ABSTRACT

The diagnosis of ulnar-sided wrist symptoms concentrates on distal radioulnar joint and triquetral-hamate joint pathology. I consider this is only looking at the "tip of the iceberg" and ignoring other possible pathologies. In particular, this ignores the role of triquetrohamate and pisotriquetral pathologies. I outline our approaches to these pathologies noting the important ligamentous structures, the clinical presentations, the relevant investigations, and the surgical treatments and outcomes that I have found to be reliable. I would encourage hand surgeons to think more widely about ulnar-sided wrist symptoms, in particular triquetrohamate and pisotriquetral joint instabilities.

ABSTRACT

The pronator teres syndrome is a set of signs and symptoms that result from compression of the median nerve in the upper forearm. It is a dynamic syndrome that is frequently associated with compression of the same nerve at the carpal tunnel. The literature describes different anatomic structures that can cause this syndrome. Experience showed us that the deep fascia of the superficial head of the pronator teres is the only anatomic structure that causes compression of the median nerve in the upper forearm. If the flexor digitorum test is negative, selective release of the deep fascia of the superficial head of the pronator teres ends the symptoms. Our surgical technique uses a mini-invasive approach that takes into account the anatomic knowledge of this region. A 3.5-cm oblique skin incision is done 6 cm distal to the medial epicondyle, over the flexor/pronator muscle mass. The medial cutaneous nerve is atraumatically retracted. The superficial fascia of the flexor/pronator muscles is opened transversely. With the section of the septum between the pronator teres and the flexor carpi radialis, access to the deep fascia of the superficial head of the pronator teres is obtained. This structure is released. The median nerve is now easily visualized, and other types of possible compression causes are excluded. The same surgeon treated 44 cases with this technique. Two assistants were always needed for this mini-invasive approach. Pronator teres symptoms disappeared in 93% of cases.

ABSTRACT

UNLABELLED: We present a new technique for the correction of thumb metacarpophalangeal joint hyperextension in patients with concomitant basal thumb osteoarthritis. For consistent result with trapeciometacarpal arthroplasty both pathologies should be treated at the same time. INDICATION: Cases with passively reducible metacarpophlangeal joint (without osteoarthritis or severe medial instability). We use a midlateral approach to transfer the volar half of the abductor pollicis brevis to the radial insertion of the A1 pulley distal to the MP joint. The transfer should be with maximum tension with the joint in 20 degrees of flexion. We protect this transfer with immobilization for 1 month.

ABSTRACT

The modified Zancolli arthroplasty for basal thumb arthritis is an alternative to different procedures described for this pathology. As most of the methods, it includes complete trapeziectomy. The main difference is that the interposition is made with capsule (resembling Colonna's method for the hips) and that the base of the metacarpal is stabilized in the correct place by a tendon transfer (performed with an accessory of abductor pollicis longus). The main advantage of this method is the short immobilization time (2 wk) and high percentage (95%) of complete pain-free results and a stable and strong thumb in key pinches.

ABSTRACT

Desde 1996, hemos utilizado el nuevo abordaje supraacromial en 114 casos, con el objetivo de disminuir el dolor y la impotencia funcional producidos por el acceso entre el deltoides anterior y medio. Se realiza seccionando la fascia delto-trapecial y disecando subperiosticamente hacia anterior ; se incluye en el colgajo la mitad anterior de la capsula superior y la capsula anterior de la articulacion acromioclavicular. Hemos utilizado con exito este abordaje para acromioplastias, remodelacion o reseccion de la clavicula distal y reconstruccion de roturas de sustancia total del manguito rotador (incluso en roturas masivas con retraccion). El dolor y la impotencia funcional del posoperatorio son similares a los de la tecnica artroscopica, ya que el abordaje no ingresa entre los fasciculos musculares del deltoides

ABSTRACT

Se describe una nueva forma de inestabilidad, del lado cubital de la articulación mediocarpiana. La hemos denominado disociación mediocarpiana. Se produce por la lesión del ligamento piramidoganchoso cubital, que presenta inserciones en el piramidal y el ganchoso y se encuentra en la profundidad del piso del compartimento del cubital posterior. La disociación piramidoganchosa difiere clínica y fisiopatológicamente de la inestabilidad descrita en la bibliografía como inestabilidad mediocarpiana. Para esta nueva patología, el autor diseñó una técnica de reconstrucción ligamentaria anatómica con los tejidos locales (ligamento, piso del 6º compartimento y tendón del cubital posterior). Fueron tratados 13 pacientes: 8 curaron con tratamiento conservador ; infiltraciones y fisioterapia. Han sido operados 5 pacientes. Luego de un seguimiento promedio de 38 meses no hubo dolor posoperatorio y

Luego de un seguimiento promedio de 38 meses no hubo dolor posoperatorio y se reintegraron en forma total al trabajo. Dos volvieron a la práctica deportiva previa y los tres restantes no hacían deportes antes de la operación
