ABSTRACT

OBJECTIVE: This study aimed to analyze the epidemiological profile and psychological disorder of the suicide attempt cases in Francisco Beltrão, Paraná. METHODS: This is an epidemiological descriptive/qualitative study of suicide attempts conducted between 2017 and 2020. This study used data from the Brazilian Information system for notifications involving suicide attempts (Sistema de Informação de Agravos de Notificação, SINAN) and 447 notifications were obtained. An analysis of the electronic medical record of these patients was performed in order to investigate the mental disorders, using a questionnaire. RESULTS: Of the 447 notifications, 382 were eligible for the study. Using the 95% confidence interval, there was a higher frequency of females with 71.7% aged between 18 and 35 years representing 48.4%, with 77.2% white race/color, the singles appeared in the majority with 47.6% with a history of previous suicide attempts, using exogenous intoxication as a method in the attempt with 67.5%. Regarding mental health, 66.5% of the patients had some mental disorders, with the highest prevalence of recurrent depressive disorder found in 40.6%. CONCLUSION: It was observed that there is a need for training of health professionals and implementation of programs and preventive measures aimed primarily at females aged between 18 and 35 years with mental disorders, especially with recurrent depressive disorder and with a history of previous suicide attempt.

OBJECTIVES: The aim of this study was to describe long-term changes in standard blood tests and ultrasound (US) findings in pediatric patients with Extrahepatic Portal Vein Obstruction (EHPVO) who have not undergone Meso-Rex Bypass (MRB) surgery. METHODS: US, laboratory, and endoscopic data of 77 patients were analyzed and compared at 2 different points in time: at initial workup and at most recent follow-up. Differences were assessed using McNemar and Wilcoxon tests, while correlations were evaluated using generalized estimating equations. RESULTS: Upper gastrointestinal bleeding was the most frequent initial manifestation of EHPVO, in 45.4% of subjects. The mean age at diagnosis was 4.3 years. Seventy-four percentage had a previous history of umbilical catheterization and/or neonatal sepsis. Over time, there was a significant increase in the prevalence of leukopenia, thrombocytopenia, and, interestingly, of left lobe atrophy (LLA), even though the number of esophageal varices and the need for endoscopic interventions reduced. A significant correlation was found between history of umbilical catheterization and LLA, splenomegaly and LLA, and gallbladder wall thickening and LLA. Overall, the number of patients with cholelithiasis at initial workup was low (and therefore, not analyzed with inferential statistics); however, we observed a relative increase over time. CONCLUSIONS: In this cohort of patients with EHPVO followed over more than 8 years without treatment with MRB, we report the novel finding of a significant increase in the prevalence of LLA over time. LLA correlated with past medical history of umbilical catheterization, and findings of splenomegaly and gallbladder wall thickening. We propose that that LLA should be further explored as a marker of portal hypertension, particularly in the context of a history of umbilical catheterization.

INTRODUCTION: Acinetobacter baumannii complex are microorganisms of critical priority of resistance, being associated with higher costs and negative outcomes for hospitalized patients. Thus, the study aimed to analyse the factors associated with A. baumannii complex infection in various hospital sectors. METHODOLOGY: This is a case-control study that included patients hospitalized from January 2017 to June 2019. Demographic, microbiological and clinical variables were collected from each patient. All cases had positive culture results for A. baumannii complex resistant to more than three classes of antimicrobials. Carbapenem-resistance was examined by the disk diffusion test, while the broth microdilution method was used to determine the susceptibility to colistin. RESULTS: A. baumannii complex infection was mostly present in ICU (74.2%) than in other hospital areas. The bacteria was also linked with the length of hospitalization until the results for the culture (OR = 1.13; 95% CI: 1.06 - 1.21; p < 0.001) and with pneumonia associated with mechanical ventilation (OR = 4.48; 95% CI: 1.55 - 13.00; p = 0.006). Moreover, patients exposed to infection with multidrug-resistant A. baumannii complex had higher risks of death (OR = 3.25; 95% CI: 1.06 - 9.91; p = 0.039). CONCLUSIONS: This study provides evidence that A. baumannii complex infection is associated with the number of days of hospitalization up to culture positivity, pneumonia associated with the use of mechanical ventilation and death. Infections appear to be more critical in ICU when compared to other areas. Taken together, these findings could support hospital infection surveillance programs, as well as prevention measures to reduce mortality rates and other complications.

