ABSTRACT

As fissuras labiopalatais são malformações faciais com sequelas funcionais e estéticas que causam grande impacto na qualidade de vida dos pacientes. O tratamento desses indivíduos requer uma abordagem multidisciplinar, incluindo tratamento plástico cirúrgico, fonoaudiológico e psicológico, além das intervenções odontológicas, ortodônticas e protéticas. Este relato de caso apresentou a reabilitação protética de uma paciente de 57 anos com fissura palatina anterior, com remoção da pré-maxila. Após vários tratamentos, a paciente ainda não havia sido reabilitada, apresentando deformidades provenientes dos tratamentos anteriores incompletos. Uma prótese parcial removível retida por dentes e implantes osseointegrados foi executada, promovendo a melhora na aparência facial, na função mastigatória, na fala e, consequentemente, na qualidade de vida da paciente.


Cleft lip and palate, which is a facial malformation with both functional and aesthetic characteristics, may have a large impact on the lives of those affected. Its correction requires an interdisciplinary approach, which includes surgical, orthodontic and prosthetic treatments. This clinical report presents the prosthetic rehabilitation of a 57-year-old woman with an anterior cleft palate defect that had not been closed completely with surgical repair. In this way, an implant-supported, retained removable partial prosthesis was delivered to improve the patient's facial aspect, masticatory function, speech, and her quality of life.

ABSTRACT

AIM: The objective of this cross-sectional study was to examine the potential of peri-implant crevicular fluid (PICF) analytes to discriminate between peri-implant health and disease using a multi-biomarker approach. METHODS: We collected PICF samples from the mesio-buccal site of every implant (n = 145) from 52 subjects with peri-implantitis and measured the levels of 20 biomarkers using Luminex. We grouped implants and subjects based on the clinical characteristic of the sampled sites and implants into: healthy sites from healthy implants (HH), diseased sites from diseased implants (DD) and healthy sites from diseased implants (HD). The significance of the differences between the HH and DD groups was determined using general linear models controlling for false discovery rate. We used logistic regression to determine the best multi-biomarker models that could distinguish HH from DD subjects and HH from HD subjects. RESULTS: There were statistically significant differences between HH and DD groups for 12/20 biomarkers. Logistic regression resulted in a 6-biomarker model (Flt-3L, GM-CSF, IL-10, sCD40L, IL-17 and TNFα) that discriminated HH from DD subjects (AUC = 0.93) and a 3-biomarker model (IL-17, IL-1ra and vascular endothelial growth factor) that distinguished HH from DD subjects (AUC = 0.90). CONCLUSION: PICF biomarkers might help discriminate peri-implant health from disease.

ABSTRACT

Background: Mucoperiosteal fl ap surgeries (MFS) are carried out to provide access to the alveolar bone and root surfaces in several clinical situations. Nevertheless, they lead to a variable degree of alveolar bone resorption. Raloxifene is an agonist in bone, and acts inhibiting bone loss. Objective: To evaluate the effect of raloxifene in preventing alveolar bone resorption after MFS using an experimental model of mouse mandibles. Methods: MFS was performed on the buccal aspect of the left side of the mandible (BL) in 20 male CF1 Musdomesticus mice divided into two groups with the same number of animals: the experimental group was treated once daily with raloxifene injections (3 mg/kg), and the placebo group was treated with daily injections of the vehicle. The buccal aspects of right hemimandibles were used as controls (BR). Mandibles were removed, defl eshed and stained with toluidine blue in a stereomicroscope. Digital images were obtained and the alveolar bone loss was measured (mm²) using an image analysis software. Results: The BL area exhibited signifi cantly more bone loss (Student t test; p < 0.01) when compared to the BR area, in both groups. No statistically signifi cant difference was observed between the experimental and the placebo groups. Conclusion: In this study, raloxifene did not inhibit alveolar bone resorption following MFS in male mice.


