ABSTRACT

Objective:To evaluate the effect of esketamine on long-term cognitive dysfunction induced by propofol anesthesia in the developing rats and the role of phosphatidylinositol-3-kinase (PI3K)/serine-threonine protein kinase (Akt) signaling pathway.Methods:Forty-eight clean-grade healthy Sprague-Dawley rats of either sex, aged 7 days, weighing 10-15 g, were divided into 4 groups ( n=12 each) using a random number table method: fat emulsion group (C group), propofol group (P group), esketamine + propofol group (EP group), and PI3K inhibitor LY294002 + esketamine + propofol group (LYEP group). Medium/long-chain fat emulsion injection 100 mg/kg was intraperitoneally injected in C group. Propofol was intraperitoneally injected at a dose of 50 mg/kg, followed by an additional dose of 50 mg/kg after the righting reflex was restored (40-60 min later) in P group. In group EP, esketamine 10 mg/kg was intraperitoneally injected, followed by propofol administration using the same method as previously described in P group. In LYEP group, LY294002 25 μg was injected via the lateral ventricle, 30 min later ketamine 10 mg/kg was intraperitoneally injected, and then propofol was given using the same method as previously described in P group. Six rats in each group were randomly sacrificed at 2 h after emergence for microscopic examination of pathological changes of hippocampal neurons and for determination of Akt, phosphorylated Akt (p-Akt), Bax, and cleaved caspase-3 in the hippocampal tissues (using Western blot). The remaining 6 rats in each group were subjected to Y-maze test to evaluate their learning and memory abilities at 30 days after birth. The p-Akt/Akt ratio was calculated. Results:Compared with C group, the p-Akt/Akt ratio in the hippocampal tissues was significantly decreased, the expression of Bax and cleaved caspase-3 was up-regulated, the number of training sessions required for learning was increased, the correct response rate was decreased ( P<0.05), and the pathological damage to neurons in hippocampal CA1 region was found in P, EP and LYEP groups. Compared with P group, the p-Akt/Akt ratio in the hippocampal tissues was significantly increased, the expression of Bax and cleaved caspase-3 was down-regulated, the number of training sessions required for learning was decreased, the correct response rate was increased ( P<0.05), and the pathological damage to neurons in hippocampal CA1 region was significantly attenuated in EP and LYEP groups. Compared with EP group, the p-Akt/Akt ratio in the hippocampal tissue was significantly decreased, and the expression of Bax and cleaved caspase-3 was up-regulated, the number of training sessions required for learning was increased, the correct response rate was decreased ( P<0.05), and the pathological damage to neurons in hippocampal CA1 region was aggravated in LYEP group. Conclusions:Esketamine can alleviate long-term cognitive impairment caused by propofol anesthesia in the developing rats, and the mechanism may be related to activation of the PI3K/Akt signaling pathway and inhibition of apoptosis in neurons.

ABSTRACT

Objective:To evaluate the effect of intravenous infusion of lidocaine on pulmonary gas exchange function during acute lung injury in septic rats.Methods:Thirty clean-grade healthy male Sprague-Dawley rats, aged 2-3 months, weighing 220-280 g, were divided into 3 groups ( n=10 each) using a random number table method: control group (C group), sepsis group (S group) and lidocaine group (L group). Model of acute lung injury in septic rats was prepared by intraperitoneal injection of LPS 10 mg/kg in S and L groups, the equal volume of normal saline was injected in group C, lidocaine was injected at a loading dose of 10 mg/kg via the tail vein at 1 min after LPS injection and then continuously infused for 3 h at a rate of 10 mg·kg -1·h -1, and the equal volume of normal saline was given instead in C and S groups. Rats were sacrificed at 24 h after LPS injection, blood samples from the abdominal aorta were collected for blood gas analysis, and the oxygenation index (OI), alveolar-arterial oxygen difference (A-aDO 2) and respiratory index (RI) were calculated. Then the chest was immediately opened, and the left lung tissues were then immediately removed to determine the levels of tumor necrosis factor-α (TNF-α), heme oxygenase-1 (HO-1) and reactive oxygen species (ROS) (by enzyme-linked immunosorbent assay). The right upper lung tissues were removed for microscopic examination of the alveolar structure and pulmonary edema with a light microscope. The right lower lung tissues were also removed to observe the vascular endothelial structure with a transmission electron microscope. Results:Compared with group C, the levels of A-aDO 2, RI, TNF-α, HO-1 and ROS were significantly increased, the PaO 2 and OI were decreased ( P<0.05), and no significant change was found in PaCO 2 in group S and group L ( P>0.05). Compared with group S, the PaO 2, OI and HO-1 were significantly increased, the levels of A-aDO 2, RI, TNF-α and ROS were decreased ( P<0.05), and no significant change was found in PaCO 2 levels in group L ( P>0.05). The pathological damages of the lung tissues were significantly attenuated in group L when compared with group S. Conclusions:Intravenous infusion of lidocaine can improve pulmonary gas exchange function during acute lung injury in septic rats.

