ABSTRACT

A patient with pulmonary hypertension and focal liver cirrhosis was hospitalized on the 12-th week of pregnancy because of threatened abortion and died after 2 days of hospital stay during which symptoms of portal and pulmonary hypertension progressed, decompensation of pulmonary heart and hepatic cellular insufficiency ensued and syndrome of disseminated intravascular coagulation developed.

ABSTRACT

Study of the hemostasis system of pregnant women at risk of developing fetal distress in the third gestation trimester showed the prognostic significance of platelet aggregation test as an additional method of diagnosis of placental insufficiency. If platelet aggregation index is more than 70%, a woman should be included in the group at risk of developing ante- or intranatal fetal distress.

ABSTRACT

Use of an algorithm for assessment of the severity of antenatal fetal distress and prediction of intranatal complications helped choose the optimal mode of delivery individually for each parturient and develop the strategy of rational management of spontaneous labor, which was confirmed by measurements of hydrocortisone levels in umbilical blood during delivery by different modes. Rational management of labor and optimization of delivery were conducive to a reduction of the incidence of grave distress in the newborns.

ABSTRACT

Introduction of a program of prophylactic and organizational and therapeutic measures helped reduce the incidence of massive blood loss in 741 women at various modes of delivery, including cases with obstetrical bleedings. The incidence of hemorrhages in the placental and postpartum periods reduced from 1.6 to 0.3%.

ABSTRACT

Use of cardiotocography as the fetal status index in complex with other criteria and medical algorithms in pregnant women during the IIIrd trimester helped better select the term and method of delivery for each patient with threatened fetal distress. The choice of the optimal individual method of delivery for each pregnant woman decreased the risk of the process of labor and reduced by 2.2 times the number of children born in a state of complicated hypoxia.
