ABSTRACT

Background: Adjuvant therapy is used to reduce the risk of hepatocellular carcinoma (HCC) recurrence and improve patient prognosis. Exploration of treatment strategies that are both efficacious and safe has been extensively performed in the recent years. Although donafenib has demonstrated good efficacy in the treatment of advanced HCC, its use as adjuvant therapy in HCC has not been reported. Objectives: To investigate the efficacy and safety of postoperative adjuvant donafenib treatment in patients with HCC at high-risk of recurrence. Design: Retrospective study. Methods: A total of 196 patients with HCC at high-risk of recurrence were included in this study. Of these, 49 received adjuvant donafenib treatment, while 147 did not. Survival outcomes and incidence of adverse events (AEs) in the donafenib-treated group were compared. Inverse probability of treatment weighting (IPTW) method was used. Results: The median follow-up duration was 21.8 months [interquartile range (IQR) 17.2-27.1]. Before IPTW, the donafenib-treated group exhibited a significantly higher 1-year recurrence-free survival (RFS) rate (83.7% versus 66.7%, p = 0.023) than the control group. Contrarily, no significant difference was observed in the 1-year overall survival (OS) rates between the two groups (97.8% versus 91.8%, p = 0.120). After IPTW, the 1-year RFS and OS rates (86.6% versus 64.8%, p = 0.004; 97.9% versus 89.5%, p = 0.043, respectively) were higher than those in the control group. Multivariate analysis revealed that postoperative adjuvant donafenib treatment was an independent protective factor for RFS. The median duration of adjuvant donafenib treatment was 13.6 (IQR, 10.7-18.1) months, with 44 patients (89.8%) experienced AEs, primarily grade 1-2 AEs. Conclusion: Postoperative adjuvant donafenib treatment effectively reduced early recurrence among patients with HCC at high-risk of recurrence, while exhibiting favorable safety and tolerability profile. However, these findings warrant further investigation.

ABSTRACT

Plasma membrane proteins (PMPs) play critical roles in a myriad of physiological and disease conditions. A unique subset of PMPs functions through interacting with each other in trans at the interface between two contacting cells. These trans-interacting PMPs (tiPMPs) include adhesion molecules and ligands/receptors that facilitate cell-cell contact and direct communication between cells. Among the tiPMPs, a significant number have apparent extracellular binding domains but remain orphans with no known binding partners. Identification of their potential binding partners is therefore important for the understanding of processes such as organismal development and immune cell activation. While a number of methods have been developed for the identification of protein binding partners in general, very few are applicable to tiPMPs, which interact in a two-dimensional fashion with low intrinsic binding affinities. In this review, we present the significance of tiPMP interactions, the challenges of identifying binding partners for tiPMPs, and the landscape of method development. We describe current avidity-based screening approaches for identifying novel tiPMP binding partners and discuss their advantages and limitations. We conclude by highlighting the importance of developing novel methods of identifying new tiPMP interactions for deciphering the complex protein interactome and developing targeted therapeutics for diseases.

ABSTRACT

BACKGROUND: To date, carbohydrate antigen 19-9 (CA19-9) and carcinoembryonic antigen (CEA) have been widely used for the screening, diagnosis and prediction of biliary tract cancer (BTC) patients. However, few studies with large sample sizes of carbohydrate antigen 50 (CA50) were reported in BTC patients. METHODS: A total of 1121 patients from the Liver Cancer Clin-Bio Databank of Anhui Hepatobiliary Surgery Union between January 2017 and December 2022 were included in this study (673 in the training cohort and 448 in the validation cohort): among them, 458 with BTC, 178 with hepatocellular carcinoma (HCC), 23 with combined hepatocellular-cholangiocarcinoma, and 462 with nontumor patients. Receiver operating characteristic (ROC) curves and decision curve analysis (DCA) were used to evaluate the diagnostic efficacy and clinical usefulness. RESULTS: ROC curves obtained by combining CA50, CA19-9, and AFP showed that the AUC value of the diagnostic MODEL 1 was 0.885 (95% CI 0.856-0.885, specificity 70.3%, and sensitivity 84.0%) in the training cohort and 0.879 (0.841-0.917, 76.7%, and 84.3%) in the validation cohort. In addition, comparing iCCA and HCC (235 in the training cohort, 157 in the validation cohort), the AUC values of the diagnostic MODEL 2 were 0.893 (95% CI 0.853-0.933, specificity 96%, and sensitivity 68.6%) in the training cohort and 0.872 (95% CI 0.818-0.927, 94.2%, and 64.6%) in the validation cohort. CONCLUSION: The model combining CA50, CA19-9, and AFP not only has good diagnostic value for BTC but also has good diagnostic value for distinguishing iCCA and HCC.