ABSTRACT

Objective:To investigate the clinical effect of minimally invasive-locking block modified Krackow (MI-LBMK) and open giftbox technique in the treatment of Achilles tendon rupture.Methods:Fifty-six patients with Achilles tendon rupture from January 2016 to December 2018 were collected, including 54 males and 2 females, aged 40.7±9.4 years (range 26 to 65 years). The MI group (30 patients) used two minimally invasive incisions without exposing the rupture site, and the LBMK technique was used to repair the Achilles tendon. The open group (26 patients) used a posteromedial longitudinal incision and the giftbox technique was used to repair the rupture tendon. The Achilles tendon was repaired with 6-strand sutures in both groups. Early rehabilitation programs were adopted for postoperative rehabilitation, and regular follow-up (6 weeks, 3, 6, 12 and 24 months after operation) was performed to record the Achilles tendon resting angle (ATRA), American Orthopaedic Foot and Ankle Society ankle-hindfoot score (AOFAS), Achilles tendon total rupture score (ATRS). The rupture gap and cross-sectional area (CSA) were measured by MRI at 6 weeks and 3 months after surgery.Results:A total of 30 patients in the MI group and 26 in the open group were enrolled. The differences between the two groups in age, body mass index, interval from injury to operation, and tendon rupture site were not statistically significant ( P>0.05). All patients were followed up to 24 months after surgery. There were no wound complications in MI group, and 2 cases of superficial infection and 1 case of wound skin necrosis occurred in open group. There was no re-rupture in both groups. The relative ATRA of MI group was -6.32°±0.99°, -3.90°±1.05°, -2.38°±0.84°, -0.25°±1.37° at 3, 6, 12 and 24 months after operation, respectively. The relative ATRA of open group was -7.88°±3.71°, -6.16°±1.10°, -4.53°±0.95°, -3.01°±0.95° at 3, 6, 12 and 24 months after operation, respectively. The differences between the two groups were statistically significant ( P<0.05). The ATRS of minimally invasive group at 6 months and 12 months were 72.70±7.41 and 92.97±3.35 respectively, and the ATRS of open group at 6 months and 12 months were 68.08±6.64 and 90.85±4.27 respectively, and the differences were statistically significant ( P<0.05). The AOFAS of minimally invasive group at 6 months and 12 months were 88.60±2.76 and 93.83±1.98 respectively, and the AOFAS of open group at 6 months and 12 months were 85.77±3.20 and 92.08±2.64 respectively, and the differences were statistically significant ( P<0.05). The difference in the gap between the tendon rupture ends measured by MRI sagittal plane T2WI between the two groups was not statistically significant ( P>0.05). The cross-sectional area of Achilles tendon in the MI group was higher than that of the open group at 12 weeks ( P<0.05). Conclusion:The MI-LBMK technique may protect the peritendon tissue and has fewer complications, and can enable the patient to return to daily life faster, with lower postoperative Achilles tendon elongation and better recovery of Achilles tendon function.