ABSTRACT

O aumento da incidência de câncer na população brasileira alerta para a necessidade de implantação de medidas de rastreamento baseadas na busca ativa de novos casos. Neste sentido, o presente trabalho avaliou o impacto de um programa itinerante de rastreamento dos cânceres de mama, colo de útero, próstata e pele realizado pelo Serviço Social da Indústria (Sesi) no período de 1 ano, em 22 municípios do estado do Paraná. O estudo é descritivo, com banco de dados secundários e abordagem quantitativa, referente ao programa Cuide-se Mais do Sesi Paraná. Através de uma unidade móvel, foram realizadas análises da superfície da pele, coleta de Papanicolau em mulheres de 25 a 69 anos, mamografia em mulheres com idade entre 40 e 69 anos e dosagem de Antígeno Prostático Específico (PSA) em homens de 45 a 69 anos de 22 municípios do Paraná. Foram atendidos 4679 trabalhadores paranaenses em 202 empresas, realizadas 4120 palestras de orientações e 1346 oficinas de prevenção. As lesões avaliadas em pele suspeitas para câncer corresponderam a 3,4% do total de 3054 exames. Foram realizadas 873 mamografias, e encaminhados 11 casos de lesões suspeitas no exame de Papanicolau de um total de 2073 coletas. O PSA esteve alterado em 8 homens de um total de 1308 exames. A instalação da unidade móvel dentro da empresa quebra duas barreiras de impedimento dos trabalhadores realizarem seus exames preventivos: a sociocultural e a institucional. Desta forma, programas e ações de rastreamento do câncer podem contribuir para o diagnóstico precoce desta patologia, especialmente na população em idade economicamente produtiva.(AU)


The increased incidence of cancer in the Brazilian population urges the need of implementing screening measures based on the active search for new cases. In this sense, the present work presents the results of the Take More Care disease-screening program. With a mobile unit, analyzes of the skin surface, Pap smear collection in women aged 25 to 69 years, mammography in women aged 40 to 69 years and specific prostatic antigen (SPA) in men aged 45 to 69 were performed in 22 municipalities of Paraná state. 4679 workers from Paraná were attended in 202 companies, 4120 orientation lectures and 1346 prevention workshops were given. Suspected skin lesions for skin cancer corresponded to 3.4% from 3054 skin assessment tests. 873 mammograms were performed, and 11 cases of suspected lesions were referred to the Pap smear, from 2073 collections. The SPA was altered in 8 men, in a total of 1308 tests performed. The installation of the mobile unit inside the company breaks two barriers preventing workers from carrying out their preventive exams: the sociocultural and the institutional. Thus, cancer screening programs and actions can contribute to the early diagnosis of this pathology, especially in the population of economically productive age.(AU)

Resumo Objetivo Caracterizar o perfil e identificar os fatores associados à Hepatite B na população com 60 anos ou mais de idade da região Sudoeste do Paraná, Brasil. Método Trata-se de um estudo epidemiológico descritivo e inferencial que analisou quantitativamente dados secundários obtidos através do Sistema Nacional de Agravos de Notificação (SINAN), entre 2007 a 2017. Resultados A Hepatite B foi mais frequente em homens, com baixa escolaridade e em indivíduos que exerciam atividade ligada à agricultura. A maioria dos casos foi notificada entre 2013 a 2017 e em sujeitos que reportaram exposição a material biológico. No modelo multivariado de regressão logística, as infecções pelos vírus da hepatite B foram associadas à raça/cor não branca (OR: 2,89; IC95% 1,07 - 7,87), ao histórico de realização de transfusão sanguínea (OR: 14,51; IC95% 5,44 - 38,74), em residentes de municípios de 10 a 20 mil habitantes (OR: 4,57; IC95% 1,59 - 13,12) e de municípios entre 20 a 50 mil habitantes (OR: 4,33; IC95% 1,61 - 11,56). Conclusão A caracterização dos possíveis fatores de risco para hepatite B nessa população podem subsidiar ações mais eficazes de prevenção e promoção de saúde, bem como fomentar estudos específicos que possam guiar políticas de atenção integral ao idoso.