ABSTRACT

BACKGROUND: Colorectal cancer is the third most prevalent malignancy globally and ranks second in cancer-related mortality, with the liver being the primary organ of metastasis. Preoperative chemotherapy is widely recommended for initially or potentially resectable colorectal liver metastases (CRLMs). Tumour pathological response serves as the most important and intuitive indicator for assessing the efficacy of chemotherapy. However, the postoperative pathological results reveal that a considerable number of patients exhibit a poor response to preoperative chemotherapy. Body mass index (BMI) is one of the factors affecting the tumorigenesis and progression of colorectal cancer as well as prognosis after various antitumour therapies. Several studies have indicated that overweight and obese patients with metastatic colorectal cancer experience worse prognoses than those with normal weight, particularly when receiving first-line chemotherapy regimens in combination with bevacizumab. AIM: To explore the predictive value of BMI regarding the pathologic response following preoperative chemotherapy for CRLMs. METHODS: A retrospective analysis was performed in 126 consecutive patients with CRLM who underwent hepatectomy following preoperative chemotherapy at four different hospitals from October 2019 to July 2023. Univariate and multivariate logistic regression models were applied to analyse potential predictors of tumour pathological response. The Kaplan-Meier method with log rank test was used to compare progression-free survival (PFS) between patients with high and low BMI. BMI < 24.0 kg/m2 was defined as low BMI, and tumour regression grade 1-2 was defined as complete tumour response. RESULTS: Low BMI was observed in 74 (58.7%) patients and complete tumour response was found in 27 (21.4%) patients. The rate of complete tumour response was significantly higher in patients with low BMI (29.7% vs 9.6%, P = 0.007). Multivariate analysis revealed that low BMI [odds ratio (OR) = 4.56, 95% confidence interval (CI): 1.42-14.63, P = 0.011], targeted therapy with bevacizumab (OR = 3.02, 95%CI: 1.10-8.33, P = 0.033), preoperative carcinoembryonic antigen level < 10 ng/mL (OR = 3.84, 95%CI: 1.19-12.44, P = 0.025) and severe sinusoidal dilatation (OR = 0.17, 95%CI: 0.03-0.90, P = 0.037) were independent predictive factors for complete tumour response. The low BMI group exhibited a significantly longer median PFS than the high BMI group (10.7 mo vs 4.7 mo, P = 0.011). CONCLUSION: In CRLM patients receiving preoperative chemotherapy, a low BMI may be associated with better tumour response and longer PFS.

BACKGROUND: A substantial proportion of sensitized individuals tolerate suspected foods without developing allergic symptoms; this phenomenon is known as sensitized tolerance. The immunogenic and metabolic features underlying the sensitized-tolerant phenotype remain largely unknown. OBJECTIVE: We aimed to uncover the metabolic signatures associated with clinical milk allergy (MA) and sensitized tolerance using metabolomics. METHODS: We characterized the serum metabolic and immunologic profiles of children with clinical IgE-mediated MA (n = 30) or milk-sensitized tolerance (n = 20) and healthy controls (n = 21). A comparative analysis was performed to identify dysregulated pathways associated with the clinical manifestations of food allergy. We also analyzed specific biomarkers indicative of different sensitization phenotypes in children with MA. The candidate metabolites were validated in an independent quantification cohort (n = 41). RESULTS: Metabolomic profiling confirmed the presence of a distinct metabolic signature that discriminated children with MA from those with milk-sensitized tolerance. Amino acid metabolites generated via arginine, proline, and glutathione metabolism were uniquely altered in children with sensitized tolerance. Arginine depletion and metabolism through the polyamine pathway to fuel glutamate synthesis were closely associated with suppression of clinical symptoms in the presence of allergen-specific IgE. In children with MA, the polysensitized state was characterized by disturbances in tryptophan metabolism. CONCLUSIONS: By combining untargeted metabolomics with targeted validation in an independent quantification cohort, we identified candidate metabolites as phenotypic and diagnostic biomarkers of food allergy. Our results provide insights into the pathologic mechanisms underlying childhood allergy and suggest potential therapeutic targets.

