ABSTRACT

This study focuses on optimizing and designing the Delayed-Fix-Later Awaiting Transmission Encoding (DEFLATE) algorithm to enhance its compression performance and reduce the compression time for models, specifically in the context of compressing NX three-dimensional (3D) image models. The DEFLATE algorithm, a dual-compression technique combining the LZ77 algorithm and Huffman coding, is widely employed for compressing multimedia data and 3D models. Three 3D models of varying sizes are selected as subjects for experimentation. The Wavelet algorithm, C-Bone algorithm, and DEFLATE algorithm are utilized for compression, with subsequent analysis of the compression ratio and compression time. The experimental findings demonstrate the DEFLATE algorithm's exceptional performance in compressing 3D image models. Notably, when compressing small and medium-sized 3D models, the DEFLATE algorithm exhibits significantly higher compression ratios compared to the Wavelet and C-Bone algorithms while also achieving shorter compression times. Compared to the Wavelet algorithm, the DEFLATE algorithm enhances the compression performance of 3D image models by 15% and boosts data throughput by 49%. While the compression ratio of the DEFLATE algorithm for large 3D models is comparable to that of the Wavelet and C-Bone algorithms, it notably reduces the actual compression time. Furthermore, the DEFLATE algorithm enhances data transmission reliability in NX 3D image model compression by 12.1% compared to the Wavelet algorithm. Therefore, the following conclusions are drawn: the DEFLATE algorithm serves as an excellent compression algorithm for 3D image models. It showcases significant advantages in compressing small and medium-sized models while remaining highly practical for compressing large 3D models. This study offers valuable insights for enhancing and optimizing the DEFLATE algorithm, and it serves as a valuable reference for future research on 3D image model compression.

ABSTRACT

PURPOSE: To investigate the clinical outcomes of reverse Z-plasty in the reconstruction of epicanthal fold. METHODS: We conducted a retrospective analysis on clinical data from patients who underwent medial canthal skin redundancy reconstruction surgery from September 2019 to January 2023. The surgical procedure involved a preoperative design for the incision line, suborbicularis oculi dissection to create a muscle flap, and the use of a reverse Z-flap for the reconstruction of the lateral canthal fold. Postoperative follow-up assessments focused on the intercanthal distance, positional changes of the medial canthus point, alterations in the medial canthus angle, and patient satisfaction levels. The statistical evaluation was carried out utilizing paired t -tests, with a P -value of less than 0.05 denoting statistical significance. RESULTS: Postsurgery, the lacrimal prominences were less exposed, and inner canthal angles naturally reshaped. Inconspicuous scarring with diminished reverse Z-plasty marks was noted within 3 months. The average ICD has increased by 3 to 6 mm, corresponding to elongation ratios of 9.09% to 28.30%. Preoperatively, the ICD averaged 31.25±2.32 mm, expanding postoperatively to 35.19±2.26 mm. The canthal angle enlarged significantly from 49.031±6.627 to 62.188±6.662. Inner canthal points shifted notably postsurgery, with a decrease in x-value and an increase in y-value, signalling a movement upwards and away from the nose. Patient satisfaction is high. CONCLUSION: The reverse Z-plasty technique has proven to be an effective approach for reconstructing the epicanthal fold. The clarity and precision of the incision design, coupled with the stability of postoperative results, demonstrate that this method can reliably achieve successful epicanthal fold reconstruction.

ABSTRACT

OBJECTIVE: To investigate the efficacy of tilianin extracted from Xiangqinglan () on apoptosis of H9c2 cell after oxygen-glucose deprivation/reoxygenation (OGD/R) and the mechanism. METHODS: Tilianin was obtained from Beijing Inluck Science and Technology Development Co. Ltd., with purity ≥ 98%. The OGD/R model was established in H9c2 cells. Flow cytometry detected the mitochondrial membrane potential, apoptosis rates, mitochondrial reactive oxygen species (ROS) and calcium ion concentration. Succinate dehydrogenase (SDH) activity, succinate content and levels of tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6) and interleukin-1 ß (IL-1ß) were detected with enzyme-linked immunosorbent assay. Western blot measured protein levels. RESULTS: Tilianin significantly reduced the apoptotic rates, ROS levels, calcium ion concentration, succinate content, and, levels of TNF-α, IL-6 and IL-1ß of OGD/R cells, while significantly increased the membrane potential and SDH activity in mitochondria. Western blot analysis showed that tilianin significantly up-regulated p-Calmodulin-dependent protein kinase Ⅱ and voltage-dependent anion selective channel levels in OGD/R cells, while significantly down-regulated p-protein kinase B, Bcl-2-associated X, and dynamin-related protein 1 levels related to apoptosis in the mitochondrial pathway. Moreover, tilianin significantly up-regulated B-cell lymphoma-2 and mitochondrial protein 2 related to the inhibition of apoptosis. Furthermore, tilianin down-regulated phosphorylated-apoptosis signal-regulated kinase 1, phosphorylated-p38 and C/EBP homologous protein related to endoplasmic reticulum stress. CONCLUSIONS: Tilianin may inhibit OGD/R-induced H9c2 cell apoptosis mediated by mitochondrial pathway and endoplasmic reticulum stress, thus protecting cardiomyocytes.