Abstract Objective to characterize the epidemiologic profile of Hepatitis B in the population aged 60 years old or more from the southwest region of the state of Paraná, Brazil, between 2007 and 2017. Method an epidemiologic, descriptive, and inferential study was conducted based on notifications obtained from the Notifiable Diseases Information System(or SINAN). Results Hepatitis B was more frequent in men, those with low levels of schooling and among individuals who worked in agriculture. Most of the cases were reported between 2013 and 2017 in subjects who reported having been exposed to biological material. According to logistic regression, hepatitis B infections were associated with non-white ethnicity (OR: 2.89; 95%CI 1.07 - 7.87), a history of blood transfusions (OR: 14.51; 95%CI 5.44 - 38.74), living in municipal regions with 10,000 to 20,000 inhabitants (OR: 4.57; 95%CI 1.59 - 13.12) and also among individuals from municipal regions with 20,000 to 50,000 inhabitants (OR: 4.33; 95%CI 1.61 - 11.56). Conclusion the epidemiological profile reported here represents a risk factor for hepatitis B in this population. The data can support more effective interventions, as well as further studies to guide comprehensive public health policies for older adults.

ABSTRACT Objective: To determine the prevalence of hepatic steatosis (HS) in children and adolescents with cystic fibrosis (CF) and associate it with nutritional status. Methods: Cross-sectional study with children and adolescents with CF diagnosis. Weight and height were used to calculate the body mass index (BMI) and subsequent classification of the nutritional status. The midarm circumference (MAC), triceps skinfold thickness (TSF) and midarm muscle circumference (MAMC) were used to evaluate body composition. Abdominal ultrasonography was performed for diagnosis of HS. The statistical tests used were Student's t test, Mann-Whitney test and chi-square test with significance level of 5%. Results: 50 patients with CF were evaluated, 18 (36%) were diagnosed with HS (Group A) and 32 (64%) without HS (Group B). The mean age of Group A was 13,2±4,9 years old and Group B 11,7±4,9; for BMI, the value for Group A was 18,0±4,1 and Group B was 15,7±3,8; the TSF of Group A was 8,4±3,5 mm and Group B was 7,0±2,5 mm. For these variables, there was no significant difference between the groups. The mean of MAC and MAMC differed significantly between the groups, being higher in the HS group, with p values of 0,047 and 0,043. Conclusions: The frequency of HS in patients with CF is high and it is not related to malnutrition, according to the parameters of BMI, TSF and MAMC. The values of MAC and MAMC indicated a greater reserve of muscle mass in patients with HS.


RESUMO Objetivo: Determinar a prevalência de esteatose hepática (EH) em crianças e adolescentes com fibrose cística (FC) e associá-la com o estado nutricional. Métodos: Estudo transversal com crianças e adolescentes com diagnóstico de FC. Foram aferidos o peso e a altura para o cálculo do índice de massa corpórea (IMC) e classificação do estado nutricional. A circunferência do braço (CB), a dobra cutânea tricipital (DCT) e a circunferência muscular do braço (CMB) foram empregadas para avaliação da composição corporal. A ultrassonografia abdominal foi realizada para o diagnóstico de EH. Os testes estatísticos empregados foram o teste t de Student, o teste de Mann-Whitney e o teste do qui-quadrado, com nível de significância de 5%. Resultados: Dos 50 pacientes avaliados, 18 (36%) apresentaram EH (Grupo A) e 32 (64%) não (Grupo B). Para as médias de idade (Grupo A: 13,3±5,0 anos; e Grupo B: 11,7±5,0 anos), IMC (Grupo A: 18,0±4,1; e Grupo B: 15,7±3,8) e DCT (Grupo A: 8,4±3,5 mm; e Grupo B: 7,0±2,5 mm), não houve diferença significativa entre os grupos. A média da CB e da CMB diferiram significativamente entre os grupos, sendo mais elevada no grupo com EH, com valores p respectivos de 0,047 e 0,043. Conclusões: É alta a frequência de EH em pacientes com FC e ela não está relacionada com a desnutrição, segundo os parâmetros de IMC, DCT e CMB. Os valores de CB e CMB indicaram maior reserva de massa muscular nos pacientes com EH.

