ABSTRACT

A review of literature is presented and a case of congenital pseudoarthrosis of the right clavicle is described. Resection of the pseudoarthrosis, reduction and fixation with a screw gave good anatomical and functional result what made authors to recall this rare defect.

ABSTRACT

The results of treatment for severely slipped capital femoral epiphysis by biplanar wedge osteotomy of the femoral neck in 13 cases are presented. They were compared to the results quoted by both advocates and opponents of this method. The authors conclude that this method renders beneficial results but they depend greatly on surgical technique and experience.

ABSTRACT

Recently, more and more interest has been focused on the local bone lost, e.g. transient osteoporosis of the hip. It is believed that its etiology is similar to that Sudeck's disease. However, it is not clear, yet. Transient osteoporosis of the hip is more frequent in man than in women, at middle age as well as in the III trimester of pregnancy. The patients suffer from increasing hip pain without any definite cause. Laboratory findings do not show pathologies but X-ray reveal marked bone loss in the area of both the head and neck of the femoral bone with normal articular space. The symptoms disappear spontaneously within approximately few months, provided that no complications develop. At the same time, a normal bone structure in the proximal end of femoral bone is restored. The author presents a case of a 37-year old pregnant woman who suffered from rare complication of undiagnosed transient osteoporosis of the hip in the form of pathological compression fracture of the femoral bone neck, immediately below its head.

ABSTRACT

A case of inveterate, rotational dislocation in the atlanto-axial joint type III in 7 year old girl hit by a car was presented. Direct skull traction was applied resulting in complete reduction and regaining of the full range of motion in the cervical spine.

ABSTRACT

The purpose of this study is retrospective analysis of clinical application of lyophilized (freeze-dried), radiation-sterilized allografts in various orthopedic disorders. Grafts of this type have been produced by Polish tissue banks for many years, even before the production of deep-frozen grafts was started. This type of graft is still produced because of their easy storage, transportation, and distribution. Their application is successful in selected cases. Freeze-dried, radiation-sterilized cancellous or cortical-cancellous blocks, cancellous chips, or diaphyseal cortical struts were applied in 435 patients ranging from three to 74 years old. There were five main groups of procedures: posterior or anterior spine fusion, 224 cases; joint arthrodeses, 36 cases; reconstructive operations, 83 cases; filling of bone lesions, 59 cases; and posttraumatic reconstructive operations, 33 cases. The efficacy of application of lyophilized, radiation-sterilized allografts was estimated retrospectively on the basis of physical examination data and roentgenographic assessment collected during postoperative visits. Incorporation and rebuilding of the grafts as well as a satisfactory therapeutic effect were achieved in 394 (91%) patients. Excluding the cases of congenital and hypovascular pseudoarthroses, post-osteomyelitis gap and focuses, as well as Stage 2 bone lesions, it is suggested that lyophilized, radiation-sterilized allografts can be applied on the condition that intimate contact between the donor bone and host bed is complete and sustained.

ABSTRACT

Osteogenic arthritis of the hip in children depending on the localization of the primary focus of infection can be classified into three types of femoral, acetabular and pelvic origin. These types differ as to the pathology as well as clinical course of the disease. Diagnostics and treatment is discussed. It was found that improper treatment in the early stage led to the chronic form of the disease. Clinical and radiological picture of osteogenic hip arthritis as well as its treatment is discussed on the basis of 125 cases reviewed. Different sequel to osteogenic hip arthritis depending on pathological origin, stage of the disease and method of treatment were pointed out.

ABSTRACT

Etiology, pathology, clinical and radiological symptoms, and course of transient synovitis of the hip was discussed on the basis of authors observation of 144 children as well as on literature. It was found that unloading the joint by staying in bed with slightly flexed hip led to subsidence of pain and return of full range of movement in 10 to 21 days. Only in older children the symptoms may persist 4 to 5 weeks. Permanent consequences of transient synovitis of the hip as elongation of the extremity and coxa magna in X-ray pictures are rarely observed. Transient synovitis of the hip seems not to have etiological links with Perthes disease.

ABSTRACT

Clinical and radiological picture as well as pathological classification of non specific hip arthritis in children were presented after reviewing 100 cases and on the base of literature. Different course and sequel of different pathological types of hip arthritis were pointed out with emphasis on suppurative arthritis of the hip. The management of all types of hip arthritis was discussed. The necessity of surgical drainage of the joint in the first 5-7 days of suppurative arthritis as the only measure to prevent irreversible hip destruction was underlined. The method should become popular among pediatric surgeons and pediatricians who are the first ones to take care of a child in the early stage of disease.
