ABSTRACT

Background: Desmoid tumor (DT) is a rare locally aggressive but non-metastatic mesenchymal soft tissue neoplasm that predominantly occurs in the abdominal wall, abdominal cavity, and extremities. Its occurrence in the mesentery is relatively uncommon. Case reports: This article reports two cases of desmoid tumor treated at the Department of Gastrointestinal Surgery, Weifang People's Hospital. The first case was a 59-year-old male patient who had previously undergone surgery for esophagogastric junction cancer. Postoperatively, he developed an intra-abdominal mass that rapidly increased in size within three months. The second case was a 60-year-old male patient who incidentally discovered a mass in the left lower abdomen. Both patients underwent surgical treatment, and the postoperative pathological diagnosis was mesenteric desmoid tumor. Conclusion: The treatment of desmoid tumor remains challenging. Simple surgical resection often yields unsatisfactory outcomes, and the efficacy of adjuvant radiotherapy and chemotherapy is also limited. Further research and clinical practice are necessary to improve diagnostic and therapeutic strategies, aiming to enhance patient survival and quality of life.

ABSTRACT

INTRODUCTION: Perineal hernia (PH) is a rare complication that can occur after abdominoperineal resection for rectal cancer. Laparoscopic repair of PHs has gained increasing popularity compared to open approaches due to advantages such as superior visualization, decreased invasiveness, and faster recovery. This case report highlights the successful use of laparoscopic tension-free mesh repair for concurrent perineal and inguinal hernias after rectal cancer surgery. CASE DESCRIPTION: A 51-year-old man underwent laparoscopic-assisted abdominoperineal resection for rectal cancer. About 2 months postoperatively, he developed reducible masses in the perineal and left groin regions, associated with urinary symptoms and sensation of prolapse. Physical exam revealed protruding masses that enlarged with Valsalva. Pelvic CT confirmed PH and left inguinal hernia. INTERVENTIONS: Laparoscopic tension-free repair of the PH and inguinal hernia was performed on this patient. The repair was completed by the steps of adhesion separation, mesh placement, and fixation. OUTCOMES: The 98-minute surgery was successful without complications. The patient recovered well, ambulating on postoperative day 2 and getting discharged on day 6. CONCLUSION: This case demonstrates that laparoscopic tension-free repair with mesh is an effective approach for treating PH and concurrent inguinal hernia following rectal cancer surgery, resulting in successful outcomes and low recurrence rates. The laparoscopic technique provides benefits of minimal invasiveness and rapid recovery.

BACKGROUND: The incidence of gastric cancer is high. The number of dissected lymph nodes was an independent factor affecting prognosis. Although preoperative labeling is helpful in lymph nodes resection, there are no guidelines for when to perform preoperative labeling. AIM: To investigate the role of nanocarbon in lymph node dissection during gastrectomy, and to discuss the relationship between the timing of preoperative injection of carbon nanoparticles and the extent of lymph node dissection. METHODS: A prospective analysis was performed on the clinical data of 307 patients with advanced gastric cancer who underwent laparoscopic surgery in the General Surgery Department of Weifang People's Hospital between June 2018 and February 2021. The patients were randomly divided into experimental group and control group based on whether they received preoperative nanocarbon injection or not. The experimental group was divided into different groups according to the preoperative labeling time. The number of dissected lymph nodes and the number of lymph nodes with black staining were compared in each group after surgery, and the role of nanocarbon in the number of dissected lymph nodes, pathological staging, and the relationship with prognosis were discussed. RESULTS: The average number of dissected lymph nodes in the experimental group was higher than that in the control group. In the experimental group, the number of lymph node dissections and number of black-staining lymph nodes in the nanocarbon-labeling group at 2 d and 1 d before surgery were higher than in the labeling group on the day before surgery (P < 0.05). CONCLUSION: Preoperative nanocarbon labeling can safely and effectively guide lymph node dissection. To improve the detection rate of lymph nodes is conducive to subsequent comprehensive anti-tumor therapy.