ABSTRACT

Objective:To evaluate the effect of lidocaine on the expression of high-mobility group box 1 protein (HMGB1) mRNA during lidocaine-induced attenuation of pulmonary vascular endothelial dysfunction in septic rats.Methods:Thirty clean-grade healthy male Sprague-Dawley rats, aged 8-12 weeks, weighing 250-300 g, were divided into 3 groups ( n=10 each) by the random number table method: sham operation group (S group), sepsis group (C group) and lidocaine group (L group). Sepsis model was developed by cecal ligation and puncture in anesthetized animals.In group S, only surgery was performed without ligation.In group L, lidocaine 10 mg/kg was injected into the tail vein immediately after model preparation, followed by 10 mg·kg -1·h -1 infusion for 3 h. Groups S and C received the equal volume of normal saline instead.At 24 h after model preparation, rats were sacrificed and blood samples were collected from the inferior vena cava, and lung tissues were harvested.Serum tumor necrosis factor-alpha (TNF-α) and syndecan-1 levels were measured by enzyme-linked immunosorbent assay, HMGB1 mRNA expression in lung tissues was detected using quantitative real-time polymerase chain reaction, wet/dry lung weight ratio was measured, and pulmonary vascular endothelial structure was observed with a transmission electron microscope. Results:Compared with group S, the serum TNF-α and syndecan-1 concentrations and wet/dry lung weight ratio were significantly increased, and the expression of HMGB1 mRNA in lung tissues was up-regulated in C and L groups ( P<0.05). Compared with group C, the serum TNF-α and syndecan-1 concentrations and wet/dry lung weight ratio were significantly decreased, and the expression of HMGB1 mRNA in lung tissues was down-regulated in group L ( P<0.05). The results of transmission electron microscope showed that the pulmonary vascular endothelium was intact and continuous, and the structure was dense in group S; the pulmonary vascular endothelium was discontinuous, and the structure was significantly loose in group C; the pulmonary vascular endothelium was basically intact, and the structure was slightly loose in group L. Conclusions:Lidocaine may reduce pulmonary vascular endothelial dysfunction in septic rats by inhibition of HMGB1 mRNA expression.

ABSTRACT

Objective:To investigate the clinical effect of superthin anterolateral thigh flap(ALTF) with retrograde dissection of perforator in the interface plane between the superficial and deep layer of superficial fascia for reconstruction of soft tissue defect in the foot.Methods:The study involved 24 Side of 23 patients with foot soft tissue defects in Department of Foot and Ankle Surgery in Wuxi Ninth People’s Hospital from August 2019 to July 2021. There were 15 males and 8 females with an average of 42(range, 22-59) years old, including 9 in left foot, 13 in right foot, and 1 in both feet. The size of soft tissue defects was 4 cm×4 cm-11 cm×17 cm. The dimension of the superthin ALTF was 4 cm×5 cm-12 cm×18 cm. CTA and high-frequency CDU were used to locate the perforator in the superficial fascia plane. The perforator was exposed and dissected retrograde in the adiposal layer. The superthin ALTF was harvested to repair the foot wound. The donor site was sutured directly. All patients enter follow-up reviews at outpatient clinic or by WeChat. The appearance of flaps were recorded.Results:The superthin ALTF survived in all patients. Two cases had partial epidermal necrosis at the distal part of the flap. The thickness of the flap averaged approximately 4(range 3-6) mm. During 8-16(mean 12) months of follow-up, all superthin ALTF were soft in texture without ulceration. Two flaps required secondary defatting procedures, others showed satisfactory appearance without bulky deformity. Only linear scars left in donor areas.Conclusion:The technique of harvesting superthin ALTF with retrograde dissection of perforator in the superficial fascia plane for repairing foot wounds is reliable and is able to achieve satisfactory functional and esthetic outcome.

ABSTRACT

Objective:To compare the application and results in repairing wounds on shank and medial malleolus by perforator pedicled middle and lower third of posterior tibial flap, saphenous neurovascular flap and posterior tibial artery perforator plus saphenous neurovascular (double blood supply) flap.Methods:Clinical data of 60 patients with wounds on shank and medial malleolus and treated between August, 2015 and December, 2018 were analyzed. Among the patients, 25 were treated with perforator pedicled middle and lower third of posterior tibial flaps, 15 with saphenous neurovascular flaps, and 20 with double blood supply flaps. The data of surgery, survival of the flaps, texture and swelling of the flaps, patient satisfaction and scores set by American Orthopaedic Foot and Ankle Society(AOFAS) were collected. Statistic analysis was carried out to compare observations among the 3 groups.Results:Postoperative followed-up was 6 to 24(average 13.4) months. One flap suffered insufficient blood supply in the group of perforator pedicled middle and lower third of posterior tibial flaps, 2 flaps suffersd necrosis and skin graft was carried out in reverse saphenous nerve neurocutaneous flaps group. In double blood supply flaps group, 2 flaps appeared purple after surgery, and improved after removing the pedicle sutuer, and flaps became swelling and received secondary repair. All AOFAS scores (93.8, 93.3 and 92.8, respectively) and patient satisfaction were high in all 3 groups.Conclusion:All 3 types of flap are able to be used in repairing the soft tissue defect on shank and medial malleolus. The inclusive of saphenous nerve should be determined in intraoperation according to the presence and size of the perforator. If artery perforation is reliable, the use of posterior tibial artery perforator flap in wound repairing will deliver satisfactory outcomes in terms of blood supply and appearance of the flap. For the wider area of wound, a flap with double blood supply is preferred.