ABSTRACT

Objective:To assess the predictive efficacy of 18F-FDG PET/CT-based radiomics models for the mutation status of Kirsten rats sarcoma viral oncogene homolog (KRAS) in patients with non-small cell lung cancer (NSCLC). Methods:From January 2016 to January 2021, the 18F-FDG PET/CT images and KRAS testing of 258 NSCLC patients (180 males, 78 females; age: 33-91 years) in the First Affiliated Hospital of the Air Force Military Medical University were retrospectively analyzed. Patients were randomly divided into training set ( n=180) and validation set ( n=78) in the ratio of 7∶3. Tumor lesions on PET and CT images were drawn respectively, and the radiomics features of PET and CT lesions were extracted. The radiomics features were screened by least absolute shrinkage and selection operator (LASSO). CT radiomics score (RS) model, PET/CT RS model and composite models of PET/CT RS combined with screened clinical information were eventually developed. ROC curves were used to assess the predictive efficacy of these models. Results:The CT RS model included 4 radiomics features and the PET/CT RS model included 4 CT radiomics features and 8 PET radiomics features. The CT RS model and the PET/CT RS model both had significant differences in RS between KRAS mutant and wild-type patients in the training set and validation set ( z values: from -8.30 to -4.10, all P<0.001). In predicting KRAS mutations, the composite model of PET/CT RS combined with age showed AUCs of 0.879 and 0.852 in the training and validation sets respectively, which were higher than those of the CT RS model (0.813 and 0.770) and the PET/CT RS model (0.858 and 0.834). The accuracy of the composite model of PET/CT RS combined with age were 81.67%(147/180) and 79.49%(62/78) in the training set and validation set respectively, which were also higher than those of the CT RS model (75.00%(135/180) and 74.36%(58/78)) and the PET/CT RS model (78.89%(142/180) and 78.21%(61/78)). Conclusion:Models based on radiomics features can predict KRAS gene mutation status, and the composite model combining PET/CT RS and age can improve the prediction performance.

ABSTRACT

The portable light-weight magnetic resonance imaging system can be deployed in special occasions such as Intensive Care Unit (ICU) and ambulances, making it possible to implement bedside monitoring imaging systems, mobile stroke units and magnetic resonance platforms in remote areas. Compared with medium and high field imaging systems, ultra-low-field magnetic resonance imaging equipment utilizes light-weight permanent magnets, which are compact and easy to move. However, the image quality is highly susceptible to external electromagnetic interference without a shielded room and there are still many key technical problems in hardware design to be solved. In this paper, the system hardware design and environmental electromagnetic interference elimination algorithm were studied. Consequently, some research results were obtained and a prototype of portable shielding-free 50 mT magnetic resonance imaging system was built. The light-weight magnet and its uniformity, coil system and noise elimination algorithm and human brain imaging were verified. Finally, high-quality images of the healthy human brain were obtained. The results of this study would provide reference for the development and application of ultra-low-field magnetic resonance imaging technology.

