ABSTRACT

Objective:To analyze the number of appeals volume and causes for complaints received by the government hotline against a hospital in Yangzhou during the pandemic of novel coronavirus pneumonia(hereinafter referred to as COVID-19), so as to provide reference for handling such hotline complaints during pandemics.Methods:Retrospective comparative analysis was made on the " 12345" government hotline work orders received from July 28, 2020 to August 28, 2020(routine prevention and control period) and July 28, 2021 to August 28, 2021(pandemic closure and control period). A descriptive analysis was made on the cause types of complaints and the distribution of departments in question, along with an analysis of the correlation between the cumulative number of cases of pandemic development and the number of complaints using Spearman rank correlation method.Results:The number of work orders for a hospital in Yangzhou during the pandemic control period(659 cases) was 7.7 times higher than that in the routine control period(76 cases). Management problems accounted for 96.7%(637 cases) in the level-1 type of the causes of complaints during the closure and control period of the pandemic, and workflow problems accounted for 90.9%(599 cases) in the level-2 type, which increased by 28.3 and 27.7 percentage points respectively compared with the routine prevention and control period; The highest proportion in the level-3 type of causes for complaints during the closure and control period of the pandemic was administrative management, accounting for 87.9%(579 cases). The departments being complained the most during the pandemic incubation period, outbreak period and recovery period of the pandemic were the fever clinic, oncology department and discharge center respectively. The cumulative number of cases of pandemic development was positively correlated with the number of complaints.Conclusions:During the COVID-19, the handling of the government hotline should be analyzed along with the causes of complaints, focusing on patients′ demands, providing timely feedback, developing collaborative management measures, and achieving accurate policy implementation.

ABSTRACT

Objective:To investigate the efficacy of repeated transcranial magnetic stimulation in the treatment of mixed depression and anxiety.Methods:Sixty-nine patients with mixed depression and anxiety who received treatment in The Second People's Hospital of Lishui from June 2018 to June 2019 were included in this study. They were randomly assigned to receive either conventional anti-anxiety and anti-depression medications (control group, n = 34) or repeated transcranial magnetic stimulation based on conventional anti-anxiety and anti-depression medications (observation group, n = 35) for 4 weeks. Therapeutic efficacy, quality of life, anxiety and depression scores, and adverse reactions were compared between the two groups. Results:Total effective rate in the observation group was significantly higher than that in the control group [97.14% (34/35) vs. 79.41% (27/34) , χ2 = 5.289, P < 0.05]. Before treatment, there were no significant differences in the scores of mental health, physical functioning, general health and role emotional between the two groups (all P > 0.05). After treatment, the scores of mental health, physical functioning, general health and role emotional in each group were significantly increased compared with before treatment (all P < 0.05). In addition, after treatment, the scores of mental health, physical functioning, general health and role emotional in the observation group were significantly higher than those in the control group ( t = 5.982, 5.130, 5.232, 4.829, all P < 0.05). Before treatment, there were no significant differences in anxiety and depression scores between the two groups (both P > 0.05). After treatment, the anxiety and depression scores in each group were significantly decreased compared with before treatment (both P < 0.05). After treatment, the anxiety and depression scores in the observation group were significantly lower than those in the control group ( t = 8.465, 9.290, both P < 0.05). The incidence of adverse reactions in the observation group was significantly lower than that in the control group [8.57% (3/35) vs. 17.64%(6/34), χ2 = 4.138, P < 0.05). Conclusion:Repeated transcranial magnetic stimulation is highly effective in the treatment of mixed depression and anxiety. It can effectively reduce the depression and anxiety symptoms, improve the quality of life, is highly safe, and therefore deserves clinical popularization.

In order to improve the utilization efficiency of nursing human resources, stable nursing troop and enhance the quality of care, at present, the post management experiences for nurses of domestic and international are adopted by many hospitals and certain results are achieved. Study about nurses stratified management is reviewed, the approach, basis, effects, features and the advantages and disadvantages of hierarchical management experiences for nurses in different countries and regions is summarized and some thinking and prospect in studying is proposed in this article, as well as the follow-up research, to explore a constructive policy of further promoting the development of the level management for nurses.

ABSTRACT

Objective To explore the effects of collaboration intervention on self-efficacy and self-perceived burden in stroke patients. Methods 92 stroke patients hospitalized in the neurology department from January to June, 2014 were divided into control group (n=46) and intervention group (n=46). The control group received routine nursing and the intervention group received collaboration intervention in addi-tion. They were assessed with Self-Efficacy for Managing Chronic Diseases and Self-perceived Burden Scale before and after intervention. Results There was no difference in the scores of self-efficacy and self-perceived burden between groups before intervention (t0.05). The score of self-efficacy was more in the intervention group than in the control group (t=3.483, P<0.001) after intervention, and was less of self-perceived burden (t=-5.698, P<0.001). Conclusion The collaboration intervention can improve self-efficacy and relieve self-per-ceived burden in stroke patients.