ABSTRACT

Objective:To investigate the operational impact of the curve laterality of degenerative lumbar scoliosis on oblique lateral lumbar interbody fusion (OLIF).Methods:Data of 40 cases with degenerative lumbar scoliosis and 20 cases without degenerative lumbar scoliosis treated in our hospital from January to December 2017 were retrospectively analyzed. There were 20 cases in left convex group (male 5, female 15, 70.62±5.45 years old) and 20 cases in left concave group (male 3, female 17, 69.73±7.24 years old), and there were 20 cases of lumbar degenerative diseases without scoliosis (lumbar spinal stenosis 13 cases, lumbar disc herniation 7 cases; male 5, female 15, 71.48±5.73 years old). The following OLIF operation-related anatomical parameters were measured on MR axial T2 weighted image and lumbar spine X-ray image: distance from the left edge of the abdominal aorta to the anterior medial edge of the left psoas muscle; distance from the left edge of the abdominal aorta to the left lumbar sympathetic trunk; distance from the anterior medial edge of the left psoas muscle to the transverse axis of the vertebral body; distance between the midpoints of adjacent vertebral bodies in L 2-5; angle of rotation of the vertebral body and angle of the OLIF operating channel. One-way analysis of variance(ANOVA) and least significant difference (LSD) were used for statistical analysis of measurement parameters of different groups. Results:There were statistically significant differences between the distance from the left edge of the abdominal aorta to the anterior medial edge of the left psoas muscle, and the distance from the left edge of the abdominal aorta to the left lumbar sympathetic trunk in the three groups of cases (All P<0.05). The L 2, 3 segment (24.41±9.54 mm, 18.18±7.1 mm) and L 3, 4 segment (18.54±7.94 mm, 13.73±6.73 mm) in the left concave group were significantly larger than those in the no scoliosis group; and the above values of the L 4, 5 segment of the left convex group (19.16±7.04 mm, 11.67±3.63 mm) were significantly larger than those in the no scoliosis group. For the distance from the anterior medial edge of the left psoas muscle to the transverse axis of the vertebral body, the values of L 2, 3 and L 3, 4 (13.76±2.98 mm, 15.87±3.53 mm) in the left convex group were significantly greater than those in the no scoliosis control group; but in the left concave group, the corresponding values (9.97±3.14 mm, 10.75±5.03 mm) were significantly smaller than those in the no scoliosis group. The distances between the midpoints of adjacent vertebral bodies of L 2, 3 and L 3, 4 (37.67±3.45 mm, 38.18±3.54 mm) in the left convex group were greater than those in the no scoliosis group and left concave group, and the differences between the three groups were statistically significant ( P<0.05). Pearson correlation analysis between the absolute value of vertebral rotation angle and OLIF surgical passage angle showed that there was a negative correlation between them in the left convex group and a positive correlation in the left concave group. Conclusion:The curve laterality of degenerative lumbar scoliosis had a certain influence on the anatomical parameters of oblique lateral lumbar interbody fusion. It was recommended to design and adjust the operation skills according to the curve laterality before surgery.