ABSTRACT

Heat stroke is a critical and health-threatening disease, triggered by thermal stimulus and progressing rapidly. It can give rise to multiple organ dysfunction syndrome (MODS), resulting in a high mortality rate. Nearly 30% of survivors will suffer with different sequelae, for instance, the neurological sequelae. Currently, the early rapid cooling is the focus of therapy for heat stroke. Therefore, it is imperative to design a cooling module suitable for the treatment of heat stroke in the field and in the hospital to realize the goal of early rapid cooling and the effective targeted temperature management (TTM). The cooling device is composed of a cooling blanket and a cooling cap. The blanket and cap are made by temperature changeable fabric. The cooling blanket comprises a backing layer, a buffer layer, a flexible heat conduction capsule body, a temperature changing component, a fixed part and a temperature sensor. The cooling cap includes a main body and two side ears, in which the main body is worn on the top of the patient's head, and the front is equipped with a flexible display screen, which is convenient for real-time monitoring the temperature of the temperature change component of the cooling blanket. The lateral ear can wrap the patient's ears and neck, and the tympanic membrane thermometer is designed to monitor the tympanic membrane temperature in real time. The tympanic membrane thermometer is also designed at the ear to monitor the tympanic auditory canal temperatures in real time.Continuous dynamic temperature monitoring can guide the duration of cooling treatment and stop cooling in time. The cooling component is portable, easy to operate, real-time temperature monitoring, excellent cooling effect and reusable. It is used for on-site first aid, transportation and continuous cooling for patients with heat stroke in the ward.

ABSTRACT

In the trend of global warming and the increasing number of participating in high-intensity sports, the incidence of heat stroke is increasing year by year. Heat stroke is categorized to the class of heat emergent diseases. If there is no quick and effective treatment, the mortality rate is as high as 50%. Early, rapid, and effective cooling is the key point in the treatment of heat stroke. The early recognition and rapid lowering the core body temperature to 38.9 ℃ within 30 minutes of exertional heat stroke results in better clinical outcome with the fewest complications, however, the "golden 30 minutes" is far from enough to complete emergency transport and evaluation. Therefore, it is urgent to develop a first aid suit for heat stroke suitable for field treatment and transportation, so as to achieve the treatment goal of early rapid and effective cooling in field environment. For this purpose, the medical staff of the department of critical care medicine of the Eighth Medical Center of the Chinese People's Liberation Army General Hospital designed and developed the split first aid suit, which is suitable for heat stroke patient in the harsh environment lacking medical supplies and water during transportation. The medical staff who designed the first aid suit obtained a National Utility Model Patent of China (ZL 2020 2 1627326.4). The split heat stroke first aid suit is composed of four main parts: a sleeveless coat, a hat, sleeves and wrapped pants. All of them are made by temperature changeable fabric which is a composite material made by fine porous silica gel particles and potassium nitrate. The cuffs of the sleeveless top of the clothes part and the sleeves are connected as a whole by the connecting pieces; and the front end of the hem of the clothes part and the wrapped trousers are also connected as a whole by the connecting pieces, which is convenient for fixing and transportation. In order to control the water injection quantity, the water injection bag and the corresponding components are designed to ensure the same amount of water can be injected to the heat stroke first aid suit at each time, so the same frigorific effect can be attainable. The service temperature is 14-18 ℃, so it will not cause local frostbite to patients when used for a long time. This split-type first aid suit can be used in the movement, rescue and transportation of heat stroke patients. It is easy to carry and use with accurate cooling effect with low cost, moreover, it is reusable and can be widely used for on-site emergency and transportation of heat stroke patients.

ABSTRACT

BACKGROUND: After the one-child policy has been effective for over 30 years, China implemented the universal two-child policy in 2015. The number of high-risk pregnant women had increased dramatically ever since, increasing negative health outcomes for both mothers and children. Our study aims to investigate the status of anxiety, psychological stress sources, and psychological capital among women pregnant with a second child, and to assess influencing factors and correlations, providing scientific basis for promoting women's health during pregnancy. METHODS: We recruited 513 participants from maternity hospitals in Guangdong and Shandong Province. All participants completed the Zung Self-Rating Anxiety Scale (SAS), Pregnant women psychological stress sources questionnaire and the psychological capital questionnaire (PCQ-24). RESULTS: The prevalence of anxiety of women pregnant with a second child was 27.5%. The score of psychological stress sources was 74.93 ± 16.07. The risk factors for the anxiety of the women pregnant with a second child included low education level of the husband, gender expectation and the sources of psychological stress of the pregnant woman. The risk factors for psychological stress were dissatisfaction of residence and unplanned pregnancy. The main factors influencing psychological capital were the participants' education level, whether they have learned antenatal care knowledge and satisfaction with the residence. LIMITATIONS: All participants were recruited only from tertiary hospitals in Guangdong and Shandong province. CONCLUSIONS: Anxiety and psychological stress is prevalent during pregnancy among women pregnant with a second child in Guangdong and Shandong. Psychological capital was a protective factor for anxiety.

