ABSTRACT

OBJECTIVE: To study the features of clinical and laboratory parameters of rhinogenic complications of the orbit (RCO) and eyelids depending on the blood leukocyte shift index (LSI) to create a predictive model in pediatric patients. MATERIAL AND METHODS: The study included 50 patients who were treated at the Regional Clinical Hospital No. 2 of Tyumen with inflammatory pathology of the paranasal sinuses. Group I with RCO - reactive edema of the eyelids and orbital tissue included 29 (58.0%) patients (of which 16 (32.0%) were boys, 13 (26.0%) were girls). In group 2 with ROC, purulent-septic complications of the eyelids and orbit included 21 (42.0%) patients (of which 10 (20.0%) were boys, 11 (22.0%) were girls). RESULTS: LSI values in the general age group (n=50) from 1 to 17 years old were: 1.61 [1.40; 1.82] in patients of group 1; 3.45 [2.96; 3.94] in patients of group 2 (p≤0.05). With an index of LSI from 1.36 to 1.96, the development of reactive edema of the eyelids and orbital tissue is predicted, from 3.14 to 4.72 - the development of purulent-septic complications of the eyelids and orbit in patients of preschool and primary school age. CONCLUSION: The marker of clinical and laboratory parameters of the severity of the disease is the LSI indicator, taking into account the age of the child, which can be used in the early diagnosis of purulent-septic rhinogenic complications of the orbit and eyelids in children.

ABSTRACT

The article analyzes the problems of population aging that are relevant for the world civilization, the peculiarities of changes in the body in old age and senility, in particular, the nasal cavity and paranasal sinuses. The analysis of algorithms of actions of an otorhinolaryngologist when prescribing drugs for the treatment of inflammatory diseases of the nasal cavity and paranasal sinuses in elderly patients is given. The advantages of using a combined nasal spray containing a nasal decongestant of 0.1% xylometazoline and 5% dexpanthenol as a means that improves regeneration and reduces the cytotoxic effect of xylometazoline are reflected. The research of domestic and foreign authors testifying to the rationality of using this combined drug in the practice of an otorhinolaryngologist in the treatment of pathology of the nasal cavity and paranasal sinuses in elderly people is presented.

ABSTRACT

The use of antibacterial drugs has become widespread over the past 10-15 years. In this regard, there is a significant increase in antibiotic resistance in the world. Combating this problem has been one of the priorities of WHO since 2015. In light of this situation, the use of antimicrobials with no antibiotic resistance is of paramount importance. Topical antimicrobial peptides are used to treat infectious skin and mucosal diseases. The review highlights the role of the antimicrobial polypeptide thyrotricin and the complex drug doritricin in the treatment of pharyngeal pathology in adults and children according to foreign and domestic literature. The obtained data indicate the effectiveness and safety of a topical complex preparation containing thyrotricin, benzalkonium chloride and benzocaine (doritricin) as a monopreparation in the treatment of viral and bacterial tonsillopharyngitis, as well as in the complex therapy of streptococcal tonsillopharyngitis.

ABSTRACT

The objective of the present study was to elaborate the program for differential diagnostics of acute pharyngeal diseases based on the 'ENT-Neuro' artificial neuronal network. The study group was formed by means of sampling patients with acute pharyngeal diseases from a set of case histories of the subjects presenting with acute inflammatory diseases. The data thus obtained were employed to develop the expert system to support the decision making process with the use of the 'ENT-Neuro' artificial neuronal network that allows to carry out diagnostics of various inflammatory diseases of the pharynx including the following nosological entities: paratonsillitis, parapharyngitis, acute tonsillitis, and acute pharyngitis, with the minimal probability of erroneous diagnosis (4%). The proposed expert system makes it possible to choose the optimal treatment strategy for the management of various pharyngeal diseases taking into consideration the severity of a concrete pathology and thereby to reduce to a minimum the risk of the related complications.