BACKGROUND: The prison system in Paraná, Brazil, is experiencing serious problems related to the increasing number of prisoners. Control of hepatitis C virus (HCV) has become more intense because the incarcerated population is considered a high-risk group for contagious diseases due to the favorable conditions found in prisons for the spread of these morbidities. The objective of this study was to identify features associated with hepatitis C infection among male prisoners in correctional institutions of Paraná state, Brazil. METHODS: This was a case-control study (27 cases and 54 controls) of men incarcerated in 11 penitentiaries in Paraná, Brazil. Information was obtained through a questionnaire in a cross-sectional epidemiological survey on HCV infection during the period from May 2015 to December 2016. Eligible men were recruited after testing positive for anti-HCV antibodies. Cases and controls were selected based on serological results of enzyme-linked immunosorbent assays and were matched by age, location of the penitentiary, and time in prison. Logistic regression analysis was used to identify risk factors for HCV seropositivity. RESULTS: The main significant independent risk factor for the acquisition of HCV infection was the use of injectable drugs (OR = 4.00; 95%CI:1.41-11.35; p < 0.001). CONCLUSIONS: This study provides evidence that HCV infection is associated with drug use by this population. This information is pivotal for tailoring prevention programs and guiding specific socioeducational measures that aim to reduce or prevent HCV transmission within the prison setting.

ABSTRACT

OBJECTIVE: To describe the prevalence of hepatic steatosis and to assess the performance of biochemical, anthropometric and body composition indicators for hepatic steatosis in obese teenagers. METHODS: Cross-sectional study including 79 adolecents aged from ten to 18 years old. Hepatic steatosis was diagnosed by abdominal ultrasound in case of moderate or intense hepatorenal contrast and/or a difference in the histogram ≥7 on the right kidney cortex. The insulin resistance was determined by the Homeostasis Model Assessment-Insulin Resistance (HOMA-IR) index for values >3.16. Anthropometric and body composition indicators consisted of body mass index, body fat percentage, abdominal circumference and subcutaneous fat. Fasting glycemia and insulin, lipid profile and hepatic enzymes, such as aspartate aminotransferase, alanine aminotransferase, gamma-glutamyltransferase and alkaline phosphatase, were also evaluated. In order to assess the performance of these indicators in the diagnosis of hepatic steatosis in teenagers, a ROC curve analysis was applied. RESULTS: Hepatic steatosis was found in 20% of the patients and insulin resistance, in 29%. Gamma-glutamyltransferase and HOMA-IR were good indicators for predicting hepatic steatosis, with a cutoff of 1.06 times above the reference value for gamma-glutamyltransferase and 3.28 times for the HOMA-IR. The anthropometric indicators, the body fat percentage, the lipid profile, the glycemia and the aspartate aminotransferase did not present significant associations. CONCLUSIONS: Patients with high gamma-glutamyltransferase level and/or HOMA-IR should be submitted to abdominal ultrasound examination due to the increased chance of having hepatic steatosis.

To describe the prevalence of hepatic steatosis and to assess the performance of biochemical, anthropometric and body composition indicators for hepatic steatosis in obese teenagers. METHODS: Cross-sectional study including 79 adolecents aged from ten to 18 years old. Hepatic steatosis was diagnosed by abdominal ultrasound in case of moderate or intense hepatorenal contrast and/or a difference in the histogram ≥7 on the right kidney cortex. The insulin resistance was determined by the Homeostasis Model Assessment-Insulin Resistance (HOMA-IR) index for values >3.16. Anthropometric and body composition indicators consisted of body mass index, body fat percentage, abdominal circumference and subcutaneous fat. Fasting glycemia and insulin, lipid profile and hepatic enzymes, such as aspartate aminotransferase, alanine aminotransferase, gamma-glutamyltransferase and alkaline phosphatase, were also evaluated. In order to assess the performance of these indicators in the diagnosis of hepatic steatosis in teenagers, a ROC curve analysis was applied. RESULTS: Hepatic steatosis was found in 20% of the patients and insulin resistance, in 29%. Gamma-glutamyltransferase and HOMA-IR were good indicators for predicting hepatic steatosis, with a cutoff of 1.06 times above the reference value for gamma-glutamyltransferase and 3.28 times for the HOMA-IR. The anthropometric indicators, the body fat percentage, the lipid profile, the glycemia and the aspartate aminotransferase did not present significant associations. CONCLUSIONS: Patients with high gamma-glutamyltransferase level and/or HOMA-IR should be submitted to abdominal ultrasound examination due to the increased chance of having hepatic steatosis...


