ABSTRACT

OBJECTIVE: To explore the relationship between ankle dorsiflexion range of motion (ROM) and hip and knee muscle strength between patients with a history of patellar dislocation (PD) to healthy controls. DESIGN: Case-control study. SETTING: Orthopedical specialty outpatient clinic at a tertiary hospital. PARTICIPANTS: Eighty-eight individuals were recruited; 44 individuals aged 16 years or older, of both sexes, with a history of at least one episode of atraumatic unilateral or bilateral PD requiring emergency care (14 men; 30 women; mean age 20 years) and 44 healthy (control) individuals (11 men; 33 women; mean age 21 years) matched for age, weight, and height to PD cases. INTERVENTION: Assessment of hip and knee strength and ankle dorsiflexion ROM. OUTCOME MEASURES: Ankle dorsiflexion ROM was assessed through the lunge test with a goniometer. Hip and knee muscle strength was evaluated through isometric hand-held dynamometry. Differences between healthy and control individuals were assessed using Student t Tests and Mann-Whitney U Test. RESULTS: Patellar dislocation individuals presented with a reduced ankle dorsiflexion ROM [mean difference (MD): 9 degrees; effect size (ES): 1.39; P < 0.001] and generalized hip and knee weakness (MD range: 4.74 kgf to 31.4 kgf; ES range: 0.52-2.35; P < 0.05) compared with healthy subjects. CONCLUSION: Individuals with a history of PD have reduced ankle dorsiflexion ROM and hip and knee muscle strength compared with healthy controls.

ABSTRACT

Tibial plateau fractures are a risk to the functional integrity of the knee, affecting the axial alignment and capable of leading to pain and disability of the individual. Early weight bearing and joint mobilization can prevent these functional deficits. the goal of the present study was to conduct a systematic review of the literature about studies that quote the beginning, evolution, and progression criteria for weight-bearing in postoperative period of tibial plateau fractures. We selected articles published in the last 12 years, in Portuguese and English, that described the time of onset and progression of weight-bearing, considering the severity of the fracture. Thirty-six articles were selected. There is no consensus in the literature as to the beginning and evolution of weight-bearing in the postoperative period of tibial plateau fractures; however, a relationship between the severity of the fracture and the fixation method has been observed.

Resumo As fraturas do planalto tibial constituem risco à integridade funcional do joelho, afetando o alinhamento axial e podendo levar à dor e à incapacidade do indivíduo. O suporte de carga e a mobilização articular precoce podem prevenir esses déficits e acelerar o processo de reabilitação. O objetivo do presente trabalho foi realizar uma revisão da literatura sobre estudos que citam o início, evolução e critérios de progressão do suporte de carga no pós-operatório das fraturas de planalto tibial. Foram selecionados artigos publicados nos últimos 12 anos, nos idiomas português e inglês, que descrevessem o tempo de início e progressão do suporte de carga, considerando a gravidade da fratura. Foram selecionados 36 artigos na literatura. Não há consenso na literatura quanto ao início e evolução do suporte de carga no pós-operatório das fraturas do planalto tibial. Contudo, observa-se relação entre a gravidade da fratura e o tempo de início da carga.

ABSTRACT

OBJECTIVE: The aim of this study was to translate, culturally adapt, and validate the "Foot Function Index - Revised" (FFI-R) for use in Brazilian Portuguese. METHODS: The scale was translated and administered (as recommended by Guillemin, 2000) to 52 patients in the postoperative period after foot and ankle surgery. Seven days after the initial assessment, the scale was readministered by a different interviewer. The data were entered into an Excel spreadsheet and analyzed using SPSS version 23.0 software for Mac. Reproducibility was assessed using intraclass correlation analysis. RESULTS: were considered statistically significant at a type I error rate of 5%. Results: The following random-effects intraclass correlation coefficients (ICC) were obtained for each score on the FFI-R: 0.625 for pain, 0.558 for stiffness, 0.757 for difficulty, 0.718 for activity restrictions, 0.854 for personal concerns, and 0.753 for the total score. CONCLUSION: The FFI-R was successfully translated to Portuguese and culturally adapted for use in Brazilian patients, demonstrating satisfactory validity and reliability. Level of Evidence I, Testing of Previously Developed Diagnostic Criteria on Consecutive Patients (with universally applied reference "golg" standard).