ABSTRACT

Objective To explore the relationship among nurses′perceptions of patient safety culture, nursing practice environment and the report barrier of nursing adverse event. Methods Totally 513 registered nurses from six hospitals in Zhengzhou were investigated with Patient Safety Culture Assessing Scale, Work Environment Questionnaire and Report Barriers Questionnaire. Results The mean scores of nurses′perceptions of patient safety culture, nursing practice environment and report barriers of nursing adverse events were (3.85±0.55), (3.84±0.53) and (2.47±0.55). The nurses′perceptions of patient safety culture were positively correlated to nursing practice environment (r=0.393,P<0.01), negatively correlated to report barriers of nursing adverse events (r=-0.372,P<0.01). The influencing factors of nurses′perceptions of patient safety culture were colleague relationship, punitive culture, allocation of medical resources, marital status, quality control, significance of report, gender and education, which could explain 33.8 percent for the variances. Conclusions The level of nurses′perceptions of patient safety culture is higher than the average level. The nursing managers should build supportive nursing practice environment and the non-punitive reporting system of nursing adverse events. Cultivate the beliefs and attitudes of nursing safety and focus on the impact of occupational stress to patient safety in order to enhance safety nursing.

ABSTRACT

Objective To investigate the management of prevention and control of catheter-related bloodstream in-fection (CRBSI)in intensive care units (ICUs)of tertiary hospitals in a city,and explore stategies.Methods Data about CRBSI prevention and control as well as quality management were collected and surveyed by self-developed questionnaires.Results Of ICUs in 8 hospitals,the incidence of CRBSI was 4.81 ‰,case infection rate was 5.02‰.6 (75.00%)hospitals had standard operating instruction for the prevention and control of CRBSI, 2 (25.00%)hospitals had maintenance record on central venous catheter,and 2(25.00%)hospitals implemented maximum sterile barrier and catheterization combination strategy,none hospital adopted anti-infective dressing and chlorhexidine bathing.Conclusion There are many problems in the standard operating instruction and implementa-tion of measures for the prevention and control of CRBSI in ICUs of tertiary hospitals in this city,which needs to be improved.

ABSTRACT

Objective To know the awareness level of patient-controlled epidual analgesia in labor related knowledge,and to provide evidence for health education,develop a comprehensive intervention program to reduce the rate of cesarean section.Methods Investigation on knowledge of patient controlled epidural analgesia during labor for a total of 400 pregnant women on Maternal and Child Health Hospital of Taiyuan City,MCH Hospital of Shanxi Province by questionnaire.Compared different characteristics of the pregnant women's scores about control epidural analgesia childbirth knowledge by nonparametric test,knowledge score of multi-factor analysis by multiple linear regression analysis.Results The maximum score of pregnant women-controlled epidural analgesia delivery knowledge was 13,the minimum score was 0,the median score was 4,range interquartile was 5.Different pregnant women with different age,education level,number of weeks of gestation,pregnancy,family income,and condition of parnatal care had different scores of knowledge of patient-controlled epidural analgesia,the difference was significant,P<0.05.Multiple linear regression analysis showed that:pregnant women's education,gestational age,the average monthly household income can be explained by maternal knowledge score of 61.4% of the variation.Conclusions The level of pregnant women-controlled epidural analgesia childbirth knowledge is low,we need to strengthen the natural childbirth education,and take a variety of modes of propaganda and increase their level of knowledge,enhance confidence of natural childbirth in pregnant women,so as to achieve the purpose of reducing cesarean section rate.

ABSTRACT

Objective To explore the therapeutic efficacy of three different treatment regimens for conservative treatment of ec‐topic pregnancy(EP) .Methods Clinical data of 198 cases of patients with EP who were conservatively treated in this hospital from June 2012 to June 2014 were retrospectively analysed .All patients were divided into group A (39 cases) ,group B(92 cases) and group C(67 cases) ,which were treated with single‐dose of methotrexate(MTX) ,MTX combined with mifepristone and multiple‐dose of MTX respectively .The curative rate and time for human chorionic gonadotropin beta‐subunit(β‐HCG) dropping to normal were observed and recorded .Results The total curative rates in three groups were statistically different (P3 000 mIU/mL ,the curative rates were statistically different (P<0 .05) .Conclusion Single‐dose of MTX has relative low curative rate .It is recommend to use combination regimens or multi‐ple‐dose of MTX regimens ,and for patients with high level of β‐HCG multiple‐dose of MTX regimens might be the best choice .