Introdução: Cirurgias de retalho mucoperiosteal (CRM) são realizadas para permitir acesso ao osso alveolar e à superfície radicular em várias situações clínicas. No entanto, elas levam a um grau variável de reabsorção óssea alveolar. O raloxifeno tem ação agonista em tecido ósseo e atua inibindo perda óssea. Objetivo: Avaliar o efeito do raloxifeno na prevenção de reabsorção óssea alveolar após CRM usando o modelo experimental de mandíbulas de camundongos. Métodos: Foram realizadas CRMs na face vestibular, lado esquerdo, das mandíbulas (VE) de 20 camundongos CF1 Musdomesticus machos, divididos em dois grupos com mesmo número de animais: o grupo experimental foi tratado uma vez ao dia com injeções de raloxifeno (3 mg/kg), e o grupo placebo foi tratado uma vez ao dia com injeções do veículo. A face vestibular do lado direito da hemimandíbula foi usada como controle (BD). As mandíbulas foram removidas, dissecadas e coradas com azul de toluidina sob um estereomicroscópio. Imagens digitais foram obtidas e a perda óssea alveolar foi medida (mm²) usando um software de análise de imagens. Resultados: A área VE exibiu perda óssea signifi cativamente maior (teste t de Student; p < 0,01) quando comparada com a área BD, em ambos os grupos. Não foi observada diferença estatisticamente signifi cativa entre os grupos experimental e placebo. Conclusão: Neste estudo, o raloxifeno não inibiu a reabsorção óssea alveolar após CRM em camundongos machos.

ABSTRACT

Evaluation of the influence of laser application on osseointegration around implants with different surface characteristics is limited. This study aims to evaluate the influence of low-level lasers on the early stages of osseointegration. Ninety-six external hex implants (3.75 mm × 5.0 mm) were placed in 24 rabbits-one machined and one sandblasted acid-etched per tibia. The rabbits were later divided into the laser group, which received a total dose of 24 J/cm(2) of gallium-aluminum-arsenide laser over 15 days, and a control group. At 16 and 30 days after surgery, removal torque and histomorphometric analyses were performed. No statistical differences in removal torque or histomorphometric analyses were verified between laser and control groups regardless of implant surface (P > .05). Time was the only variable presenting significant differences between measurements (P < .05). Low-level laser had no significant short-term effect on bone-to-implant contact and removal torque values regardless of implant surface characteristics.

ABSTRACT

Implantes osseointegrados podem ser instalados cirurgicamente, em diversas fases temporais, após a extração dentária. A escolha do prazo depende de fatores estruturais ósseos, estéticos e funcionais. A técnica de implante imediato pós-exodontia preserva as cristas marginais ósseas, o que é determinante para o sucesso estético da reabilitação. A razão para esse procedimento é também a de diminuir o tempo de tratamento e custo. Neste artigo, é relatado um caso clínico no qual foi realizada a extração e colocação imediata do implante, porém sem a aplicação de carga imediata.


Osseointegrated implants can be installed after tooth's extraction at various moments. The choice of this depends on bone, functional and aesthetic aspects.The aim of this paper is to demonstrate the advantages of the immediate implant placement in extraction sites, when it is indicated. The immediate implant surgery preserves the alveolar bone height and width, and it is very important to the successful esthetic outcome. And also, the treatment time and costs are reduced. In this article will be described a clinical case of immediate implant placement into extraction sockets, but without immediate loading.

ABSTRACT

PURPOSE: To assess osseointegration by measuring the removal torque of smooth cylindrical implants placed in rabbit tibiae, having been subjected, or not, to low-intensity laser irradiation (GaAlAs). METHODS: Two implants were inserted into one tibiae of 16 male New Zealand rabbits weighing 3.5-4.0 kg; the animals were divided into four groups (control and laser at 16 and 30 days of wound healing). In the irradiated groups, laser was applied at a 830-nm wavelength, in continuous mode and with 100 mW of potency, and the applications were punctual, with 1 J/cm³ on three points, and were repeated eight times at 48-hour intervals starting immediately after implantation surgery, thus totaling 24 J/cm². After the wounds healed, the animals were sacrificed, the tibiae were removed, implant removal torques were measured by means of a handheld digital torque-meter, and a factorial variance analysis statistical test was applied. RESULTS: There was significant intergroup difference regarding time as a single factor. The removal torque observed in the control groups was, for smooth implants, 20.42±3.06 N/cm at 16 days and 27.42±19.65 N/cm at 30 days; in the irradiated groups, the removal torque for smooth implants was 19.15±4.65 N/cm at 16 days and 30.08±4.14 N/cm at 30 days. CONCLUSION: Removal torque values were higher at 30 days than at 16 days, independent of laser application.


