ABSTRACT

Pig islets are an alternative source for islet transplantation to treat type 1 diabetes (T1D), but reproducible curative potential in the pig-to-nonhuman primate (NHP) model has not been demonstrated. Here, we report that pig islet grafts survived and maintained normoglycemia for >6 months in four of five consecutive immunosuppressed NHPs. Pig islets were isolated from designated pathogen-free (DPF) miniature pigs and infused intraportally into streptozotocin-induced diabetic rhesus monkeys under pretreatment with cobra venom factor (CVF), anti-thymocyte globulin (ATG) induction and maintenance with anti-CD154 monoclonal antibody and low-dose sirolimus. Ex vivo expanded autologous regulatory T cells were adoptively transferred in three recipients. Blood glucose levels were promptly normalized in all five monkeys and normoglycemia (90-110 mg/dL) was maintained for >6 months in four cases, the longest currently up to 603 days. Intravenous glucose tolerance tests during the follow-up period showed excellent glucose disposal capacity and porcine C-peptide responses. Adoptive transfer of autologous regulatory T cells was likely to be associated with more stable and durable normoglycemia. Importantly, the recipients showed no serious adverse effects. Taken together, our results confirm the clinical feasibility of pig islet transplantation to treat T1D patients without the need for excessive immunosuppressive therapy.

ABSTRACT

This study considers the characteristics of carbon monoxide (CO), nitrogen dioxide (NO(2)), ozone (O(3)) and sulfur dioxide (SO(2)) in two major South Korean cities, including the capital city of Seoul, over a time period of 7-8 years. Changes in the annual mean and percentiles of the daily 1-h maximum and other hour-based concentrations varied according to the compound and city type. Seasonal variations varied according to the compound, yet not with the city type. Both Seoul and Taegu exhibited lower O(3) concentrations in July compared to other summer months. There was a high degree of correlation between the daily 1- and 8-h maximum or daily mean concentrations of all compounds in both cities, with an R(2) of 0.66-0.90 at p<0.0001. It was indicated that for CO and O(3), the 8-h standard was more stringent than the 1-h standard, while for NO(2) and SO(2), the 1-h standard was more stringent than the 24-h standard. The correlation coefficients between the daily 1-h maximum and daily mean concentrations decreased as the maximum concentration values of NO(2), O(3 ), and SO(2) increased in the two cities. For all the target compounds, Seoul recorded a substantially higher frequency of days with concentrations above the relevant 1-, 8-, and 24-h standards compared to Taegu.

ABSTRACT

The oscillatory potentials (OPs) in electroretinogram (ERG) have clinical values in measuring retinal functions of the early stage of diabetic retinopathy. However, OPs have, until now, been evaluated as components of total ERG recorded with a full field stimulus over the entire retina. Therefore, little information could be obtained about the macular condition (diabetic retinopathy). Using focal stimuli, we successfully recorded OPs in the human macular region. We modified our previously reported system for recording local macular ERG under the fundus monitor with an infrared television fundus camera. We evaluated macular OPs with simultaneously recorded a- and b-waves in many patients with diabetic retinopathy. In some kinds of early diabetic maculopathy, the macular OPs were selectively reduced, leaving the a- and b-waves intact. The macular OPs can be a valuable indicator in assessing the macular function in diabetic maculopathy.