ABSTRACT

Purpose: To investigate the clinical efficacy of avatrombopag, an oral thrombopoietin receptor agonist, versus subcutaneous recombinant human thrombopoietin (rh-TPO) in the treatment of severe thrombocytopenia (TCP) associated with chronic liver disease (CLD). Methods: Clinical data of 250 patients with severe TCP associated with CLD were collected in a single hospital from January 2019 to January 2022. The main parameters measured were the therapeutic response rate, changes in platelets (PLTs), and adverse events. Propensity score matching (PSM) was used to avoid possible selection bias. Results: After PSM, a total of 154 patients were enrolled in the study: 77 in the avatrombopag group and 77 in the rh-TPO group. There was no statistically significant difference between the two groups in the effect of increasing the PLT count (Waldχ 2 = 1.659, p = 0.198; Waldχ 2 = 0.220, p = 0.639). In addition, no interaction between time and different medications was found (Waldχ 2 = 0.540, p = 0.910; Waldχ 2 = 1.273, p = 0.736). Interestingly, in the subgroup analysis, both before and after PSM, avatrombopag showed better clinical efficacy than rh-TPO in the treatment of TCP associated with CLD in Child‒Pugh Class A (88.89% vs. 63.41%, p =0.003; 81.33% vs. 61.76%, p = 0.043). Fewer patients reported dizziness in the avatrombopag group than in the rh-TPO group both before and after PSM (7.8% vs. 25.0%; 7.8% vs. 24.7%, p < 0.05). Conclusion: Both before and after PSM, avatrombopag showed better clinical efficacy than rh-TPO in the treatment of TCP associated with CLD in Child‒Pugh Class A and showed a lower incidence of dizziness in all patients.

ABSTRACT

The purpose is to explore new cultivation modes of college students' national cognition and cultural acceptance. Deep learning (DL) technology and Educational Psychology theory are introduced, and the influence of art journal reading on college students' national cognition and cultural acceptance is analyzed under Educational Psychology. Firstly, the background of Educational Psychology, national cognition and cultural acceptance, and learning system are discussed following a literature review. The DL technology is introduced to construct the journal reading guidance system. The system can provide users with art journals and record the user habits like reading duration and preferences. Secondly, hypotheses are proposed, and a questionnaire survey is designed, with 12 specific indicators to investigate and collect research data. Finally, the collected data are analyzed. The results show that women's cognition of Chinese traditional culture, Chinese excellent revolutionary culture, and Chinese national identity is higher than that of men. By comparison, men's cognition of Chinese advanced socialist culture is higher than women's. After using the journal reading guidance system, the cognition of female college students on traditional Chinese culture is improved by 16.3%. Before and after reading art journals, the overall national cognition and cultural acceptance of Minority students are higher than that of Han students. The overall cognition of Literature and History students is higher than that of Science and Engineering students in traditional Chinese culture and China's excellent revolutionary culture and lower in advanced Chinese socialist culture and Chinese national identity. The overall cognition of college students' party members to the advanced socialist culture is higher than league members. As students read more art journals through the guidance system, their overall national cognition and cultural acceptance have increased. Therefore, reading art journals can promote college students' national cognition and cultural acceptance. A national cognition and cultural acceptance promotion system that conforms to the current situation of college students is constructed. The finding provides a reference for developing complex emotion recognition technology in human-computer interaction.