Describir la prevalencia de la esteatosis hepática y evaluar el desempeño de indicadores bioquímicos, antropométricos y de composición corporal para identificar la enfermedad en adolescentes obesos. MÉTODOS: Estudio transversal con 79 adolescentes de 10 a 18 años. Se diagnosticó la esteatosis hepática por ultrasonido abdominal en caso de contraste hepatorrenal moderado o intenso y/o diferencia en el histograma ≥7 con relación al córtex del riñón derecho. Se determinó la resistencia a la insulina por el índice Homeostasis Model Assessment-Insulin Resistence (HOMA-IR) para valores >3,16. Los indicadores antropométricos y de composición corporal fueron índice de masa corporal, porcentaje de grasa corporal, circunferencia abdominal y grasa subcutánea. Se dosificaron glucemia e insulina en ayuno, perfil lipídico y las encimas hepáticas aspartato aminotransferasa, alanina aminotransferasa, gama-glutamiltransferasa y fosfatasa alcalina. Se analizaron curvas ROC para evaluar el desempeño de los indicadores en identificar adolescentes con esteatosis hepática. RESULTADOS: La esteatosis hepática estuvo presente en el 20% de los pacientes y la resistencia a la insulina, en el 29%. La gama-glutamiltransferasa y el HOMA-IR se mostraron buenos indicadores para predecir la esteatosis hepática, con punto de corte de 1,06 veces por encima del valor de referencia para la gama-glutamiltransferasa y de 3,28 para el HOMA-IR. Descrever a prevalência da esteatose hepática e avaliar o desempenho de indicadores bioquímicos, antropométricos e de composição corporal para identificar a doença em adolescentes obesos. MÉTODOS: Estudo transversal com 79 adolescentes de dez a 18 anos. Diagnosticou-se a esteatose hepática por ultrassom abdominal em caso de contraste hepatorrenal moderado ou intenso e/ou diferença no histograma ≥7 em relação ao córtex do rim direito. Determinou-se a resistência à insulina pelo índice Homeostasis Model Assessment-Insulin Resistance (HOMA-IR) para valores >3,16. Os indicadores antropométricos e de composição corporal foram: índice de massa corpórea, porcentagem de gordura corporal, circunferência abdominal e gordura subcutânea. OBJECTIVE: The aim of this study was to identify ultrasonographic predictors of esophageal varices (EVs) in children and adolescents with chronic liver disease (CLD) and extrahepatic portal venous obstruction (EHPVO). METHODS: This study evaluates 53 patients younger than 20 years with CLD or EHPVO and no history of bleeding or prophylactic EVs treatment. They were divided into 2 groups: group I (35 with CLD) and group II (18 with EHPVO). Splenorenal shunt (SS), gallbladder wall varices, gallbladder wall thickening (GT), and lesser omental thickness (LOT) were compared with the presence of EVs, gastric varices, and portal hypertensive gastropathy (PHG). Univariate (χ² test, Fisher exact test, and Wilcoxon signed rank test) and multivariate (logistic regression) analyses were performed. The area under the receiver operating curve was calculated. RESULTS: EVs were observed in 48.5% of patients with CLD and in 83.3% of patients with EHPVO. SS (P = 0.0329) and LOT (P = 0.0151) predicted EV among patients with CLD. A median of 5.3 mm of LOT was considered a predictor of EVs among these patients. Multivariate analysis showed SS as an independent predictor of EVs in patients with EHPVO (odds ratio 15). Gallbladder varices (P = 0.0245) and GT (P = 0.0289) predicted EVs among patients with EHPVO. PHG occurred more often among patients with CLD who had SS (P = 0.0384) and greater LOT (P = 0.0226). CONCLUSIONS: SS and a greater LOT were indicative of EV among children and adolescents with CLD. Gallbladder varices and GT were indicative of EVs among patients with EHPVO. SS and a greater LOT were indicative of PHG among patients with CLD.

