ABSTRACT

Introduction: Repetitive transcranial magnetic stimulation (rTMS) is an effective non-invasive cortical stimulation technique in the treatment of neuropathic pain. As a new rTMS technique, intermittent theta burst stimulation (iTBS) is also effective at relieving pain. We aimed to establish the pain-relieving effectiveness of different modalities on neuropathic pain. The study was conducted in individuals with spinal cord injury (SCI) and different modalities of rTMS. Methods: Thirty-seven individuals with SCI were randomly allocated to three groups, in which the "iTBS" group received iTBS, the "rTMS" group received 10 Hz rTMS, and the "iTBS + rTMS" group received iTBS and 10 Hz rTMS successively of the primary motor cortex 5 days a week for 4 weeks, and they all underwent the full procedures. The primary outcome measure was change in the visual analog scale (VAS), and the secondary outcomes were measured using the Hamilton Rating Scale for Depression (HAM-D) and the Pittsburgh Sleep Quality Index (PSQI). All the outcomes were evaluated at 1 day before stimulation (baseline), 1 day after the first week of stimulation (S1), and 1 day after the last stimulation (S2). Results: The VAS scores showed significant pain improvement after 4 weeks of stimulation (p = 0.0396, p = 0.0396, and p = 0.0309, respectively) but not after 1 week of stimulation. HAM-D scores declined, but the decreases were not significant until 4 weeks later (p = 0.0444, p = 0.0315, and p = 0.0447, respectively). PSQI scores were also significantly decreased after 4 weeks of stimulation (p = 0.0446, p = 0.0244, and p = 0.0088, respectively). Comparing the three modalities, VAS, HAM-D, and PSQI scores at S1 showed no differences, and, at S2, VAS scores showed significant differences (p = 0.0120; multiple comparisons showed significant differences between iTBS and iTBS + rTMS, p = 0.0091), while the HAM-D and PSQI scores showed no differences. Discussion: The primary and secondary outcomes all showed significant improvement, indicating that the three different modalities were all effective at relieving the pain. However, not all the three stimulations were of same effectiveness after treatment; there were statistical differences in the treatment of neuropathic pain between iTBS as a priming stimulus and as a single procedure.

ABSTRACT

Early diagnosis is important to reduce the incidence and mortality rate of diabetes. The feasibility of early diagnosis of diabetes was studied via near-infrared spectra (NIRS) combined with a support vector machine (SVM) and aquaphotomics. Firstly, the NIRS of entire blood samples from the population of healthy, pre-diabetic, and diabetic patients were obtained. The spectral data of the entire spectra in the visible and near-infrared region (400-2,500 nm) were used as the research object of the qualitative analysis. Secondly, several preprocessing steps including multiple scattering correction, variable standardization, and first derivative and second derivative steps were performed and the best pretreatment method was selected. Finally, for the early diagnosis of diabetes, models were established using SVM. The first overtone of water (1,300-1,600 nm) was used as the research object for an aquaphotomics model, and the aquagram of the healthy group, pre-diabetes, and diabetes groups were drawn using 12 water absorption patterns for the early diagnosis of diabetes. The results of SVM showed that the highest accuracy was 97.22% and the specificity and sensitivity were 95.65 and 100%, respectively when the pretreatment method of the first derivative was used, and the best model parameters were c = 18.76 and g = 0.008583.The results of the aquaphotomics model showed clear differences in the 1,400-1,500 nm region, and the number of hydrogen bonds in water species (1,408, 1,416, 1,462, and 1,522 nm) was evidently correlated with the occurrence and development of diabetes. The number of hydrogen bonds was the smallest in the healthy group and the largest in the diabetes group. The suggested reason is that the water matrix of blood changes with the worsening of blood glucose metabolic dysfunction. The number of hydrogen bonds could be used as biomarkers for the early diagnosis of diabetes. The result show that it is effective and feasible to establish an accurate and rapid early diagnosis model of diabetes via NIRS combined with SVM and aquaphotomics.

Nasal polyp (NP) is a common chronic inflammatory disease of the nasal cavity and sinuses. Although some authors have suggested that NP is related to inflammatory factors such as interleukin (IL)-1ß, IL-5, IL-8, granulocyte-macrophage colony-stimulating factor (GM-CSF), tumor necrosis factor (TNF)-α, and IL-17, the mechanisms underlying the pathogenesis and progression of NP remain obscure. This study investigated the expression and distribution of IL-17 and syndecan-1 in NP, and explored the roles of these two molecules in the pathogenesis of eosinophilic chronic rhinosinusitis with nasal polyps (Eos CRSwNP) and non-Eos CRSwNP. Real-time PCR and immunohistochemistry were used to detect the expression of IL-17 and syndecan-1 in samples [NP, unciform process (UP) from patients with CRS, and middle turbinate (MT) from healthy controls undergoing pituitary tumor surgery]. The results showed that the expression levels of IL-17 and syndecan-1 were upregulated in both NP and UP tissues, but both factors were higher in NP tissues than in UP tissues. There was no significant difference in IL-17 levels between the Eos CRSwNP and non-Eos CRSwNP samples, and syndecan-1 levels were increased in the non-Eos CRSwNP tissues as compared with those in Eos CRSwNP tissues. In all of the groups, there was a close correlation between the expression of IL-17 and syndecan-1 in nasal mucosa epithelial cells, glandular epithelial cells, and inflammatory cells, suggesting that IL-17 and syndecan-1 may play a role, and interact with each other, in the pathogenesis of non-Eos CRSwNP.