The related literature is studied to explore the psychological characteristics of juvenile delinquency groups and implement their psychological characteristics model for the intervention of health behavior. Drawing on the results of current literature research, the Youth Psychological Characteristics Crime Prevention Questionnaire (YPPQ) was compiled, which can be simply referred to as the Crime Prevention Questionnaire. The whole psychological characteristics of juvenile delinquency are analyzed by means of a questionnaire. Firstly, the YPPQ scores of different groups were compared, and a structured interview was conducted on the reasons for the crime of the problem youth group. Secondly, data analysis was carried out on the results of questionnaires and interviews, and the psychological characteristics of juvenile delinquency were summarized. A "mixed hierarchical intervention model" was proposed to intervene in the mental health behavior of juvenile delinquency groups, and corresponding intervention strategies were also proposed. The results reveal that through the questionnaire survey, the educational background of juvenile subjects was generally distributed in middle school, the number of juveniles with primary school education was less than 30% of the juvenile delinquency groups, the middle school education accounted for more than 60% of the juvenile delinquency groups, and the approximate age was about 18 years old. The largest number in each group were adolescents with secondary school education, indicating the importance of psychological education on crime prevention for adolescents in secondary school. By comparing the YPPQ test scores of different groups, the adolescent group has higher test scores than the juvenile delinquency groups in five of the dimensions. Through the comparative analysis of the YPPQ test results of the juvenile delinquency groups, the problem youth group, and the normal youth group, it is found that the YPPQ has high reliability and validity, so its detection and evaluation are highly feasible. By comparing the odds ratio (OR) of each question in the YPPQ test between the experimental group and the control group, it is found that the psychological characteristics of the experimental group are significantly affected by family, school, and even society. Finally, it proposes a "mixed hierarchical intervention model" for juvenile delinquency to intervene in health behaviors. The purpose is to provide some research ideas for the study of the psychological characteristics of juvenile delinquency groups and to put forward some suggestions for the prevention of juvenile delinquency and the intervention of health behavior.

ABSTRACT

A novel nanocomposite, named as nZVI@LH, was prepared by nanoscale zero-valent iron (nZVI) supported on lignin hydrogel and was used in the remediation of Cr(VI)-contaminated soil collected from an industrial site. Meanwhile, scanning electron microscopy with energy dispersive X-ray (SEM-EDX) and X-ray diffractometry (XRD) results determined that nZVI nanoparticles disperse uniformly on hydrogel. After the 14 days remediation, the immobilization efficiency of Cr(VI) could reach over 87% in the treatment of 3% (w/w%) nZVI@LH and 26% in the treatment of bare-nZVI. Leaching experiment results showed that the treatment group with 3% (w/w%) nZVI@LH was up to the national leaching toxicity identification standard, and there was no threat in simulation of acid rain over the long term. The water-soluble (WS) fraction in 3# nZVI@LH treatment decreased 31.1%, while the Fe-Mn oxide bound (OX) fraction and organic matter-bound (OM) fraction increased 10.9% and 13.4%, respectively. Moreover, nZVI@LH had limited impact on soil properties and the capability to immobilize Cr over a long period exposure to acid rain. This work prove that nZVI@LH has the potential to remediate Cr contaminated soil. Furthermore, details of possible mechanistic insight into the Cr remediation were carefully discussed.

ABSTRACT

A novel hydrogel-supported nanoscale zero-valent iron (nZVI) composite (nZVI@LH) was synthesized by ion exchange and in-situ reduction. The removal efficiency was tested, and the mechanism was also explored. The nZVI@LH at the precursor Fe(II) ion concentration of 0.1 mol/L presented an enhanced Cr(VI) removal capacity of 310.86 mg/g Fe0 at pH 5.3, which was 11.6 times more than that of the pure nZVI. The removal efficiency of the composite at pH 2.1 was more than double compared with alkaline or neutral conditions. Scanning electron microscopy (SEM) suggested that the nZVI particles were uniformly immobilized in the lignin hydrogel. Fourier transform infrared spectroscopy (FTIR) and X-ray photoelectron spectroscopy (XPS) provided evidence supporting the removal mechanism. According to the XPS results, the high removal capacity of the composite was attributed to chemical reduction/precipitation (69.7%), surface sorption (19.7%), and swelling uptake (10.6%). The pseudo-first-order reduction kinetics and pseudo-second-order kinetic model were employed to simulate the kinetic data, which supported the mechanism that chemical reduction and surface sorption could simultaneously remove Cr(VI). The electron acceptor and electron donor affected the reaction rate, and the presence of humic acid significantly inhibited the reaction. The present study demonstrated that lignin hydrogel acted as a carrier to prevent aggregation of nZVI particles. nZVI particles loaded on lignin hydrogel showed high reactivity and high degree of utilization compared with bare-nZVI. These results exhibited the great potential of nZVI@LH in practical water treatment due to its high activity.