ABSTRACT

Objetivo: Pelo fato de que o simples diagnóstico de esplenomegalia é importante na faixa etária pediátrica e frequentemente implicar em exames desnecessários e invasivos, o objetivo deste trabalho foi tentar definir o melhor meio de se fazer esse diagnóstico. Fontes de dados: Os dados foram selecionados de sete livros de texto, Uptodate, e de 24 trabalhos indexados na MEDLINE, tendo a maioria deles sido publicada nos últimos 10 anos. Síntese dos dados: A avaliação do baço através do exame físico pode não ser específica em definir suas dimensões e, além disso, possui dificuldades técnicas para sua execução. Há situações em que o baço pode estar rebaixado e, com isso, levar um falso diagnóstico de esplenomegalia e acarretar extensa investigação desnecessária, que poderia ser evitada por um diagnóstico mais acurado. O exame de ultrassonografia é um método relativamente simples, não invasivo e que permite uma avaliação mais objetiva das dimensões do baço, uma vez que na literatura existem gráficos de normalidade para a faixa etária pediátrica, pemitindo assim um diagnóstico mais preciso da esplenomegalia. Conclusões: O diagnóstico preciso da esplenomegalia é muito importante e o exame de USG é o melhor meio de fazer este diagnóstico em crianças. Trata-se de um método cada vez mais disponível em vários níveis de atenção à saúde, não invasivo, que não expõe à radiação e que é relativamente fácil de executar e de repetir...

OBJECTIVE: Prevention is the best treatment for cerebrovascular disease, which is why early diagnosis and the immediate treatment of carotid stenosis contribute significantly to reducing the incidence of stroke. Given its silent nature, 80% of stroke cases occur in asymptomatic individuals, emphasizing the importance of screening individuals with carotid stenosis and identifying high-risk groups for the disease. The aim of this study was to determine the prevalence and the most frequent risk factors for carotid stenosis. METHODS: A transversal study was conducted in the form of a stroke prevention campaign held on three nonconsecutive Saturdays. During the sessions, carotid stenosis diagnostic procedures were performed for 500 individuals aged 60 years or older who had systemic arterial hypertension and/or diabetes mellitus and/or coronary heart disease and/or a family history of stroke. RESULTS: The prevalence of carotid stenosis in the population studied was 7.4%, and the most frequent risk factors identified were mean age of 70 years, carotid bruit, peripheral obstructive arterial disease, coronary insufficiency and smoking. Independent predictive factors of carotid stenosis include the presence of carotid bruit or peripheral obstructive arterial disease [corrected] and/or coronary insufficiency. CONCLUSIONS: The population with peripheral obstructive arterial disease [corrected] and/or coronary insufficiency and carotid bruit should undergo routine screening for carotid stenosis.

RESULTS: EV were observed in 48.5% of group I patients and in 83.3% of group II patients. Low platelet count (p = 0.0015), splenomegaly (p = 0.0003) and splenic index (p = 0.0007) were statistically significant predictors of EV among group I patients. The multivariate analysis showed low platelet count (odds = 21.7) as an independent predictor of EV in patients with chronic liver disease. CONCLUSION: Platelet count, splenic index and platelet-splenic index ratio were predictors of EV in children and adolescents with chronic liver disease. There were no EV predictors among group II patients.