ABSTRACT

Objective:To investigate effects of bone-resorptive lesion on stress distribution of femoral head and on progression in patients with osteonecrosis of the femoral head (ONFH).Methods:From April 2014 to September 2018, a total of 155 femoral heads from 94 patients diagnosed with ARCO stage II and III ONFH were retrospectively reviewed, including 77 males and 17 females with aged 39.90±10.45 years old (ranged from 18-64 years). The hips were divided into two groups according to whether there were bone-resorptive lesions. Further, we compared whether there was statistical difference between the two groups in staging. Then, a case of ARCO II hip joint without bone-resorptive lesion was selected from the included patients. Six femoral head with different diameters of spherical bone-resorptive lesion of 5 mm, 7 mm, 10 mm, 14 mm, 18 mm, and 23 mm were simulated. The influence of bone-resorptive lesion on the stress distribution of necrotic area and a spherical shell extending 1 mm radially around the bone-resorptive lesion was investigated by finite element method in slow walking conditions.Results:Of the 155 ONFH hips, 67 hips are complicated by bone-resorptive lesions, of which 17 were ARCO II, 50 were ARCO III. A total of 88 hips did not contain bone-resorptive lesions, of which 58 were ARCO II, ARCO III 30 cases. The proportion of ARCO stage II in the group with bone-resorptive lesions was significantly higher than that in the group without bone-resorptive lesions (χ 2=25.03, P=0.000). The finite element stress distribution cloud diagram showed that there was a stress concentration area around the bone-resorptive lesions. The maximum von Mises stress around bone-resorptive lesions in the models that contained a synthetic bone-resorptive lesions were significantly higher than those reported in the matched, non-synthetic bone-resorptive lesions finite element models ( t=3.139, P=0.026). The values for maximum von Mises stress around bone-resorptive lesions were 6.94±1.78 MPa and 5.01±0.35 MPa for the group with synthetic bone-resorptive lesions and the group non-synthetic bone-resorptive lesions, respectively. There was a positive correlation between the diameter of bone-resorptive lesions and the maximum and mean von Mises stress of necrotic areas as well as the maximum von Mises stress around bone-resorptive lesions. Conclusion:Bone-resorptive lesions can increase the maximum stress and average stress in the necrotic area. The larger the bone-resorptive lesion, the more the stress increases. There is a stress concentration area around the bone-resorptive lesions, which may accelerate the collapse of the femoral head.

OBJECTIVE:To provide reference for summarize the management and clinical use levels of biosimilar products . METHODS:Related policies and regulations about approval and application of biosimilar products were retrieved from domestic and foreign supervision departments and WHO. The biosimilar products were described from multi-dimensional aspects of whole life cycle of drugs ,generic name and prescription ,indication extrapolation ,clinical drug exchange ,pharmacovigilance,medical insurance payment system ,education and training. RESULTS & CONCLUSIONS :Biosimilar products refer to therapeutic biological products which are similar to the reference drugs which have been approved for marketing in terms of quality ,safety and effectiveness. In R&D ,production,circulation,use and supervision links,the management of biosimilar products has its own characteristics in different countries/areas/organization. In the R&D stage ,biosimilar products do not need to independently verify their safety and effectiveness ,but only step by step use analytical methods to gradually clarify their high similarity with reference drugs in terms of structure and function. The name of biosimilar products in China is the same as that of the original drugs ,and the general name was used in prescription. For FDA to approve the indication extrapolation of biosimilar products ,it needs to be based on the data and information at the time of application ,the safety and effectiveness information of reference drugs ,and the consideration of relevant scientific elements of indications. It needs to be evaluated and used conditionally under supervision. The standard of FDA approval is strict,that is to say ,the approval standard of realizing interchangeability is higher than that of biological similarity which has no such concept in China. The enterprise community ,regulatory agencies ,academic institutions and hospital drug rooms need to communicate and exchange ,and further strengthen post market risk control and safety monitoring. China’s medical insurance department should establish an appropriate payment system and encourage the use of biosimilar products through the payment system. Meclical workers should learn the characteristics of biosimilar products so that they can make good use of biosimilar products in practice on the basis of understanding their technical evaluation.

ABSTRACT

Nowadays, nanotechnology is revolutionizing the approaches to different fields from manufacture to health. Carbon nanotubes (CNTs) as promising candidates in nanomedicine have great potentials in developing novel entities for central nervous system pathologies, due to their excellent physicochemical properties and ability to interface with neurons and neuronal circuits. However, most of the studies mainly focused on the drug delivery and bioimaging applications of CNTs, while neglect their application prospects as therapeutic drugs themselves. At present, the relevant reviews are not available yet. Herein we summarized the latest advances on the biomedical and therapeutic applications of CNTs and for neurological diseases treatments as inherent therapeutic drugs. The biological mechanisms of CNTs-mediated bio-medical effects and potential toxicity of CNTs were also intensely discussed. It is expected that CNTs will exploit further neurological applications on disease therapy in the near future.

