ABSTRACT

Basing on data, concerning the injured persons state, the investigations results for elaboration and application of statistic models, prognosticating the trauma outcome, were analyzed. Selected information about 373 injured persons, who were treated in Traumatic Shock Department, is presented. There was established, that the statistical models elaborated is expedient to use together with other qualitative and quantitative methods of prognostic determination of outcome in severely injured persons.

ABSTRACT

In the clinic in 2000 - 2013 yrs of 42 injured persons with severe combined trauma of abdominal organs were treated, in 18 of them the method of a multi-staged treatment (damage control) with a short-term operative intervention on the first stage was applied, what permitted to lower postoperative lethality by 22.3%, and rate of purulent-septic complications--by 18.1%.

ABSTRACT

The results of complex treatment of 40 injured persons with severe thoracic trauma, in whom pulmonary abscesses and pleural empyema had occurred postoperatively, were analyzed. Complex treatment of pulmonary abscesses and pleural empyema, using the methods of a local superhighfrequency irradiation of purulent foci have promoted their accelerated regression occurrence and reduction of a systemic inflammatory reactions severity, as well as prophylaxis of secondary purulent-septic complications, permitting to achieve lethality lowering and to escape practically reoperations performance.

ABSTRACT

The results of surgical treatment analysis was conducted in 39 injured persons, aged 16-74 yrs, who suffered severe trauma and pancreatitis and in whom the Damage Control tactics (DCT) was applied for massive bleeding and vasodilatation shock occurrence. According to TRISS scale for injured persons, suffering severe trauma, operated on using the DCT principles, the primary "approximate" prognosis of the death ("blunt") had constituted (37.4 +/- 24.7)% at average (range, 10.7-89.7%, mediana 31.7%); the primary "accurate" ("penetrating") prognosis--(39.56 +/- 22.14)% (range, 12-91.3%, mediana 39.65%); the severity of general state according to APACHE II scale was estimated as (19.54 +/- 7.65) points at average (range, 12-36 points, mediana 16 points). The characteristic for main principles of operation performance, using the DCT, was suggested. Of the total of 39 injured persons 12 (30.8%) did not survive. Postoperative complications had occurred in 18 (46.2%) of patients.

ABSTRACT

Issues of arterial hypoxemia treatment in early period of traumatic disease, taking into account modern approaches, adopted in surgery of injuries and anesthesy-resuscitation. It is recommended to use the method proposed for the arterial oxygen content enhancing in an acute respiratory insufficiency of various genesis, including the cases of progressive posttraumatic disease.

ABSTRACT

Results of surgical treatment of 148 injured persons with polytrauma in dominating damage of the abdominal cavity organs and concurrent damage of various anatomic-functional regions were analyzed. Taking into account the delineated reanimation-operative period there were presented surgical and reanimation tactic, intensive infusion-transfusion therapy in an acute massive blood loss, which was conducted in conditions of artificial pulmonary ventilation and general multicomponent anesthesy performed. Introduction of the tactic elaborated into the clinical practice had promoted significant improvement of the treatment results in injured persons with polytrauma.
