ABSTRACT

PURPOSE: The purpose of this study was to clarify whether the timing of initial surgery for infantile esotropia contributes to better sensory outcomes and to the severity of dissociated vertical deviation (DVD). METHODS: This retrospective study examined 55 children, aged 8 years and younger, who underwent esotropia surgery. Based on age at the time of the surgery, patients were divided into the very early surgery group (birth to 8 months, N = 14), early surgery group (9-24 months, N = 23) and late surgery group (25 months and older, N = 18). Sensory and motor outcomes of the three groups were statistically evaluated. RESULTS: No significant differences in the incidences of DVD were noted among the three groups at the final visit. However, all DVDs of the very early surgery group were latent, whereas 38.9% of the DVDs for both near and distance of the late surgery group were manifest. Significant differences were found among the three groups for the proportion of patients with manifest DVD, latent DVD and without DVD, and for the incidence of additional surgery for manifest DVD (p < 0.05, G-test). CONCLUSION: Early surgery for infantile esotropia decreases the severity of DVD and lowers the need for an additional operation for DVD.

ABSTRACT

The case of a 58-year-old woman with a primary adenocarcinoma of the duodenojejunal junction and an extraluminal growth is reported. The patient's chief complaint had been lumbago and emaciation, and a tumor in her left upper abdomen was found. Hypotonic duodenography showed a filling defect with ulceration of the duodenojejunal junction. Angiography revealed irregular vessels and a slight tumor stain. A CT scan further revealed that this tumor had invaded into the abdominal aorta with paraaortic lymphadenopathies.

ABSTRACT

SM-4300, a newly developed human immunoglobulin preparation for intravenous use, has been evaluated clinically in 11 patients with severe bacterial infections in the combination therapy with antibiotics in the surgery. SM-4300 was administered to the patients, for whom antibiotics alone had not been effective, at a dosage of 2.5 or 5 g/day with continuous use of the same antibiotics. The results obtained were as follows: Clinical effects of SM-4300 were excellent in 1 case, good in 2, fair in 4, poor in 3 and unknown in 2. The efficacy rate was summarized as 30.0%, and the rate including fair response was 70.0%. No subjective and objective clinical side effect was observed. No abnormal laboratory finding was observed. Clinical usefulness of SM-4300 were usefulness in 3 cases, fairly usefulness in 2, useless in 3 and unevaluable in 4. The usefulness rate was 37.5%, and the rate including fairly usefulness was 62.5%. In conclusion, combination therapy with SM-4300 and antibiotics was considered to be safe and effective against severe bacterial infections in the surgery.