Phenicol antibiotics, such as chloramphenicol, thiamphenicol, and florfenicol, are commonly used in the veterinary and aquaculture fields to treat diseases and have frequently been detected in aquatic environments. Nevertheless, there is limited information regarding the effects of phenicol antibiotics on aquatic nontarget species. Thus, the present study aims to investigate the long-term (21-d) influence on the reproduction and growth of and the acute (24-h) oxidative response and tissue damage in the crustacean Daphnia magna after exposure to phenicol drugs, including their environmental concentrations. The results indicate that D. magna exposed to florfenicol are likely to cause more adverse effects than those exposed to chloramphenicol and thiamphenicol over long-term (21-d) exposures. Furthermore, changes in biochemical biomarkers such as malondialdehyde (MDA), catalase (CAT), and reduced glutathione (GSH) induced by individual and mixtures of phenicol antibiotics were also observed. Low concentrations of chloramphenicol, thiamphenicol + florfenicol, and chloramphenicol + thiamphenicol significantly increased the MDA levels of D. magna after 24-h exposures, causing cellular oxidative damage in the animals. In addition, discrepancies between CAT activities and GSH levels were observed, underscoring the need to evaluate multiple indicators of oxidative stress in toxicological studies using D. magna as a model. ABSTRACT

Oblique lateral interbody fusion (OLIF) surgery uses the retroperitoneal gap between the abdominal aorta and left psoas as the surgical approach to perform discectomy and interbody fusion. It has the advantages of shorter operation duration and hospital stay, less blood loss, lighter postoperative pain and quicker recovery compared with traditional lumbar interbody fusion surgery. OLIF surgery has been gradually applied in treating degenerative diseases of the lumbar spine. However, the complications, such as the injury of blood vessels, sympathetic nerves, lumbosacral plexus, peritoneum and ureteral, cannot be negligible. Previous studies reported that the overall incidence of complications about OLIF surgery was 3% to 53.1% with an average of 15.5%, which can be divided into intraoperative and postoperative complications. The incidence of iliac vascular injury was found to be 0.3%-15.4% in OLIF at the L5S1 segments. The anatomy about vascular in this area is complex because the aorta is branched into the left and right iliac artery. The surgical approach in L5S1 segments is also different from the traditional OLIF but similar to the lateral anterior lumbar interbody fusion, which could increase the risk of vascular injury. The other complications which do not show significant segmental difference were based on the previous literatures. The incidence of abdominal aortic injury is 0.1%, which is related to direct damage caused by the narrow operation window of OLIF. The incidence of lumbar segmental arterial injury was 0.7% to 5%, which may be caused by the anatomical variation of L4, 5 lumbar segmental artery. The incidence of lumbar sympathetic nerve injury is 1.7%. More attention should be paid to protect the lumbar sympathetic trunk which lying in the front of the psoas muscle. The incidence of cage-related complications ranges from 2.9% to 13.4%, which perhaps is associated with older age, osteoporosis and use of large-sized cages. Although the incidence of ureteral injury is 0.3% to 1.6%, care should also be taken due to not obvious injury without urinary tube.

ABSTRACT

Depression has become a serious disease affecting the health of Chinese people. Yueju pill has shown a unique rapid and lasting anti-depression effect, and Yueju pill can rapidly improve Parkinson's disease with depression.The drug pair of Fructus Gardeniae and Rhizoma Ligustici Chuanxiong is necessary for traditional Chinese formula Yueju Pill for antidepressant effect. Recent studies have found the antidepressant effect of Yueju pills at regular doses, and an integrative pharmacological method was adopted to predict the targets and pathways of Yueju pill and explore its molecular mechanism for depression. In recent years, there has been an increasing number of clinical studies on the treatment of depression with Yueju pills and significant therapeutic effects. A new clinical study found that Yueju pill may have antidepressant effects and increase serum BDNF concentration. This paper reviewed the recent research progress of Yueju pill in the treatment of depression in recent 5 years.