OBJETIVO: Identificar preditores não invasivos de varizes esofágicas em crianças e adolescentes com hepatopatia crônica ou obstrução extra-hepática da veia porta. PACIENTES E MÉTODOS: Estudo transversal que incluiu 53 crianças e adolescentes com hepatopatia crônica ou obstrução extra-hepática da veia porta, sem antecedente de hemorragia digestiva ou tratamento de varizes esofágicas, com até 20 anos de idade. Dois grupos foram formados: grupo I (35 pacientes com hepatopatia crônica) e grupo II (18 com obstrução extra-hepática da veia porta). Foram realizados hemograma, razão normalizada internacional, albumina, bilirrubina total, ultrassonografia de abdome e endoscopia digestiva alta. O índice esplênico foi determinado dividindo a dimensão esplênica pelo valor do limite superior da normalidade. As variáveis foram comparadas quanto à presença ou não de varizes esofágicas através de análise univariada (testes qui-quadrado, exato de Fischer e de Wilcoxon) e multivariada (regressão logística). A acurácia foi determinada a partir da área sob a curva ROC. RESULTADOS: As varizes esofágicas foram observadas em 48,5% dos pacientes do grupo I e em 83,3% do grupo II. Plaquetopenia (p = 0,0015), esplenomegalia (p = 0,0003) e a razão plaquetas/índice esplênico (p = 0,0007) se mostraram indicadores preditivos de varizes esofágicas entre os pacientes do grupo I. Após análise multivariada, a plaquetopenia (odds ratio = 21,7) se manteve como um indicador independente da presença de varizes esofágicas entre os pacientes com hepatopatia crônica. CONCLUSÃO: O número de plaquetas, o índice esplênico e a razão plaquetas/índice esplênico se mostraram preditivos de varizes esofágicas em crianças e adolescentes com hepatopatia crônica. Não foram encontrados preditores de varizes esofágicas entre os pacientes com obstrução extra-hepática da veia porta.

OBJECTIVE: The aim of this study was to evaluate gallbladder motility in constipated children. PATIENTS AND METHODS: A cross-sectional study was conducted on pediatric patients with refractory constipation examined in a pediatric gastroenterology unit of a teaching hospital between January 2005 and August 2006. All of the patients seen during the period were included (age range, 2-16 years). The control group, matched for sex and age, was selected among patients seen at the same basic health units where patients were seen before being referred to pediatric gastroenterologists. A gallbladder contractility index was calculated based on fasting and postprandial gallbladder areas, using an ultrasonographic method. Ultrasonography studies were performed prospectively and blindly by the same physician. Measurements obtained in patients were compared with those obtained in controls. RESULTS: A total of 132 constipated children and the same number of healthy controls were included in the study. Median and mean fasting gallbladder areas were similar in patients and controls. Postprandial gallbladder areas were larger in the patient group, mean +/- SD = 594.8 +/- 206.4 mm, median, 562.5 mm, than in the control group, mean +/- SD = 541.1 +/- 156.1 mm, median, 530.5 mm; P = .031. Logistic regression showed that the risk of a contractility index <25% was 5.2 times greater in patients (odds ratio [OR] 5.2; 95% confidence interval [CI] 2.5-10.81) than in controls. Age was found to be a protective factor (OR 0.79; 95% CI 0.71-0.88). CONCLUSIONS: Gallbladder motility disorder was identified in a group of children with severe constipation. This finding may contribute to the understanding of functional constipation in children.

ABSTRACT

Estudou-se a largura dos dentes anteriores superiores de 121 jovens universitários brasileiros para determinar quais modelos e qual marca de dentes artificiais seriam os mais adequados para esse grupo. As arcadas dentais completas foram moldadas com alginato e vazadas em gesso especial na parte oclusal e gesso pedra, na base. Os dentes não apresentavam nenhum sinal de abrasões anormais, restaurações ou deformidades. A saúde periodontal era boa com mínima ou nenhuma retração gengival, inflamação ou hipertrofia. Os dentes do modelo em gesso e os artificiais tiveram sua dimensão mesio-distal (largura) mensurada com um paquímetro digital. Não foi observada diferença significativa entre os sexos e entre os hemi-arcos direito e esquerdo com relação à largura dos dentes anteriores superiores. Das marcas analisadas, nenhuma foi considerada apropriada para o grupo em questão devido ao fato de possuírem poucas opções de modelos e estes serem fabricados em tamanhos predominantemente maiores (mais largos), o que nos apontou a necessidade de se adaptar ou criar modelos que sejam mais adequados à realidade brasileira.
