ABSTRACT

UNLABELLED: Synechia is the most frequent complication after sinus surgery and has been reported in up to 36% of cases. Several types of materials have been used to reduce the incidence of synechia, including Mitomycin C (MMC). OBJECTIVE: This prospective study aimed to assess the effectiveness of topical MMC in the prevention of synechia after sinus surgery in humans. METHODS: At the end of surgery, MMC solution (1.0 mg/ml) was topically applied randomly to one of the middle meatuses (MMC group) of 14 patients while saline solution was applied to the contralateral meatus (control group). The author remained blind to the medicated side. Synechiae were classified as partial or total. RESULTS: Three patients had middle meatus synechia in the MMC group (21.43%) versus nine (64.29%) in the control group (p = 0.054). In the MMC group, all three middle meatus synechia were partial, while in the control group there were four partial (28.57%) and five total (35.71%) cases of synechia (p = 0.025). CONCLUSIONS: Mitomycin C was not effective in preventing middle meatus synechia, but reduced the probability of total synechia formation.

As sinéquias constituem a complicação pós-operatória mais frequente das cirurgias endoscópicas sinusais, chegando a 36% dos casos. Na tentativa de diminuir esse índice, inúmeros materiais têm sido utilizados, dentre eles a Mitomicina-C (MMC). OBJETIVO: Avaliar a eficácia da MMC tópica na prevenção de sinéquias após cirurgias sinusais em humanos. MÉTODO: Ao final das cirurgias, colocava-se cotonoide com MMC (1,0 mg/ml) em um dos meatos médios (grupo MMC) e outro embebido em solução salina, no meato médio contralateral (grupo controle), por 5 minutos, de forma randomizada e com o avaliador cego em relação aos lados. As sinéquias foram classificadas em parciais, quando não fechavam totalmente os meatos médios, e totais, quando a oclusão era completa. Desenho: prospectivo. RESULTADOS: No grupo MMC, ocorreram três sinéquias (21,43%) versus nove (64,29%) no grupo controle (p = 0,054). No grupo MMC, todas as três (21,43%) foram parciais, enquanto no grupo controle houve quatro (28,57%) parciais e cinco (35,71%) totais (p = 0,025). CONCLUSÃO: A MMC não foi eficaz na prevenção sinéquias como um todo, porém, preveniu a formação de sinéquias totais.

ABSTRACT

Inverted papilloma is a nasal benign tumor that usually arises from the lateral nasal wall, especially from the middle meatus. It has high local invasive likelihood, high recurrence rates and malignancy potential. Sphenoethmoidal recess involvement is rare and is usually due to sphenoid sinus primary disease. In the literature, no case of isolated sphenoethmoidal recess inverted papilloma has been reported yet. The present report describes an exceptional location of inverted papilloma, arising from the sphenoethmoid recess, without involving the sphenoid sinus.

ABSTRACT

O papiloma invertido é um tumor benigno nasal que geralmente se origina na parede lateral da fossa nasal, mais especificamente da região do meato médio. Apresenta grande potencial invasivo local, com alta taxa de recorrência e potencial para transformação maligna. O acometimento do recesso esfenoetmoidal é raro e, quando ocorre, é decorrente de doença primária do seio esfenoidal. Não há relato de papiloma invertido isolado em recesso esfenoetmoidal na literatura. Relatamos um caso de papiloma invertido originado em recesso esfenoetmoidal, sem comprometimento do seio esfenoidal.

ABSTRACT

SUNCT syndrome (short lasting unilateral neuralgiform headache with conjunctival injection and tearing) is defined as short attacks of periorbital unilateral pain and accompanied by ipsilateral lacrimation and redness of the same eye. We present an unusual SUNCT case with bilateral pain that started five years ago after an acute maxillary sinus infection that evolved to chronic sinusitis. This association has been described in few SUNCT cases, but its causal role remains uncertain. The patient was a 58 year old man that fulfilled a headache diary that showed the usual circadian pattern, worsening in the morning and afternoon, and responded to treatment with gabapentine. He was submitted to a functional endoscopic sinus surgery and evolved with milder pain. In a review of 21 patients, 5 had a past medical history of sinusitis, but the causal role of this association remained uncertain.