ABSTRACT

Objective:To investigate the mechanism of promoting wound healing of Shengji Huayu ointment. Methods: Db/db mice were divided into 5 groups according to the randomized block design: three Shengji Huayu ointment groups respectively at high (15 g·kg-1),middle (10 g·kg-1) and low(5 g·kg-1) dose,the positive control group(Beifuji) and the model control group, and balb/c mice were used in the normal control group,and each group was with 10 mice. The model of full-thickness skin defect in mouse skin was used,and after HE staining,IL-1β and collagen type I and type III in granulation tissue were observed by immunohis-tochemistry and image analysis. Results:HE staining showed that the wound healing was obvious in the high dose group and the mid-dle dose group. The expression of IL-1 β in each group was not obvious. The levels of collagen type I and type III in the three Shengji Huayu ointment groups were higher than those in the model control group (P<0.05).Conclusion: Shengji Huayu ointment can pro-mote wound healing and reduce scar formation in mice.

ABSTRACT

Objective:To observe the effect of Yulin mixture on ovarian function in rats with kidney-yang deficiency ovulatory dysfunction.Methods:The hydrocortisone-induced ovulation disorder rat models were established,and the rats were divided into the model control group,Yulin mixture group and positive control group (clomiphene citrate group),and the normal control group was set up,and 10 rats were in each group.The general situation of the rats was observed.The wet weight indices of uterus and ovary were measured by an electronic analytical balance.Serum estradiol (E2),progesterone (P),follicle stimulating hormone (FSH) and luteinizing hormone (LH) were measured by radioimmunoassay.Ovarian morphology was observed after HE staining.Results:Yulin mixture could improve the general status of ovulation disorder rats,increase the wet weight indices of uterus and ovary (P<0.05) and the levels of serum E2,P,LH and FSH (P<0.01),and improve the pathological state of ovarian morphology.Conclusion:Yulin mixture can improve the levels of serum E2,P,LH and FSH resulting in promoting ovarian development.

ABSTRACT

Objective To assess the risk of sympathetic nerve injury in oblique lumbar interbody fusion (OLIF) in different lumbar spine segments based on anatomical study.Methods Twenty-four healthy adult volunteers (12 male and female) were selected and routine lumbar spine scanning was performed with MAG MAGOMOM Verio 3.0 T.The anatomical structures of left lumbar sympathetic trunk,abdominal aorta and left psoas muscle were identified on T2 images of L2,3,L3,4,L4,5 intervertebral space.And the anatomical parameters of the OLIF operation approach and the anatomical parameters of the left sympathetic trunk and adjacent structures were measured.The t-test was used to compare the parameters between the different sexes.The comparison of the data between the different segments was performed by the least significant difference (LSD) single factor analysis of variance.Results From the L2,3 to L4,5 segments,the anatomical parameters of the OLIF operation approach and the anatomical parameters of the left sympathetic trunk and the adjacent structures showed regular changes.The distances between the anterior margin of the left psoas muscle and the abdominal aorta from L2,3 to L4,5 were 13.65±4.10 mm to 9.42 ± 4.00 mm in adult healthy male individuals,and 13.89±3.18 mm to 8.38 ± 3.33 mm in female individuals,showing a significant downward trend.The distances between the left sympathetic trunk and the abdominal aorta from L2,3 to L4,5 were 10.76±3.89 mm to 6.68±3.39 mm in adult healthy male individuals,and 11.52±3.02 mm to 6.12±2.95 mm in female individuals,also showing a significant downward trend.There were significant differences in the operation area of OLIF surgery between different segments.The operation area of OLIF surgery was relatively large in L 2,3 segment,and the risk of sympathetic nerve injury was relatively small.The left lumbar sympathetic trunk in the L3,4 intervertebral space was walking front and inside,and there was a greater risk of injury in the OLIF surgery.Conclusion The left lumbar sympathetic trunk located in or close to OLIF surgery operation field in L2-L5 segments.There was a certain risk of sympathetic nerve injury in OLIF surgery,and the risk of sympathetic nerve injury was different in L2,3,L3,4,L4,5 segments.

