ABSTRACT

The exact mechanism of a ketogenic diet (KD) as a suitable alternative therapeutic approach for drug-resistant epilepsy (DRE) in alleviating seizures is not yet fully understood. The present study aimed to evaluate the role of the KD in reducing oxidative stress (OS) by increasing the ketone body beta-hydroxybutyrate (BHB) and Arachidonic acid (ARA), an essential polyunsaturated fatty acid, as a possible mechanism in relieving seizure attacks in children with DRE. Forty children with refractory epilepsy were included in the present study. The serum levels of BHB, ARA, and OS markers, malondialdehyde (MDA), and 8-hydroxyl-deoxyguanosine (8-OHdG), were evaluated in children with DRE and compared before and after the three months of KD therapy. Thirty-four of 40 included children could complete the three-month KD therapy. Twenty-one (61.76%) patients had more than a 50% reduction in seizure frequency after the KD (responders). The remaining 13 children were considered non-responders to the diet. The serum levels of ARA and BHB significantly (p < 0.05) increased after the KD therapy. The serum levels of OS parameters MDA and 8-OHdG before the diet therapy were significantly (p < 0.05) higher than those after the administration. The serum levels of BHB and MDA after the KD therapy in the responders were respectively higher and lower than those in the non-responders (p < 0.001). Ketogenic diet might reduce brain OS by increasing BHB and ARA. The role of BHB in diminishing OS and seizure might be more remarkable than ARA.

ABSTRACT

BACKGROUND: COVID-19 virus is on the rise globally, and people with the disease experience a variety of physical and mental problems. According to studies, depression can be a complication of the virus. So far, limited measures have been taken to prevent and treat emotional-psychological complications of COVID-19. The aim of this study was to evaluate the effect of online cognitive behavioral therapy (CBT) on depressive symptoms in recovered patients with COVID-19. MATERIALS AND METHODS: This study was conducted in a quasi-experimental design. A sample of 150 recovered patients with COVID-19 who referred to Imam Reza Hospital in Tabriz city, were selected by random sampling method. The patients' emotional-psychological status was assessed by the Beck Depression Inventory, and they entered the treatment process according to the inclusion criteria. As a therapeutic intervention, a scientific and valid approach called CBT was implemented, which two occupational therapists performed in eight treatment sessions in the context of a social communication application. Paired sample t-test was used to analyze the time sequence of quantitative data due to its normality. RESULTS: After the last session of the intervention, the effect of this therapeutic approach on reducing the depressive symptoms of patients was evaluated. According to the findings, the Beck score of the participants decreased significantly after the intervention (MD = -45/9, P < 0/001). CONCLUSION: Therefore, in order to promote the recovery of patients with COVID-19 and help health authorities to develop preventive measures and effective treatment of emotional-psychological problems caused by this virus, it is recommended to use the online CBT approach.

BACKGROUND: Scientific misconduct is a prevalent phenomenon with many undesirable consequences. In Iran, no original research have been done about scientific fraud. So, this study aimed at describing a challenging research misconduct in Iran, its related causes, and the ways Iranian authorities deal with it. METHODS: In this cross-sectional study, through a two-year period, all the advertisements installed in the study sites were collected and the content analysis was performed. Semi-structured interviews were held with experts for discovering the causes of misconduct. Also, published issues were collected for review of the laws on confronting the fraud in Iran. RESULTS: The content analysis resulted in identifying four categories of misconduct issues: advertising approach, types of services, outcome guarantee, and justifying the academic credit. Besides, reviewing the related literature indicated that Iranian government and the responsible authorities have recently established serious penalties for dealing with scientific misconduct through legislation. CONCLUSIONS: This study revealed some misconduct in scientific activities which has persuaded the authorities to enforce strict rules to deal with it. The effectiveness of this legislation needs to be investigated in some further studies.
