ABSTRACT

OBJECTIVE: To investigate the prevalence of psychiatric disorders in patients with myasthenia gravis (MG). METHOD: Forty-one patients with MG answered to a structured psychiatric interview (MINI-Plus). RESULTS: Eleven (26.1%) patients were diagnosed with a depressive disorder and 19 (46.3%) were diagnosed with an anxiety disorder. Patients with dysthymia were older (p=0.029) and had longer disease duration (p=0.006). Patients with social phobia also had longer disease duration (p=0.039). CONCLUSION: Psychiatric disorders in MG are common, especially depressive and anxiety disorders.

OBJETIVO: Investigar a prevalência de transtornos psiquiátricos em pacientes com miastenia gravis (MG). MÉTODO: Quarenta e um pacientes com MG responderam a uma entrevista psiquiátrica estruturada (MINI-Plus). RESULTADOS: Onze (26,1 por cento) pacientes foram diagnosticados com transtorno depressivo e 19 (46,3 por cento) foram diagnosticados com um transtorno de ansiedade. Pacientes com distimia eram mais velhos (p=0,029) e tinham maior tempo de doença (p=0,006). Os pacientes com fobia social também tiveram maior tempo de doença (p=0,039). CONCLUSÃO: Transtornos psiquiátricos na MG são bastante comuns, principalmente transtornos depressivos e de ansiedade.

ABSTRACT

Bipolar disorder may be overrepresented in multiple sclerosis (MS) patients. Although research in this area is limited, studies assessing the nature of this association have focused on genetic aspects, adverse reaction to drugs and brain demyelinating lesions. Herein we report three patients with MS that also presented bipolar disorder. The coexistence of neurological and psychiatric symptoms in most MS relapses highlights the relevance of biological factors in the emergence of mood disorders in these patients.


O transtorno bipolar do humor parece estar super-representado em pacientes com esclerose múltipla (EM). Apesar de a pesquisa nessa área ser limitada, estudos acerca da natureza dessa associação têm se concentrado em aspectos genéticos, reações adversas a drogas e lesões desmielinizantes no sistema nervoso central. Relatamos, aqui, três casos de pacientes com EM que também apresentaram transtorno bipolar do humor. A coexistência de sintomas neurológicos e psiquiátricos em grande parte das recaídas de EM reforça a importância de fatores biológicos na gênese dos sintomas afetivos nesses pacientes.

ABSTRACT

OBJECTIVE: To evaluate compliance in children with acute lymphoblastic leukemia. METHOD: Compliance was assessed through specific interviews. RESULTS: A total of 73 patients who had concluded the maintenance phase of chemotherapy were enrolled on the study. Eighty-one percent of the interviews were conducted with the patients' mothers; 92% of the families stated that medical instructions had been understood well. Interviews indicated that 27% of the patients did not receive their medication twice or more during the maintenance phase, without medical direction for this. These children were considered non-compliant. Sixteen percent of the children failed to receive their medication three times or more. The main reason for non-compliance was forgetfulness. In ten cases the reported dosage of drugs was not that which was prescribed. No significant associations of non-compliance with parents' schooling level, number of family members or per capita family income were detected. The 8.5-year estimated probability of event free survival was 72.4% (95% CI: 59.2-82.3). The event free survival curves for non-compliant children were not statistically different from those for the compliant group. CONCLUSIONS: Results suggest that comprehensive approaches to the problem of non-compliance are urgently needed.

ABSTRACT

OBJETIVO: Avaliar a adesão ao tratamento em crianças com leucemia linfoblástica aguda. MÉTODO: A adesão foi avaliada através de questionários específicos. RESULTADOS: Foram estudados 73 pacientes com idade inferior a 18 anos que atingiram a fase de manutenção da quimioterapia. A mãe foi a informante em 81 por cento das entrevistas; 92 por cento dos informantes entendiam bem as orientações médicas. Em 27 por cento dos casos, houve falha na adesão ao tratamento quando considerada a não-administração da droga sem orientação médica por ao menos duas vezes durante a fase de manutenção; em 16 por cento dos casos, constatou-se falha quando considerada a não-administração da droga por três vezes ou mais. O principal motivo para a não-administração foi "esquecimento". Em 10 casos, a dose informada não correspondia à prescrita. Não se observou associação entre falhas na adesão e escolaridade do responsável, número de membros da família, ou renda familiar per capita. A probabilidade estimada de sobrevida livre de eventos para todo o grupo aos 8,5 anos foi de 72,4 por cento (IC 95 por cento: 59,2-82,3). Não foi observada diferença na sobrevida livre de eventos quando comparados os grupos com falhas na adesão com o grupo sem falhas. CONCLUSAO: Esses resultados apontam a necessidade de elaboração de programas e intervenções urgentes que proponham uma abordagem mais ampla do problema da não-adesão ao tratamento.
