ABSTRACT

Glycemic variability (GV) refers to swings in blood glucose levels and is an emerging measure of glycemic control in clinical practice. It is associated with micro- and macrovascular complications and poor clinical outcomes in diabetic humans. Although an integral part of patient assessment in human patients, it is to a large extent neglected in insulin-treated diabetic dogs. This prospective pilot study was performed to describe canine within-day GV in non-diabetic dogs with the aim to provide a basis for the interpretation of daily glucose profiles, and to promote GV as an accessible tool for future studies in veterinary medicine. Interstitial glucose concentrations of ten non-diabetic, non-obese beagles were continuously measured over a 48-h period using a flash glucose monitoring system. GV was assessed using the common indices MAGE (mean amplitude of glycemic excursion), GVP (Glycemic variability percentage) and CV (coefficient of variation). A total of 2260 sensor measurements were obtained, ranging from 3.7 mmol/L (67 mg/dL) to 8.5 mmol/L (153 mg/dL). Glucose profiles suggested a meal-dependent circadian rhythmicity with small but significant surges during the feeding periods. No differences in GV indices were observed between day and night periods (p > 0.05). The MAGE (mmol/L), GVP (%) and CV (%) were 0.86 (± 0.19), 7.37 (± 1.65), 6.72 (± 0.89) on day one, and 0.83 (± 0.18), 6.95 (± 1.52), 6.72 (± 1.53) on day two, respectively. The results of this study suggest that GV is low in non-diabetic dogs and that glucose concentrations are kept within narrow ranges.

ABSTRACT

Adrenal computed tomography characteristics (aCTc) in healthy cats are known, but reference intervals for diseased cats are lacking. aCTc of cats without evidence of adrenal disease (NAD group) were compared to parameters of cats with possible concurrent adrenal disease (PAD group). The PAD group was assessed for adrenal masses or other morphological deviations using the NAD group as reference. Associations of aCTc with patient variables were explored, and all results were compared with published aCTc of healthy cats. No incidental adrenal masses were identified in the PAD group (n = 92), and only few aCTc differed compared to the NAD group (n = 30). The NAD group showed similar associations of patient variables and aCTc as length (right: 11.5 ± 2.2 mm, left: 11.8 ± 1.7 mm), width (right: 6.4 ± 1.2 mm, left: 5.4 ± 0.8 mm), height (right: 4.5 ± 0.9 mm, left: 4.5 ± 0.8 mm), attenuation (right: 33.1 ± 5.0 Hounsfield units, left: 32.5 ± 5.3 Hounsfield units) and position, but markedly more mineralization (right: 10%, left: 13.3%) than reported in healthy cats. This study provides references of aCTc for diseased cats without evidence of adrenal disease. The result suggests that adrenal incidentalomas seem to be rare in cats.

ABSTRACT

Objective of this study was to demonstrate the ubiquitous presence of glucose in urine of euglycemic cats by a highly sensitive glucose assay. The local electronic database was searched for results of quantitative urine glucose measurements in cats. A total of 325 feline urine glucose measurements were identified, of which 303 (93%) had been submitted by one of the co-authors working in a near-by small animal practice. After the exclusion of patients with kidney disease (n = 60), hyperthyroidism (n = 15), diabetes mellitus (n = 11), multiple diseases (n = 9) or steroid treatment (n = 3), as well as serial measurements (n = 87) and outliers (n = 8), the final study population consisted of 132 cats. Urine creatinine concentration was unavailable in five patients. Whereas all but one cat had glucose concentrations above the detection limit of the assay (0.11 mmol/L, Gluco-quant Enzyme Kit/Roche Diagnostics), no positive glucose dipstick test result (Combur 9-Test, Roche Diagnostics) was observed. The median (range) of urinary glucose concentration and the glucose-to-creatinine ratio (UGCR) was 0.389 (<0.11-1.665) mmol/L and 0.0258 (0.007-0.517) respectively. The UGCR was not affected by age, gender, breed or leukocyturia, whereas cats with hematuria had slightly higher values. Data show that so-called "basal glucosuria" is present in the majority of cats and by no means diagnostic for diabetes mellitus or renal glucosuria. This has to be considered when using bio-analytical methods with a low limit of quantification.
