical methods, including (but not limited to) multiple analysis of variance (ANOVA), multiple linear regression, and logistic regression, is higher than that for basic statistical methods. The use rates of the Statistical Package for the Social Sciences (SPSS) software and statistical analysis software (SAS) are only 55% and 15%, respectively. CONCLUSION: The overall statistical competence of undergraduates, graduates, and medical staff is insufficient, and their ability to practically apply their statistical knowledge is limited, which constitutes an unsatisfactory state of affairs for medical statistics education. Because the demand for skills in this area is increasing, the need to reform medical statistics education in China has become urgent.