ABSTRACT

Chronic obstructive pulmonary disease (COPD) is a severe respiratory disease characterized by airway obstruction with high mortality and rate of recurrence. Rehabilitation exercise can lessen the possibility of the progressive exacerbation of the patient's condition, exerting an active role in improving their lung function and the quality of the patients' life. Therefore, we have designed a lung function exercise bottle, which is capable of adjusting the pressure according to the patient's needs. It is easy to operate and apply in practice. The exercise bottle is composed of three components, including bottle body, threaded round cap and air blow pipe. The bottle body is formed in rectangle shape, and utilized to contain liquid. The filling opening is designed on the top of the bottle. Outside of the filling opening, there is a screw thread round cap, which can adjust the pressure according to the patient's tolerance, gradually increasing the training intensity, and improving the training effect thereby. The blowpipe can be inserted into the bottle body and the patient can blow the air through the tube. Compared with the traditional rehabilitation training, when the newly invented lung function exercise bottle has been used for rehabilitation exercise, the air pressure in the bottle can be adjusted, with the advantage of wider range of application, lower cost and more convenient. It provides a good solution for clinical and home-based rehabilitation and is worth of popularizing.

ABSTRACT

High-flow nasal cannula oxygenation (HFNC) characterized as a new non-invasive respiratory support technology, has been widely used in recent years. Compared with conventional oxygen therapy (COT), non-invasive ventilation (NIV), HFNC can offset patient discomforts, and effectively arrest the deterioration of acute respiratory failure (ARF) in immunosuppressed patients. Although there is no benefit of HFNC over COT on reducing mortality in immunocompromised patients with ARF, HFNC is associated with a lower intubation rate and the improved prognosis of transplant recipients and solid cancer patients. Although the association between the prognosis of HFNC treatment and the pathogenic differences of ARF patients remains unknown, HFNC has, as one of the alternative methods for treating ARF in immunosuppressed patients, outstanding clinical significance. The treatment of HFNC in immunosuppressed patients with ARF, such as transplantation, malignancy, pneumocystis pneumonia, and interstitial pneumonia are reviewed in this article, in order to guide the clinical application of HFNC in such patients.

ABSTRACT

Platelets play a pivotal role in the thromboembolic, inflammatory, and immunomodulatory process. The alteration of platelet quality often affects the treatment and prognosis of critically ill patients, and has a significant correlation with mortality. With the further research on the function of the platelet, it is found that the abnormal platelet quality can present during the early stage of the illness of the critically ill patients. In order to evaluate the alterations of the platelet quality more accurately, the further studies of platelet parameters, including platelet counts (PLT), platelet hematocrit (PCT), platelet large cell ratio (PLCR), mean platelet volume (MPV), platelet distribution width (PDW), immature platelet fraction (IPF) and so on, are still be the focus of current researches in the field of critical illness. At the same time, the application of thromboelastography/thrombelastography-platelet mapping (TEG/TEG MP) to the measurement of platelet function, especially the researches on the inhibitory rate of adenosine diphosphate (ADP) and the inhibitory rate of arachidonic acid (AA), is the hot spot of current researches. With regard to the diagnosis, prognosis and early goal-directed therapy (EGDT) of the critically ill patients, it is important to comprehensively apply the methods of platelet function evaluation.

