ABSTRACT

PURPOSE: To construct the comfort status scale for patients with lung cancer after thoracoscopic surgery. DESIGN: Delphi method inquiry to 15 clinical and nursing experts. METHODS: On the basis of the comfort status scale and the subjective experience and objective symptoms of patients with lung cancer after thoracoscopic surgery, the relevant literature was consulted, semistructured interviews and group discussions were conducted, the pool of items of the postoperative comfort status scale for patients with lung cancer was initially formed, and the postoperative comfort status scale for patients with lung cancer was finally established. FINDINGS: The positive coefficient of experts was 100%, the coefficient of authority was 0.92 and 0.93, and the Kendal's W was 0.257 and 0.298, the degree of coordination of expert opinions was statistically significant (P < .05). Finally, a total of 28 items in four dimensions were formed to assess the postoperative comfort status of patients with lung cancer after thoracoscopic surgery. CONCLUSIONS: The Delphi method-based comfort status scale for patients with lung cancer after thoracoscopic surgery is scientific and reliable, and can provide a quantitative basis for the evaluation of the comfort status of patients after lung cancer thoracoscopic surgery, to further provide individual comfort care measures.

ABSTRACT

BACKGROUND: Lung cancer is one of the most common malignant tumours that threaten human health globally. Radical resection under thoracoscopic guidance has been accepted as the major therapeutic option for treating lung cancer clinically. However, the procedure still has some adverse impacts on the comfort of patients following thoracoscopic surgery. OBJECTIVE: To analyse the reliability and validity of the postoperative comfort scale for patients with lung cancer undergoing endoscopic surgery and to evaluate patient comfort. METHODS: With 210 patients with lung cancer undergoing endoscopic surgery as the participants, this study was performed to assess the reliability and validity of the postoperative comfort scale for patients with lung cancer undergoing endoscopic surgery, with the assessment performed by eight experts. RESULTS: The postoperative comfort scale included 28 items and consisted of four dimensions (physiological, psychological, socio-cultural and environmental). The total Cronbach's alpha coefficient of the scale was 0.851, and the split-half reliability coefficient was 0.875. Meanwhile, the content validity index (CVI) was 0.875∼1, and the scale-level average CVI was 0.99. The Chi-square/degree-of-freedom ratio of construct validity was 2.844, suggesting a good model-fitting. Furthermore, the overall average score of patient comfort was 3.72 ± 0.57, with scores ranging between 3.59 ± 0.71 and 3.83 ± 1.06 across all four dimensions, with the lowest score in the physiological dimension. CONCLUSION: The postoperative comfort scale has good reliability and validity and can be applied for the postoperative comfort assessment of patients undergoing endoscopic surgery for lung cancer. Overall, the degree of patient comfort in this assessment was moderate, meaning targeted measures may be required to further improve patient comfort, especially in the physiological dimension.

ABSTRACT

To establish a method for the determination of three volatile components: menthone, menthol and pulegone in Yinqiao powder (YQP) decoction, explore the change rules of volatile components in decocting process, and provide evidence for elucidating the scientific connotation of its traditional decocting method "taking when the fragrance is volatilized fiercely". YQP decoctions with different decocting time were prepared, and GC-MS was used to qualitatively analyze the volatile components and determine the contents of menthone, menthol and pulegone in decoctions. Then the effects of different decocting time on contents of volatile components were investigated. The results showed that the volatile components in YQP decoctions mainly come from Menthae Haplocalycis Herba, Schizonepetae Herba and Forsythiae Fructus. With the extension of decocting time, the concentrations of all the above 3 volatile components in Yinqiao powder decoction were first increased and then decreased. When soaking for 30 minutes, as well as boiling for 0, 5, 10, 15, 20, 30, 40, 50, 60 minutes, the concentrations of menthone in YQP decoction were 0.058, 0.268, 0.216, 0.073, 0.065, 0.048, 0.048, 0.041, 0.038, 0.034 mg•L ⁻¹; the concentrations of menthol were 0.965, 2.847, 3.633, 2.420, 1.539, 1.189, 1.273, 1.188, 0.905, 0.663 mg•L ⁻¹; the concentrations of pulegone were 0.355, 0.522, 0.598, 0.477, 0.374, 0.374, 0.339, 0.355, 0.248, 0.251 mg•L ⁻¹; and the total concentrations were 1.377, 3.637, 4.446, 2.970, 1.979, 1.611, 1.660, 1.583, 1.191, 0.947 mg•L ⁻¹, respectively. The results showed that the contents of menthone, menthol and pulegone in YQP decoctions were heavily influenced by the decocting time. The fragrance was volatilized fiercely at about 5 minutes after boiling, with larger concentrations of the above three volatile components in decoction; the fragrance got weak after 15 minutes of boiling, the concentrations of menthone, menthol and pulegone in YQP decoctions were significantly decreased, indicating that the traditional decocting method "taking when the fragrance is volatilized fiercely" has some scientific foundation.

ABSTRACT

OBJECTIVE: To explore the causes of chronic cough in children and its outcome after treatment of chronic rhinosinusitis (CRS). METHOD: Fifty-six cases of children who were previously diagnosed as chronic cough by pediatrician with poor treatment outcomes, were made a definite diagnosis of CRS and then treated by otolaryngologist. RESULT: Fifty-six cases of children were made a definite diagnosis of CRS according to CT and/or nasal endoscopy examination. The cardinal symptom was cough in all cases, in which 35 cases (62.5%) with nasal congestion and rhinorrhea, 15 cases (26.8%) with snoring and mouth breathing, 6 cases (10.7%) with postnasal drip and expectoration. Among all the patients, stick purulent secretion in nasal cavity and posterior pharyngeal wall was found in 45 cases (80.4%), nasal polyps in 5 cases (8.9%), adenoidal hypertrophy in 21 cases (37.5%), nasal polyps and adenoidal hypertrophy in 4 cases (7.1%), allergic rhinitis in 19 cases (33.9%). After the combined treatment of CRS, 45 cases (80.4%) were cured, 9 cases (16.1%) relieved and 2 cases (3.6%) ineffective. CONCLUSION: Chronic cough is the commonly cardinal symptom of CRS in children and it can be cured or relieved by combined treatment of CRS.