MÉTODOS: A escala foi traduzida e aplicada (segundo recomendado por Guillemin, 2000) a 52 pacientes depois de cirurgia do pé e tornozelo. Sete dias depois da avaliação inicial, a escala foi novamente aplicada por outro entrevistador. Os dados foram inseridos em uma planilha do Excel e a análise estatística foi realizada no software SPSS 23.0 para Mac. A análise de correlação intraclasse foi realizada para avaliar a reprodutibilidade. Os resultados foram considerados estatisticamente significantes em erro do tipo I de até 5%. RESULTADOS: Foram obtidos os seguintes coeficientes de correlação intraclasse (CCI) de efeitos aleatórios para cada pontuação no FFI-R: 0,625 para dor, 0,558 para rigidez, 0,757 para dificuldade, 0,718 para restrição de atividades, 0,854 para preocupações pessoais e 0,753 para o escore total. CONCLUSÃO: O FFI-R foi traduzido com sucesso para o português e adaptado culturalmente para aplicação em pacientes brasileiros, demonstrando validade e confiabilidade satisfatórias. Nível de Evidência I, Teste de Critérios Diagnósticos Desenvolvidos Anteriormente em Pacientes Consecutivos (com padrão de referência "ouro" aplicado).

PURPOSE: Our aim was to evaluate the effects of the use of oral contraceptives (OC) on the hip and knee kinematics of healthy women during anterior stair descent. METHODS: Forty volunteers aged from 18 to 26 years were divided into two groups: 1-Group of women who had used OC for at least 3 months prior to evaluation (n = 20) and 2-Group of women who did not use OC (n = 20). The knee flexion/extension and abduction/adduction, hip flexion/extension, abduction/adduction and medial/lateral rotation excursions (degrees) were calculated for the dominant (supporting) limb during anterior stair descent. T tests for independent samples were used to compare the kinematic differences between the groups (α = 0.05). RESULTS: No significant difference was verified between the groups regarding the maximum excursion of knee flexion (n.s.) and abduction (n.s.) or hip flexion (n.s.), adduction (n.s.) and medial rotation (n.s.). When considering the knee flexion at 50°, no significant difference was verified between the groups regarding the excursion of knee abduction (n.s.) or hip flexion (n.s.) adduction (n.s.) and medial/lateral rotation (n.s.). CONCLUSION: These results suggest that the use of OC does not influence the hip and knee kinematics during anterior stair descent. Therefore, the role of this medication as a protective factor against anterior cruciate ligament injuries remains questionable.

ABSTRACT

PURPOSE: This study aimed to verify the effects of functional stabilization training on lower limb kinematics, functional performance, and eccentric hip and knee torques. METHODS: Twenty-eight women were divided into a training group (TG; n = 14), which carried out the functional stabilization training during 8 wk, and a control group (CG; n = 14), which carried out no physical training. The kinematic assessment of the lower limb was performed during a single-leg squat, and the functional performance was evaluated by way of the single-leg triple hop and the timed 6-m single-leg hop tests. The eccentric hip abductor, adductor, lateral rotator, medial rotator, and the knee flexor and extensor torques were measured using an isokinetic dynamometer. RESULTS: After 8 wk, the TG significantly reduced the values for knee abduction (from -6.86° to 1.49°), pelvis depression (from -10.21° to -7.86°) and femur adduction (from 7.08° to 5.19°) as well as increasing the excursion of femur lateral rotation (from -0.55° to -3.67°). Similarly, the TG significantly increased the values of single-leg triple hop (from 3.52 to 3.92 m) and significantly decreased the values of timed 6-m single-leg hop tests (from 2.43 to 2.14 s). Finally, the TG significantly increased the eccentric hip abductor (from 1.31 to 1.45 N · m · kg(-1)), hip lateral rotator (from 0.75 to 0.91 N · m · kg(-1)), hip medial rotator (from 1.45 to 1.66 N · m · kg(-1)), knee flexor (from 1.43 to 1.55 N · m · kg(-1)), and knee extensor (from 3.46 to 4.40 N · m · kg(-1)) torques. CONCLUSIONS: Strengthening of the hip abductor and lateral rotator muscles associated with functional training improves dynamic lower limb alignment and increases the strength and functional performance.