ABSTRACT

Anaerobic digestion of sludge produces a large amount of sewage sludge anaerobic digestate (SSAD) that can be reused. A novel green substrate was prepared by mixing SSAD and its biochar (SSBC) filled with perlite and quartz sand for plant growth, as a replacement of soil. We carried out pot experiment, measured ryegrass biomass, seedling survival rate, and evaluated the emission of greenhouse gas (GHG), NH3 volatilization. The results showed that the seedling survival rate and individual biomass of ryegrass in green substrate were 100% and 100.02 mg, which were 14.4% and 231.4% higher than those in only SSAD, but were 1.3% and 19.6% higher than those in soil. SSBC significantly reduced N2O and CO2 emission, inhibited the NH3 volatilization, but increased CH4 emission. However, the cumulative emission of N2O and CH4 was approximation to that in soil. Global warming potential of CH4 and N2O (GWP(CH4+N2O)) green substrate was 11,842.01 kg CO2·hm-2, which was 1.35-fold higher than that of soil. Microbial community structure analysis showed that fermentative bacteria and methanogenic archaeal had a higher abundance in green substrate than in soil, which caused the different gas emission. This study will provide an effective and economical way to dispose excessive SSAD.

Cadmium (Cd) due to its strong toxicity and high mobility, which poses a considerable threat to soil environment and human health, has aroused widespread concern. Biochar has been used for remediating Cd-contaminated soil recently, however this method has the risk of fixed-Cd re-release. Phytoremediation can make up for its shortcoming. In this study, a pot experiment was carried out, where Bidens pilosa L. (B.pilosa) was as the tested plant and biochars (maize straw biochar and wheat straw biochar with two particle sizes) were as amendments. The mechanism of how biochars promoted B.pilosa Cd accumulation in Cd-contaminated farmland soil was explored. Results showed that the application of 5% wheat straw fine biochar (WF), wheat straw coarse biochar (WC), maize straw fine biochar (MF) and maize straw coarse biochar (MC) increased the total Cd accumulation of B.pilosa to 251.57%, 217.41%, 321.64% and 349.66%, respectively. Biochars amendment significantly promoted B.pilosa growth and increased Cd accumulation by improving soil physical properties, nutrient levels (available nitrogen, available phosphorus (AP), available potassium (AK) and organic matter (OM)) and microbial activity, and changing the nutrients distribution in B.pilosa organs although tissues although DTPA-Cd reduced to some extent. The effect of MF on AP increase was better than MC, while the effect of WF on AK increase was better than WC. Fine-particle was superior to coarse-particle in increasing B.pilosa biomass of aboveground, OM and microbial activity in soil. The changes of N, P and K concentrations in B.pilsosa roots, stems and leaves were closely related to the changes of AN, AP and AK in soil after biochars application. The results indicated that the combination of straw biochars and hyperaccumulators had the synergistic effect. This study can provide data support and meaningful reference values for remediating actual Cd-contaminated soil.

BACKGROUND: This study was to explore the potential mechanism of naprapathy in treating neuropathic pain (NP) after brachial plexus injury (BPI). METHODS: Totally 72 rats were randomly divided into normal group, model control group, and naprapathy group (n=24 per group). A right upper-limb chronic NP model was established, and naprapathy was administered at C5-T1 Jiaji Points on 4th day after modeling. Naprapathy was performed for 15 min once daily with a frequency of 60 per minute. The treatment was applied for altogether 28 days. Cold pain threshold and mechanical pain threshold were measured 1 day before modeling, 3 days after modeling, and 7, 14, 21 and 28 days after naprapathy; 7, 14, 21 and 28 days after naprapathy, rats were killed and the ß-endorphin expression and γ-aminobutyric acid (GABA) content were detected in the thalamus. RESULTS: After 14-day treatment, there was significant difference of mechanical pain threshold between the naprapathy group and the normal group (P<0.05); after treatment for 21 and 28 days, there was no significant difference between the naprapathy group and the normal group (P>0.05); after 28-day naprapathy, there was significant difference of ß-EP expression between the normal group and the naprapathy group (P<0.05), while the difference between model control group and naprapathy group was statistically significant (P<0.05). After 14-day treatment, there was significant difference of GABA content between the model control group and the naprapathy group (P<0.05). After 28-day treatment, significant difference was also found between the model control group and the naprapathy group (P<0.05). CONCLUSIONS: After naprapathy, chronic NP is attenuated in rats with BPI, which might be ascribed to the upregulation of ß-endorphin and GABA.

