ABSTRACT
THE CLINICAL APPEARANCE OF KERATOMYCOSIS IS FAIRLY CHARACTERISTIC, AND A PROBABLE DIAGNOSIS OF THE DISEASE MAY BE MADE WHENEVER SOME OR ALL OF THE FOLLOWING SYMPTOMS ARE PRESENT TO A GREATER OR LESSER DEGREE: VIO LENT OCULAR REACTION, WITH PHOTOPHOBIA AND CILIARY IRRITATION. ULCER ON THE CORNEA, VARYING FROM A FEW MILLIMETERS TO THE SIZE OF THE ENTIRE CORNEA. IN SOME CASES, HYPHA-LIKE RADIATIONS APPEAR ON THE EDGE OF THE ULCER THROUGHOUT THE STROMA; THEY ARE INFILTRATED LEUKOCYTES. SOMETIMES SMALL ROUND SATELLITE LESIONS ARE ALSO SEEN. A DENSE WHITE SPOT CONSIS TING OF FIBRIN AND LEUKOCYTES ON THE CORNEAL ENDOTHELIUM. A CHARACTERIS TIC, STERILE OR CONTAMINATED HYPOPYON, WHICH MAY EVEN OCCUR WITH SMALL LESIONS

Cladorrhinum spp (strain CBS 604.75) was isolated, for the first time, from a case of keratomycosis. The patient, a 12 year-old boy, showed a soft white yellowish mycotic excrescence with clear borders which had followed the introduction of a small piece of straw into the cornea. The direct examination of corneal scrapings revealed dark-brown filaments. He was successfully treated with miconazole.

Reviewing the world literature and including some personal communications and our own case, we have found 14 cases of keratomycosis by Monosporium apiospermum or by its perfect state Petrillidium (Allescheria) boydii. A table with only the 12 well-documented observations was prepared.

The Drechslera state of Cochiobolus spicifer, Nelson 1964, was isolated from a case of keratomycosis. The patient, a 19 year old man, showed a large corneal ulcer with hypopyon associated with the introduction of dust. The direct examination of several scrapings revealed dark-brown hyphae. This species has been reported as a casual agent of a nodular granulomatous mass in the foot of a cat and in the skin of a horse.

World literature on the subject reports 112 cases of keratomycosis by Fusarium, including the two cases described in this paper and some personal communications. In most of these cases (60--70%) there is a history of trauma inflicted by vegetable matter or by foreign bodies introduced into the eye (metal, sand, stone, etc.). The different species of Fusarium isolated are F. episphaeria, F. dimerum, F. moniliforme, F. nivale, F. oxysporum, and F. solani. The treatment recommended in a 5-percent suspension or unguent of pimaricin (natamycin).