ABSTRACT

A síndrome SUNCT (short lasting unilateral neuralgiform headache with conjuntival injection and tearing) é definida como curtos ataques de dor periorbital unilateral, acompanhada de lacrimejamento e hiperemia conjuntival ipsilateral. Apresentamos um raro caso de SUNCT com dor bilateral com evolução de cinco anos e iniciado após uma infecção de seio maxilar que evoluiu para sinusite crônica. Esta associação foi descrita em poucos casos de SUNCT, porém pouco esclarecida. O paciente era um homem de 58 anos que preencheu um diário de dor que demonstrou o típico padrão circadiano da síndrome, com pioras matinais e vespertinas, e apresentou melhora com uso de gabapentina. Submetido a cirurgia endoscópica funcional em seio maxilar e evoluiu com modulação da dor, sugerindo um potencial efeito benéfico após tratamento da sinusopatia.Na revisão de literatura encontramos 21 casos de SUNCT bilateral, cinco dos quais apresentavam história de sinusite; no entanto, a relação entre as duas entidades permanece ainda incerta.

ABSTRACT

A surdez súbita é um sintoma cuja etiologia nem sempre é elucidada mesmo dispondo-se de toda propedêutica atual. Neste estudo avaliaremos as alterações encontradas em ressonância magnética de pacientes portadores de surdez súbita. FORMA DE ESTUDO: coorte transversal. MATERIAL E MÉTODO: Estudo prospectivo com realização de RM em 49 dos 61 pacientes com surdez súbita atendidos no pronto socorro de Otorrinolaringologia do Hospital São Paulo, no período de abril de 2001 a maio de 2003. Doze pacientes abandonaram ou não foram submetidos à ressonância magnética por outros motivos. RESULTADOS: 23 (46,9 por cento) pacientes apresentaram alterações à ressonância magnética. Foram encontrados dois tumores sugestivos de meningioma e três schwannomas do oitavo par craniano. Lesões subcorticais e periventriculares esparsas e hiperintensas em FLAIR foram encontradas em 13 pacientes. Cinco (21,7 por cento) pacientes apresentaram alterações periféricas. CONCLUSÃO: A surdez súbita deve ser abordada como um sintoma comum a diferentes doenças. A presença de tumores do ângulo pontocerebelar em 10,2 por cento dos nossos casos, entre outras causas tratáveis, justifica o uso da ressonância magnética com contraste tanto para o estudo do sistema auditivo periférico quanto para o estudo das vias auditivas centrais, incluindo o cérebro.

ABSTRACT

UNLABELLED: The etiology of sudden deafness can remain undetermined despite extensive investigation. This study addresses the value of magnetic resonance imaging in the analysis of sudden deafness patients. STUDY DESIGN: Transversal cohort. MATERIAL AND METHOD: In a prospective study, 49 patients attended at otolaryngology emergency room of Federal University of Sao Paulo--Escola Paulista de Medicina, from April 2001 to May 2003, were submitted to magnetic resonance imaging. RESULTS: Magnetic Resonance abnormalities were seen in 23 (46.9%) patients and revealed two tumors suggestive of meningioma, three vestibular schwannomas, thirteen microangiopathic changes of the brain and five (21.7%) pathological conditions of the labyrinth. CONCLUSION: Sudden deafness should be approached as a symptom common to different diseases. The presence of cerebellopontine angle tumors in 10.2% of our cases, among other treatable causes, justifies the recommendation of gadolinium-enhanced magnetic resonance use, not only to study the auditory peripheral pathway, but to study the whole auditory pathway including the brain.