Objective To establish the nurse layer classification standard of Henan province.Methods Nursing experts were selected within the scope of Henan province,nursing layer classification standard system was established through two rounds of experts enquiry with Delphi method.Results According to two rounds of consultation,the index system was divided into five layers:the base layer,growth layer,the professional layer,backbone layer and the experts level,which corresponded to the five classes:N0 assistant nurses,N1 registered nurse,N2 primary nurse,N3 specialized nurses/nursing managers,N4 clinical nursing specialist/nursing researchers/nursing management experts.Various levels of nurses had corresponding requirements and responsibilities framework.The various levels of nurses had corresponding requirements.Conclusions This study built in Henan province has good concentration,such as the representation of the expert advice and high credibility,which can provide decision-making reference for the nurse station management of Henan province.

ABSTRACT

Objective To summarize experience in treating patients with native value endocarditis ( NVE) , and discuss the effect of timing of surgery on the infection control , cardiac function recovery and long-term mortality among patients . Methods Eighty NVE cases with antibiotic treatment were recruited between January 2006 and May 2013 .According to the timing of surgery, the subjects were divided into the early surgery group (38 cases) and the late surgery group (42 ca-ses) .The infection-related markers and cardiac function indexes before and after surgery , and 1-year, 3-year and 5-year mortality after surgery were analyzed .Results The infection control and recovery of cardiac function in the early surgery group were better than in the late surgery group (P<0.05).The difference in 3-year and 5-year mortalities between the two groups was significant .Conclusions For NVE patients with routine antibiotics treatment , the surgical effect and long-term survival rate are much better in the early surgery group than in the late surgery group .

ABSTRACT

Objective To evaluate the structure and function of left ventricle in diabetes mellitus (DM) patients without complications by myocardial velocity gradient (MVG) measured by myocardial velocity profile (MVP). Methods Thirty type 2 DM patients without complications and 30 healthy volunteers as controls were enrolled. The heart structure, systolic function and diastolic function of left ventricle were measured by echocardiography and the left ventricular mass index (LVMI) was calculated. Mitral annular systolic movement (Sm) , early rapid filling phase movement ( Em) and atrial contraction movement(Am) were measured by tissue Doppler image and the left ventricular MVG at diastole and systole in subendocardium and subepicardium (MVGs,MVGd) were measured by MVP. Results The diameter of left atria and left ventricle, thickness of interventricular septum and LVMI were higher in DM group than those of control group ( P <0. 05 or P <0. 01) , MVGs, MVGd, and Em were lower in DM group than those of control group( P <0. 05 or P <0. 01). There were no significant differences on E/A, Em/Am and E/Em between two groups. In addition, there were also no significant differences on Sm,left ventricular ejection fraction and left ventricular fraction shortening between two groups. Conclusions Structure and function of left ventricle have changed in patients of DM without complications. MVG measured by MVP is an accurate and sensitive index to assess left ventricular systolic and diastolic function.

@#ObjectiveTo observe the effects of Balance Performance Monitor (BPM) on balance capacity of children with spastic cerebral palsy. Methods96 children (2~5 years old) with spastic cerebral palsy were randomly divided into the experimental group and the control group. BPM training combined with Bobath approach was used in the experimental group, and the Bobath approach was used in the control group. They were assessed with the BPM before and 2 months after training. Results2 months after training, the balance capacity in both groups improved than before(P<0.05). Furthermore, the index of experimental group is better than those of the control group(P<0.05). ConclusionBPM can improve the effects of Bobath approach on the balance capacity of the children with spastic cerebral palsy.

ABSTRACT

AIM: To study the survival, transfer and distribution of bone marrow CD34+/CD45+ cells from transgenic GFP mouse after transplanted into the completed transversional spinal cord rat model. METHODS: The bone marrow cells isolated from transgenic GFP mice were cultured in vitro. The cultured cells were identified by anti-CD34 and anti-CD45 monoclonal antibodies, and were transferred into the end of transection spinal cord. Paraformaldehyde was infused into the left ventricle of the rat model at the 24 h, 48 h, 1 week, 2 weeks, 4 weeks and 8 weeks after cell transplantation. Through sank and frozen, the spinal cord was sectioned at 10 ?m thickness. The green fluorescence positive cells were observed under the fluorescence microscope. CD34+/CD45+ cells were identified by immunohistochemistry staining. RESULTS: Green fluorescence positive cells were found at the head and the end of the completed transection part of spinal cord. Most of the green fluorescence positive cells were distributed in the gray substance of spinal cord. CD34+/CD45+ cells were found by immunohistochemistry staining. CONCLUSION: CD34+/CD45+ cells survived in spinal cord of SD rat, and migrated to the head of the transection part. The distance of migration was extended by the time.