OBJETIVO: Avaliar a osseointegração através do torque de remoção de implantes cilíndricos lisos, colocados em tíbias de coelhos, submetidos ou não à irradiação com laser de baixa intensidade (AsGaAl). METODOLOGIA: Foram utilizados 16 coelhos machos da raça Nova Zelândia, pesando entre 3,5 a 4,0 quilos, sendo inseridos dois implantes em uma tíbia, dividida em quatro grupos (controle e laser para 16 e 30 dias de cicatrização). No grupo irradiado foi aplicado laser com comprimento de onda de 830 nm, modo contínuo e potência de 100 mW, sendo a aplicação pontual de 1 J/cm² em três pontos e oito aplicações, com intervalo de 48 horas, iniciada imediatamente após a cirurgia de colocação dos implantes, perfazendo um total de 24 J/cm². Após o período de cicatrização, os animais foram sacrificados, e então, realizada a remoção das tíbias e, em seguida, a medição dos torques para remoção dos implantes com torquímetro digital manual, aplicando-se o teste estatístico de Análise de Variância Fatorial. RESULTADOS: Foi verificado que existe diferença significativa intra-grupos em relação ao fator isolado tempo. O torque de remoção obtido para o grupo controle: Implantes lisos com 16 dias (20,42±3,06 N/cm) e 30 dias (27,42±19,65 N/cm); e para o grupo irradiado: Implantes lisos com 16 dias (19,15±4,65 N/cm) e 30 dias (30,08±4,14 N/cm). CONCLUSÃO: Os valores do torque de remoção no período de 30 dias são superiores ao período de 16 dias, independentemente da aplicação de laser.

ABSTRACT

Introdução: Os implantes osseointegrados foram introduzidos no início dos anos 1980, possibilitando outras alternativas de tratamento protético reabilitador. Muitos problemas oriundos das próteses convencionais foram solucionados por intermédio da osseointegração, porém complicações estéticas, fonéticas, funcionais, biológicas e mecânicas podem acontecer caso um adequado plano de tratamento não seja executado previamente à cirurgia do paciente. Objetivo: O objetivo do presente trabalho foi relatar a remoção de implantes dentários mal posicionados, em virtude da falta de planejamento prévio contraindicando a reabilitação protética. Relato de caso: Paciente do sexo feminino, 36 anos de idade, foi submetida à cirurgia de remoção de dois implantes, por causa de posicionamento inadequado e falha na osseointegração. Conclusão: O uso de todos os métodos diagnósticos disponíveis no planejamento, juntamente com a abordagem clínica adequada, é imprescindível para o sucesso do tratamento com implantes, evitando ao máximo a necessidade de remoção e substituição.


Introduction: Osseointegrated implants were introduced in the beginning of the 80s, allowing other alternatives in the prosthetic rehabilitation treatment. Many conventional prostheses' problems were solved by osseointegration. However, some aesthetic, phonetic, functional, biological and mechanical complications may happen if a proper treatment planning is not made prior to the patient?s surgery. Objective: The aim of this study was to report the removal of dental implants badly located due to the lack of previous planning contraindicating the prosthetic rehabilitation. Case report: Female patient, 36 years-old, was submitted to removal surgery of two dental implants, due to inadequate position and lack of osseointegration. Conclusion: The use of all available diagnostic methods during planning, associated to adequate clinical approach, is very important to the success of the treatment with implants, avoiding the need for removal and substitution.