ABSTRACT

BACKGROUND:Studies have found that adipose-derived stem cells (ADSCs)/collagen complexes can promote the ADSCs differentiation and maturation into mature adipocytes and promote angiogenesis.OBJECTIVE:To explore the biological properties of the ADSCs/collagen sponge composite material and to detect its effect on angiogenesis.METHODS:(1) ADSCs were cultured on collagen sponge (experimental group) or cultured alone (control group).After 24 hours of culture,cell adhesive rate of ADSCs was determined with flow cytometry.After 2,4,6 days of culture,cell proliferation and level of vascular endothelial growth factor (VEGF) in the culture medium were detected.(2) Chick embryo chorioallantoic membranes were exposed and incubated for 7 days and then divided into four groups:0.2 mL of sterile PBS was added in the blank group,0.2 mL of 2× 108/L passage 3 ADSCs suspension was added in the ADSCs group,collagen sponge was added in the collagen sponge group,and collagen sponge with 0.2 mL of 2× 108/L passage 3 ADSCs suspension was added in the composite group.After 7 days of incubation,the microvessel count around the chorioallantoic membrane was measured.RESULTS AND CONCLUSION:(1) The cell adhesive rate of ADSCs to collagen sponge reached to (93.04±0.67)%.(2)The absorbance value (at 6 days of culture) and level of VEGF (at 4 and 6 days of culture) in the experimental group were significantly higher than those in the control group (P < 0.01 or P < 0.05).(3) Compared with the blank group,the number of microvessels was significantly higher in the ADSCs,collagen sponge and composite groups (P < 0.05).Moreover,higher amount of microvessels were found in the composite group than the ADSCs and collagen sponge groups (P < 0.05).To conclude,ADSCs can adhere well to the collagen sponge with good biocompatibility and their combined use can improve angiogenesis further by enhancing cell proliferation and VEGF secretion of ADSCs.

ABSTRACT

Open and percutaneous pedicle screw insertion would have the risk of damaging the nerve or blood vessels,due to the complexity of the spine structure.Despite the aid of various means of preoperative CT and intraoperative navigation,pedicle screw insertion failure rate is still 4.1%-28.9%.In recent years,the development of spine surgical robots might bring new solutions to this problem.The use of the spine surgical robots has been more than 20 years,and developed rapidly with the development of intraoperative navigation and intraoperative CT in recent years.Some robots have been certified by the FDA or the CE.And the number of reported robot-assisted surgeries such as scoliosis surgeries or severe lumbar spondylolisthesis surgeries has increased rapidly as well.Current literatures have verified the robot-assisted pedicle screw insertion has high accuracy,good stability and short learning curve,which may be able to become one of the main trend of spinal surgery in the future.This article mend to review some developed spine surgery robot and their clinical application indication and results.

ABSTRACT

OBJECTIVE:To investigate the situation of hospital pharmaceutical administration in China,and provide basis for developing measures in improving the quality and safety of medication in hospitals. METHODS:According to the organization of National Health and Family Planning Commission,National Quality Control Center of Pharmaceutical Administration drafted the re-search indicators,and adopted internet survey to screen,confirm and statistically analyze the data about pharmacy staffing and drug use management of hospitals in 2014 that was completed and reported as requested by the secondary general hospitals and above in each area selected by Health and Family Planning Commission in provinces (cities,districts). RESULTS:The rates of pharmacy technicians in secondary and tertiary general hospitals were 5.2% and 4.4%,the numbers of clinical pharmacists per hundred-bed were 0.42 and 0.44 person,the numbers of dispensing prescription per person in a day were 119 and 173 in pharmacist in outpa-tient;the monitoring rates for key patients were 1.8% and 6.9%,the rates of intravenous infusion in outpatients were 12.8% and 10.1%,and inpatient were 93.1% and 90.4%;the rates of intravenous infusion of antibiotics in inpatients were 51.8% and 44.3%,and the rates of intravenous infusion of TCM were 34.7% and 29.0%,respectively. CONCLUSIONS:The problems exist-ing in pharmaceutical quality administration in China include lacking of pharmacy technicians,large workload,lacking of clinical pharmacists,imperfect pharmaceutical care model,non-standardized choice and use of intravenous infusion,etc. For these prob-lems,it is suggested to increase the propaganda of health education and management efforts of intravenous drugs,equipped with ad-equate pharmacy technicians and clinical pharmacists,continuously improve the quality evaluation system of hospital pharmaceuti-cal administration in China.
