ABSTRACT

OBJECTIVES: To further investigate the clinical characteristics and circulating lymphocyte profiles of patients with early-onset primary Sjögren's syndrome (pSS). METHOD: Data of 333 patients with pSS were analysed retrospectively. Early onset was defined as a pSS diagnosis at an age of 35 years or younger. The clinical, laboratory and immunophenotypic profiles of peripheral blood lymphocyte subsets were compared between early- and later-onset pSS. RESULTS: Thirty-six (10.81%) patients matched the definition of early-onset pSS, with age at disease onset being 28.97 (5.53) years. Elevated serum IgG level (77.14% vs 31.16%, P <0.001), low C3 (41.67% vs 20.20%, P =0.004) and C4 levels (27.78% vs 6.40%, P <0.001), anti-SSA positivity (91.67% vs 51.85%, P <0.001) and anti-SSB positivity (50% vs 20.54%, P <0.001) were more frequent in early-onset patients. The frequencies of hematological (80.56% vs 52.53%, P =0.001), renal (19.44% vs 5.05%, P =0.005) and mucocutaneous involvement (50% vs 22.56%, P <0.001) were significantly higher in the early-onset pSS group, which showed a higher 2010 EULAR SS Disease Activity Index (ESSDAI) [11(6.25-17) vs 7(3-12); P =0.003], compared with the later-onset group. In addition, profound CD4+ T-cell lymphopenia was found in patients with early-onset. CONCLUSIONS: Patients with early-onset pSS have distinctive clinical manifestations and greater activation of the cellular immune system, present with more severe clinical symptoms and immunological features, have increased activation of circulating T cells and have an unfavourable prognosis. Thus, they require more positive treatment with glucocorticoids and/or immunosuppressants and merit closer follow-up and regular monitoring.

ABSTRACT

Objective To investigate the expression of CXCR4 and CXCL12 in the labial gland of patients with primary Sj?gren's syndrome (pSS), and to explore their role in the pathogenesis of pSS. Methods The expression of CXCR4 and CXCL12 in labial gland tissues was detected by immunohistochemistry in 32 cases of newly diagnosed pSS and 30 cases of oral mucosa cysts or trauma patients. The expression level, intensity and location were analyzed and compared statistically. χ2 test and Spearman correlation analysis were used for statistical analysis. Results The positive rate of CXCR4 in the test group was 91%(n=29), which was significantly higher than that in the control group (33%, n=10, χ2=21.77, P=0.001). The positive rate of CXCL12 in the test group was 97%(n=31), which was significantly higher than that in the control group 40%(n=12), and the difference was statistically significant ( χ2=23.57, P=0.001). Conclusion CXCR4 and CXCL12 are highly expressed in labial gland of pSS patients, this suggests that they participate in the pathological process of pSS local inflammatory response and play an important role in pSS pathogenesis.

ABSTRACT

@# Objective To observe the protective effect of Qiangxin oral liquid on experimental left ventricular hypertrophy induced by isoproterenol and ultrastructure in rats.MethodsMyocardial hypertrophy model of rats were established by injection of isoproterenol (5 mg/kg/d) for 7 days. Thirty rats were divided into the control group, model group and treatment group (treated with Qiangxin oral liquid from second day after myocardial hypertrophy model made and continued for 12 weeks). Cardiac structure and function were detected in all groups by ultrasonography on 2nd, 6th and 12th week after model made respectively. After 12 weeks, the left ventricular weight/body weight index (LVW/BW) of all animals was tested and myocardial ultrastructure was examined.ResultsCompared with the model group, the interventricular septal end systolic thickness, left ventricular posterior wall thickness, left ventricular diameter, and left ventricular mass index of the treatment group decreased significantly ( P<0.05~0.01), while the damage of myocardial ultrastructrue lightened.ConclusionQiangxin oral liquid can improve the experimental myocardial hypertrophy of rats induced by isoproterenol.

ABSTRACT

@#ObjectiveTo observe the effect of 99Tc-methylene diphosphate (99Tc-MDP, Yunke) combined with rehabilitation on ankylosing spondylitis (AS).Methods118 AS patients were randomly divided into the treatment group (n=58) and control group (n=60). The treatment group was treated with meloxicam, sulfasalazine and vein injecting Yunke combined rehabilitation. The control group was treated only with meloxicam and sulfasalazine. The changes of both clinical and experimental indexes of two groups before and after treatment were analyzed.ResultsAfter two therapeutic courses, a much better result was obtained in the treatment group compared with the control group at the respects of releasing arthralgia, shortening morning stiffness, improving range of spinal motion, and decreasing the levels of ESR, CRP, PLT and IgM, ect (P<0.05~0.01) without obvious side effects.ConclusionThe effect of Yunke combined with rehabilitation on AS is evident.
