ABSTRACT

The aim of investigation is to reveal dependence of indicators of a phagocytes-macrophageal link and cytokine regulation at the operated patients with malignant new growths from a way of anesthesia. 274 patients with a gastroenteric cancer which was exposed to radical operative interventions with use of various ways of anesthesia (total intravenous, inhalation and intravenous-epidural) are surveyed. Existence of dependence of indicators of a phagocytes link of immunity and cytokine regulation from a way of surgical anesthesia at oncological patients is revealed. Most the imbalance of the studied phagocytes and cytokine mechanisms was shown at total intravenous, in the smallest--at combined intravenous epidural anesthesia that serves one of factors for the recommendation of its priority use in oncosurgical practice.

ABSTRACT

Under analysis is an experience with using Clophelin (0.1 mg i.v. by a dropper per 150 ml of 0.9% solution of sodium chloride or Ringer solution against the background of epidural anesthesia with lidocaine 2%--300-500 mg or trimecain 2.5-3% 480-600 mg). Pronounced influence of Clophelin on the quality of epidural anesthesia is thought to be proved. It is recommended for using in operations on elderly oncogynecological patients.

ABSTRACT

The effect of perineural administration of narcotic analgesics on the efficacy of nerve trunk and plexus blockade has been studied in 290 patients with traumas and surgical diseases. It has been established that combined administration of a local anesthetic and narcotic analgesic into a brachial fascia and sciatic nerve ensures a more even, adequate and durable analgesia during and after surgery on the upper and lower extremities. Similarly, the results of paravertebral analgesia after abdominal surgery are improved. It is also concluded that thermographic registration of infrared irradiation intensity in a "pain" dermatome zone gives an objective information on the efficacy and duration of postoperative analgesia.
