ABSTRACT

A unique finding of an extraskeletal pseudomalignant osseous lesion of the mesentery and omentum was detected in a 63-yr-old insulin-dependent diabetic man, following aortic bifemoral bypass and two subsequent laparotomies at 2-wk intervals for intractable intestinal obstruction. An analogy is made to myositis ossificans and fasciitis ossificans. The histogenesis of new bone formation from connective tissue following chemical or physical stimuli supports a metaplastic mechanism. Our review of the literature reveals no prior report of a similar case.

ABSTRACT

A case of composite lymphoma consisting of an anaplastic large-cell Ki-1 lymphoma and a small-cell follicular lymphoma was found in the splenic hilar lymph node of a 66-year-old woman. The Ki-1 lymphoma showed monoclonal IgM-lambda and CD 20, CD 74, and CDw 75 antigens by immunostaining and CD 19, CD 20, CD 22, and lambda antigens by flow cytometry. The follicular lymphoma also showed monoclonal IgM-lambda, and CD 20 and CDw 75 antigens but not CD 74 and CD 30 (Ki-1) by immunostaining. Flow cytometric analysis of the follicular lymphoma component was not conclusive, as it was impossible to separate the neoplastic from the normal small B lymphocytes. Ki-1 lymphoma usually is seen in childhood and is mostly of T cell origin. It is, therefore, unusual to find Ki-1 antigen component in a composite lymphoma of B-cell origin in an adult. However, there has been evidence to suggest that B-cell Ki-1 lymphoma may be related to follicular lymphoma. Thus, our case may represent a follicular lymphoma transforming into a Ki-1 lymphoma. Immunogenotyping in this case revealed that the two components were probably of the same clonal origin, as they seemed to share the same light chain gene. The presence of rearrangement in the switch region of the IgH in our case without the actual occurrence of heavy chain switching may have triggered somatic recombination in the IgH complex. This series of events may have led to the transformation of a low-grade lymphoma into a high-grade lymphoma, accounting for the two morphologic patterns seen in our bimorphic lymphoma.

ABSTRACT

The prophylactic Papanicolaou Test contributes in diagnosing dysplasias and carcinoma in situ of the uterine cervix with a realistic 100% cure. Surgery and radiotherapy improved considerably quantity and quality of life in stages 1 and 2 lesions. However, stages 3 and 4 have shown little improvement in results. Systemic treatment with chemotherapy offers hope for control. Most of the drugs are ineffective against squamous cell carcinoma of the cervix. Experimental and clinical trials with CIS-platinum (2) diamminedichloride (DDP) show effectiveness against cervical carcinoma. In an effort to individualize the treatment, a chemosensitivity testing (CST) is applied analogous to the radiosensitivity testing (RST). Basically, a control biopsy and posttest dose of DDP biopsies are studied to evaluate the response using objective morphological criteria. This safe, relatively simple, clinically applicable approach offers an objective dimension in selecting the patients and in monitoring the treatment. An important observation that gives this study an original authenticity is the fact that radiation and chemotherapy affects the cells in an identical way. This similar morphological reaction most probably means an analogous biological expression of cellular injury and necrosis of the neoplastic cells.

ABSTRACT

A relatively simple, clinically applicable, analytic-pathologic approach to the complex problem of carcinoma of the uterine cervix is given. Careful study of control and post-test radiation cervical biopsies and evaluation of the type and grade of the carcinoma, the lymphatic vessel involvement, the stromal host reaction, and the tumor response to test radiation offer additional parameters in the therapeutic approach, aiming at the improvement of prognosis. We also hope that this work offers a contribution to the complex problem of tumor biology and pathology.

ABSTRACT

A rare case of malignant struma ovarii is presented. The tumor was excised together with the ovary and interpreted as a papillary strumal adenocarcinoma arising in a cystic teratoma with predominant thyroid elements. Prior to surgery the patient did not show any symptoms of thyroid hyperfunction. The thyroid function tests performed subsequent to surgery were within normal limits. Periodic examinations of the patient over a period of three years did not reveal any abnormalities. Statistically, this type of malignancy carries a good prognosis with a high "cure" rate.