ABSTRACT

Ultrathin unobstructed gas transport channels through the membrane selective layer are constructed in mixed matrix membranes (MMMs) by using gravity-induced interface self-assembly of poly(vinylamine) and polymer-modified MIL-101(Cr). For CO2 /N2 (15/85 by volume) mixed gas, the MMMs achieve a high CO2 permeance of 823 gas permeation units and CO2 /N2 selectivity of 242 at 0.5 MPa. Based on economic analyses, a two-stage membrane process can achieve gas separation and economic targets.

Colon cancer remains the second most common cause of cancer-related death, indicating that a proportion of cancer cells are not eradicated by current therapies. Investigation of the molecular mechanisms involved in the development and progression of the disease will aid in the further understanding of the pathogenesis and progression and offer new targets for effective therapies. In the present study, we initially confirmed that ABCA1 was aberrantly expressed in colon cancer tissues and colon cancer cells. Its overexpression inhibited the proliferation of colon cancer HCT116 cells while silencing of ABCA1 promoted the proliferation and inhibited the apoptosis of colon cancer LDL1 cells. Upregulation of specific miRNAs can contribute to the downregulation of tumor-suppressive genes. Thus, we aimed to ascertain whether ABCA1 is downregulated by overexpression of a specific miRNA in colon cancer. We screened microRNAs that may target ABCA1 by miRanda which is a commonly used prediction algorithm. We found that miR-183 targets the 3'UTR of ABCA1 mRNA. Subsequent experiments confirmed that miR-183 degraded ABCA1 mRNA in the colon cancer cells. Finally, we demonstrated that miR-183 promoted the proliferation and inhibited the apoptosis of colon cancer cells. Thus, we conclude that miR-183 promotes proliferation and inhibits apoptosis by degrading ABCA1 in colon cancer.

BACKGROUND/AIMS: This study aimed to evaluate the efficacy of modified Roux-en-Y gastric bypass for type 2 diabetes mellitus. METHODOLOGY: Forty-five type 2 diabetes mellitus patients underwent modified Roux-en-Y gastric bypass at WeiFang People's Hospital. Data on patient demographics, fasting plasma glucose (FPG), body mass index (BMI), medication use, remission and hemoglobin A1c (HbAlc) were prospectively collected and analyzed. RESULTS: At 6 months after surgery, all of these 45 patients obtained remission or a marked improvement. FPG was in the normal range in 39 (86.7%) patients stopping medicine treatment for their diabetes. Six patients (13.3%) had an obvious reduced abnormal FPG and they only required lower drug dosage. No statistically significant differences were found between the obese or non-obese groups (p=0.311). The mean BMI dropped from 28.9±3.0 kg/m2 to 27.4±2.8 kg/m2 (p=0.000) at the third month and 26.3±2.5 kg/m2 (p=0.000) at the sixth month. HbAlc decreased from their preoperative values of 7.4%±2.2% to 6.3%±1.5% (p=0.000) at the third month and 5.1%±0.9% (p=0.000) at the sixth month. CONCLUSIONS: Modified Roux-en-Y gastric bypass was effective in treating type 2 diabetes mellitus, independent of body mass index.

ABSTRACT

Traditional farming practices suggest that cultivation of a mixture of crop species in the same field through temporal and spatial management may be advantageous in boosting yields and preventing disease, but evidence from large-scale field testing is limited. Increasing crop diversity through intercropping addresses the problem of increasing land utilization and crop productivity. In collaboration with farmers and extension personnel, we tested intercropping of tobacco, maize, sugarcane, potato, wheat and broad bean--either by relay cropping or by mixing crop species based on differences in their heights, and practiced these patterns on 15,302 hectares in ten counties in Yunnan Province, China. The results of observation plots within these areas showed that some combinations increased crop yields for the same season between 33.2 and 84.7% and reached a land equivalent ratio (LER) of between 1.31 and 1.84. This approach can be easily applied in developing countries, which is crucial in face of dwindling arable land and increasing food demand.
