ABSTRACT

OBJETIVO: El tumor de Wilms o nefroblastoma es el tumor renal más común en la infancia. Su incidencia en adultos es escasa, suponiendo acerca del 1 por ciento de los casos, con aproximadamente 200 casos descritos en la literatura. Aportamos un nuevo caso de tumor de Wilms en adulto y hacemos una breve revisión. OBSERVACIÓN CLÍNICA: Presentamos un caso de tumor de Wilms en un varón de 15 años, con alteraciones génito-urinarias asociadas, diagnosticado recientemente y tratado con nefrectomía radical y quimioterapia post-operatoria. CONCLUSIONES: La presentación del tumor de Wilms en adultos es excepcional, y su diagnóstico definitivo sólo puede realizarse tras el estudio anatomopatológico. La aplicación de los protocolos terapéuticos realizados habitualmente en pediatría con llevan unas tasas de curación meno-res y un peor pronóstico. Protocolos terapéuticos más agresivos son necesarios para elevar el pobre pronóstico aunque la escasa incidencia hace difícil el poder establecer el tratamiento más efectivo (AU)

ABSTRACT

OBJECTIVE: Wilms' tumor or nephroblastoma is the most common renal tumor of childhood. The incidence in adults is rare, estimated at about 1% of all cases and approximately 200 cases have been described in the world literature. We report a new case of adult Wilms' tumor and realize a short review. METHODS/RESULTS: We describe a case of Wilms' tumor in a young male, aged 15 years, with associate genitourinary abnormalities. The patient underwent radical nepherctomy with postoperative chemotherapy. CONCLUSIONS: The Wilms' tumor in the adult population is exceptional, and the definitive diagnostic is based in the findings of the pathological analysis. The application of therapeutic protocolos for children to adults offers an smaller percentage of cure and poorer prognostic. Therapheutic protocolos more aggressive are necessary for increase the worse prognostic in the adult, but because of the rarity is difficult to define the most effective form of treatment.

ABSTRACT

The association of a carcinoid tumour with non-endocrine urologic neoplasms is an infrequent finding. This paper contributes one case of multiple primary neoplasm where a prostatic adenocarcinoma coexists with an intestinal carcinoid tumour in a 63-year old asymptomatic patient. Diagnosis of prostate cancer was achieved during a screening for this conditions whereas the intestinal carcinoid tumour was found in the extension study.

ABSTRACT

The development of echographic transducers allows now to study Female Urinary Exertional Incontinence (FUEI) using different approaches (vaginal, perineal and transrectal). A total of 122 transrectal echographies were performed. Based on the clinical symptoms: 14 patients are continent, 15 report micturition urgency, 11 micturition urgency plus FUEI, and 82 FUEI. All parameters Xrest, Yrest, AUVPrest, Xexertion, Yexertion, AUVPexertion, X', Y', AUVP' were measured. Three groups were defined: Group I, normal ultrasound diagnosis; Group II, vesical neck hypermobility; Group III, FUEI using Kolmogorov-Smirnov's test, the data fits a standard distribution curve and the results were analyzed through a variance analysis and Newman-Keul's test. During Valsalva's manoeuvre, patients with FUEI show greater caudal mobility of the vesical neck which becomes more significant as the degree of FUEI increases. Dorsal mobility is also greater in patients with FUEI although this parameter is of no use to quantify the degree of FUEI. Posterior urethro-vesical angle variability only discriminates patients with no FUEI from those with a higher degree of FUEI.

ABSTRACT

UNLABELLED: Antimicrobial prophylaxis in surgery has proven to be effective in controlled randomized trials. Usage in Urology is known at least since the '30s although its effectiveness has only become known since 1979. METHODS: Review of literature related to surgical antibiotic prophylaxis, more specifically urological surgery, basically from 1991 to 1995, but without overlooking those papers that have become classics due to their impact. RESULTS AND CONCLUSIONS: Efficacy of antimicrobial prophylaxis in urological surgery is nowadays beyond all doubt. Usage is indicated in the presence of sterile urine and dosage must be short, in single dosis in the immediate pre-operative or within 24 hours after the procedure. However, there is a number of issues that deserve to be treated in more detail for better understanding. Those are the establishment of adequate prophylactic regimes in renal transplantation and the use of antimicrobials based on their pharmacokinetic characteristics to optimize the prophylactic purpose.

ABSTRACT

INTRODUCTION AND OBJECTIVE: Urinary catheterization by means of an internal ureteral catheter in an usual procedure that may present complications. In this article we presented a case of perforation and later renal subcapsular hematoma after the placement of a "double-J" catheter. CONCLUSIONS: This complication, although rare, should be kept in mind. The manner of avoiding it is to maintain the tightness of the guidewire while inserting the ureteral catheter. Usually, the consequences are not life-threatening and are solved with conservative treatment. The administration of LFRE should be delayed until the resolution of the hematoma.

ABSTRACT

OBJECTIVE: To describe a case of paratesticular liposarcoma in a 69-year-old male. The literature is briefly reviewed and the prognostic and therapeutic aspects are discussed. METHODS/RESULTS: The patient was submitted to surgery. Pathological analysis of the surgical specimen disclosed a myxoid liposarcoma with undifferentiated pleomorphic areas. Patient follow up has shown no recurrence or metastasis. CONCLUSIONS: This tumor type develops in scrotal soft tissue, slowly increasing scrotal size, and should be distinguished from inguinoscrotal hernia. Together with physical examination, ultrasonography is generally useful. Treatment is by radical orchiectomy through an inguinal approach; the role of adjuvant therapy remains unclear. It has a good prognosis, with frequent local relapses; metastatic spread is uncommon.

ABSTRACT

The purpose of this paper is to report one case of left pyelic Wilms' tumour in a 17-year old male patient. Pre-operative diagnosis was complicated by a background of prior surgery on the same kidney due to a benign fibroepithelial polyp. The possible concomitences are analyzed while the relevance of an extensive pathoanatomical study to avoid possible diagnostic errors is emphasized.
