ABSTRACT

Objective@#To explore the effects of the combination of butylphthalide and clopidogrel on the blood viscosity and parameters of transcranial Doppler in patients with cerebral infarction.@*Methods@#Ninety patients with cerebral infarction were selected, and they were divided into control group and observation group according to the digital table, with 45 cases in each group.The control group was orally given clopidogrel, the observation group was given clopidogrel combined with butylphthalide.The clinical curative effect, blood viscosity changes and transcranial Doppler changes were compared between the two groups.@*Results@#The total effective rate of the observation group was 93.33%, which was significantly higher than 73.33% of the control group (χ2=5.67, P<0.05). After treatment, the HSV and PSV of the two groups were lower than those before treatment(t=4.34, 4.56, 7.89, 7.23, all P<0.05), and the decrease of HSV and PSV in the observation group was more significant than those in the control group after treatment(t=6.55, 6.67, all P<0.05). Compared with those before treatment, the Vm of ACA and MCA in the two groups after treatment significantly increased(t=4.34, 4.90, 7.33, 6.92, all P<0.05), the PI of ACA and MCA significantly decreased(t=4.77, 4.66, 7.12, 7.29, all P<0.05), and the above indicators in the observation group after treatment improved more significantly compared with those in the control group(t=6.31, 5.50, 6.54, 6.28, all P<0.05).@*Conclusion@#The effect of the early combination of butylphthalide and clopidogrel in the treatment of patients with cerebral infarction is significant, and it can effectively improve the change of blood viscosity.

ABSTRACT

Objective To investigate the changes in cognitive function in patients with early Parkinson's disease (PD) in a 5-year follow-up.Methods A total of 181 PD and 173 normal participants were recruited between January 2009 and January 2012 at the Department of Neurology,the Second Hospital of Jiaxing City.Regression analysis was used to evaluate the risk factors of cognitive impairment,and changes in subdomains of the Montreal Cognitive Assessment (MoCA) were compared annually with baseline data.Results Baseline clinical data were similar between the two groups.The level of cognitive impairment was positively correlated to the age of onset and the Hamilton Anxiety Scale (H AMD) (t =3.326,P< 0.05;t =5.211,P<0.01),and negatively correlated to education level (t=-2.505,P<0.05).There were no statistical differences in the first (26.5 ± 2.6),second (26.3±3.2) and third year (25.9±2.9) in the total scores of MoCA,which significantly increased in the forth (24.4 ± 2.3,P<0.05) and fifth (24.1 ± 1.2,P<0.05) year compared with baseline levels (26.7±2.9).However,in the control group,differences between any two years in total scores were not statistically significant (all P>0.05).Seven subdomains of MoCA were attenuated,and four of them were significant different between the groups,including delayed recall,attention,abstraction task and orientation after the forth year of follow-up (all P < 0.05).Meanwhile,visuospatial execution capacity was attenuated before the third year,and then rose markedly (P<0.05).In the control group,the between-year differences of seven subdomains were not statistically significant (all P>0.05).Conclusions Cognitive function of PD patients decreases significantly in some domains,such as delayed recall,attention,abstraction task and orientation after a five-year followup.

ABSTRACT

Integration of heterogeneous systems is the key to hospital information construction due to complexity of the healthcare environment. Currently, during the process of healthcare information system integration, people participating in integration project usually communicate by free-format document, which impairs the efficiency and adaptability of integration. A method utilizing business process model and notation (BPMN) to model integration requirement and automatically transforming it to executable integration configuration was proposed in this paper. Based on the method, a tool was developed to model integration requirement and transform it to integration configuration. In addition, an integration case in radiology scenario was used to verify the method.

ABSTRACT

The digital clinical guidelines could greatly improve the safety and quality of clinical diagnosis and treatment. However, most of such guidelines were written in engineering language, which was difficult for clinicians to understand in practice. To tackle this problem, we adoped the flowchart as the visual representation method of digital clinical guidelines. The corresponding criterions expressed by the flowchart were easily understood by clinicians. Then we set the digital clinical guidelines written in Arden syntax as an example, the interconversion between flowcharts and digital clinical guidelines was realized. The result of using the visual representation method proposed in this paper shows that the clinical diagnosis logic becomes clear and intuitive. So this is an effective method for clinicians to understand and edit the digital clinical guidelines.

ABSTRACT

System integration is inevitable since there are lots of heterogeneous medical information systems in the complicated medical environment. The current medical communication standards often focus on one aspect of the integration and do not provide a general scheme. Based on the analysis of the application of medical integration, the medical integration model HIM (Healthcare integrating model) is put forward, and the dataflow integration framework, function integration framework and interface integration framework in the HIM are designed subsequently. HIM provides a 3-D scheme for the integration of medical information systems, which not only contains the three aspects of integration application vertically, but covers the whole medical area horizontally.

ABSTRACT

It is very important to recognize arrhythmia in clinical electrocardiography (ECG) analysis. The fundamental of beat pattern recognition is presented in this paper. Various prevalent methods for arrhythmia recognitiion are categorized and summarized, based on which the advantages and disadvantages among the methods are compared, and the main problems are discussed in depth. At last, the development trend of arrhythmia recognition technology is pointed out.