ABSTRACT

Electroacupuncture has traditionally been used to treat pain, but its effect on pain following brachial plexus injury is still unknown. In this study, rat models of an avulsion injury to the left brachial plexus root (associated with upper-limb chronic neuropathic pain) were given electroacupuncture stimulation at bilateral Quchi (LI11), Hegu (LI04), Zusanli (ST36) and Yanglingquan (GB34). After electroacupuncture therapy, chronic neuropathic pain in the rats' upper limbs was significantly attenuated. Immunofluorescence staining showed that the expression of ß-endorphins in the arcuate nucleus was significantly increased after therapy. Thus, experimental findings indicate that electroacupuncture can attenuate neuropathic pain after brachial plexus injury through upregulating ß-endorphin expression.

ABSTRACT

Developmental and environmental cues induce Ca(2+) fluctuations in plant cells. Stimulus-specific spatial-temporal Ca(2+) patterns are sensed by cellular Ca(2+) binding proteins that initiate Ca(2+) signaling cascades. However, we still know little about how stimulus specific Ca(2+) signals are generated. The specificity of a Ca(2+) signal may be attributed to the sophisticated regulation of the activities of Ca(2+) channels and/or transporters in response to a given stimulus. To identify these cellular components and understand their functions, it is crucial to use systems that allow a sensitive and robust recording of Ca(2+) signals at both the tissue and cellular levels. Genetically encoded Ca(2+) indicators that are targeted to different cellular compartments have provided a platform for live cell confocal imaging of cellular Ca(2+) signals. Here we describe instructions for the use of two Ca(2+) detection systems: aequorin based FAS (film adhesive seedlings) luminescence Ca(2+) imaging and case12 based live cell confocal fluorescence Ca(2+) imaging. Luminescence imaging using the FAS system provides a simple, robust and sensitive detection of spatial and temporal Ca(2+) signals at the tissue level, while live cell confocal imaging using Case12 provides simultaneous detection of cytosolic and nuclear Ca(2+) signals at a high resolution.

ABSTRACT

BACKGROUND: Brachial plexus injury is one of the difficult medical problems in the world. The aim of this study was to observe the clinical therapeutic effect of comprehensive rehabilitation in treating dysfunction after brachial plexus injury. METHODS: Forty-three cases of dysfunction after brachial plexus injury were divided into two groups randomly. The treatment group, which totaled 21 patients (including 14 cases of total brachial plexus injury and seven cases of branch brachial plexus injury), was treated with comprehensive rehabilitation including transcutaneous electrical nerve stimulation, mid-frequency electrotherapy, Tuina therapy, and occupational therapy. The control group, which totaled 22 patients (including 16 cases of total brachial plexus injury and six cases of branch brachial plexus injury), was treated with home-based electrical nerve stimulation and occupational therapy. Each course was of 30 days duration and the patients received four courses totally. After four courses, the rehabilitation effect was evaluated according to the brachial plexus function evaluation standard and electromyogram (EMG) assessment. RESULTS: In the treatment group, there was significant difference in the scores of brachial plexus function pre- and post-treatment (P < 0.01) in both "total" and "branch" injury. The scores of two "total injury" groups had statistical differences (P < 0.01), while the scores of two "branch injury" groups had statistical differences (P < 0.05) after four courses. EMG suggested that the appearance of regeneration potentials of the recipient nerves in the treatment group was earlier than the control group and had significant differences (P < 0.05). CONCLUSION: Comprehensive rehabilitation was more effective in treating dysfunction after brachial plexus injury than nonintegrated rehabilitation.