Objective To explore the protective effect of Xuebijing on the endothelium and extracellular matrix in sepsisinduced acute kidney injury (AKI) rats for providing a new clinical treatment strategy. Methods The method of cecal ligation and puncture (CLP) was used to duplicate severe sepsis model.Thirty healthy male SD rats were randomly divided into 3 groups:Sham group (n=10),NS group (normal saline 4ml/kg,n=10),Xuebijing group (Xuebijing 4ml/kg,n=10).6h after CLP,the rats were sacrificed and their kidneys were resected and histopathological characteristic was observed by light microscopic and transmission electron microscopic techniques. The expressions ofET-1 mRNA,iNOS mRNA,MMP-9 mRNA,TIMP-1 mRNA in the renal tissues were measured semiquantitatively by reverse transcription-polymerase chain reaction (RT-PCR).Results The histopathological changes in renal tissue were observed by light microscope. The changes of renal glomerulus and renal tubuli in Xuebijing group were better than NS group. The ultrastructural changes in renal tissue were observed under electron microscope.Compared with NS group, ultrastructural changes of renal glomerulus and proximal convoluted tubule were smaller in Xuebijing group. The expressions of ET-1mRNA (0.631 ± 0.169vs 0.770 ± 0.154,P<0.05),iNOS mRNA (0.507 ± 0.071 vs 0.587 ± 0.073,P<0.05),MMP-9mRNA (0.641 ± 0.082 vs 0.742 ± 0.116,P<0.05) and TIMP-1 mRNA (0.434 ± 0.052 vs 0.520 ± 0.049,P<0.01) were significantly lower in XBJ group renal tissues than in NS group. The expressions of ET-1 mRNA(0.770 ± 0.154 vs 0.394 ± 0.105,P<0.01),iNOS mRNA (0.587 ± 0.073 vs 0.326 ± 0.085,P<0.01),MMP-9 mRNA (0.742 ± 0.116 vs 0.356 ± 0.055,P<0.01) and TIMP-1 mRNA (0.520 ± 0.049 vs 0.351 ± 0.041,P<0.05) in renal tissues were more significantly increased in NS group compared with sham group. Conclusions Xuebijing could reduce the levels of ET-1,iNOS,MMP-9 and TIMP-1 mRNA,protect the stability of endothelium and extracellular matrix, and reduce tissue injury in septic rats.

ABSTRACT

Hyperspectral images provide great potential for target detection, however, new challenges are also introduced for hyperspectral target detection, resulting that hyperspectral target detection should be treated as a new problem and modeled differently. Many classical detectors are proposed based on the linear mixing model and the sparsity model. However, the former type of model cannot deal well with spectral variability in limited endmembers, and the latter type of model usually treats the target detection as a simple classification problem and pays less attention to the low target probability. In this case, can we find an efficient way to utilize both the high-dimension features behind hyperspectral images and the limited target information to extract small targets? This paper proposes a novel sparsity-based detector named the hybrid sparsity and statistics detector (HSSD) for target detection in hyperspectral imagery, which can effectively deal with the above two problems. The proposed algorithm designs a hypothesis-specific dictionary based on the prior hypotheses for the test pixel, which can avoid the imbalanced number of training samples for a class-specific dictionary. Then, a purification process is employed for the background training samples in order to construct an effective competition between the two hypotheses. Next, a sparse representation-based binary hypothesis model merged with additive Gaussian noise is proposed to represent the image. Finally, a generalized likelihood ratio test is performed to obtain a more robust detection decision than the reconstruction residual-based detection methods. Extensive experimental results with three hyperspectral data sets confirm that the proposed HSSD algorithm clearly outperforms the state-of-the-art target detectors.

ABSTRACT

OBJECTIVE:To observe clinical efficacy and safety of Qizhi qushi jianpi prescription in the treatment of chronic synovitis of knee joint in middleaged and elderly patients after debridement under arthroscope. METHODS:A total of 80 mid-dleaged and elderly inpatients with chronic synovitis of knee joint were selected and randomly divided into treatment group and con-trol group, with 40 cases in each group. Clinical efficacy,VAS,swelling score of modified Cincinnati knee joint score system, Lysholm score of knee joint function, the occurrence of ADR were observed in 2 groups. RESULTS:Total effective rate of treat-ment group(92.5%)was significantly than that of control group(77.5%),with statistical significance(P0.05). On the first day after surgery,VAS score of 2 groups decreased significantly, and there was statistical signifi-cance compared to before surgery (P<0.05). One month after surgery,VAS score of 2 groups decreased significantly, and the treatment group was significantly lower than the control group, with statistical significance(P<0.05). Swelling score of knee joint 1 month after surgery and Lysholm score 3 months after surgery increased significantly in 2 groups, and the treatment group was significantly higher than the control group, with statistical significance(P<0.05). 5 patients of treatment group suffered from stom-ach discomfort, and the symptom was relieved after adding the ingredient of comforting stomach and removing dampness through diuresis. CONCLUSIONS:Qizhi qushi jianpi prescription is effective for chronic synovitis of knee joint in middleaged and elderly patients after debridement under arthroscope, and can relieve pain, remove swelling and recover knee joint function with good safety.
