ABSTRACT

OBJECTIVE@#To observe the changes of functional connectivity of brain pain-emotion regulation region in patients with cervical spondylosis of cervical type by functional magnetic resonance imaging (fMRI).@*METHODS@#Thirty-two subjects were selected. Of them, 16 patients with cervical spondylosis of cervical type were divided into an observation group and 16 healthy subjects into a control group. The patients in the observation group were treated with acupuncture at Tianzhu (BL 10), Jingbailao (EX-HN 15), Jianzhongshu (SI 15) and @*RESULTS@#In the observation group, the VAS score was (1.94±1.12) after the treatment, which was lower than (5.62±1.20) before treatment (@*CONCLUSION@#Pain involves the formation and expression of "pain-emotion-cognition". Acupuncture can systematically regulate the brain functional connections between cognitive regions such as dorsal prefrontal lobe and anterior cingulate gyrus and emotional regions such as insula and VTA in patients with cervical spondylosis of cervical type, suggesting that acupuncture has a multi-dimensional and comprehensive regulation effect on pain.

ABSTRACT

The self-assembly behavior of polypeptides plays an essential role to form biological and functional macromolecules, which have attracted a lot of attention due to their excellent characters. Understanding the polypeptide self-assembly systems and dynamic behaviors is fundamental to improve the potential of biomedical applications. In this work, coiled coil polypeptides PC10 and PC10 P were designed and biosynthesized. PC10 and PC10 P could form nanogels when the concentration of polypeptides was less than 2% (m/v). The dynamic behaviors of PC10 and PC10 P were measured by Förster resonance energy transfer method based on a capillary electrophoresis system. The Förster resonance energy transfer efficiency of this system was 60.4%, and the distance of self-assembled domains in the polypeptides was calculated as 6.14 nm, demonstrating that the exchange behavior occurred between two different polypeptides containing the same coiled coil region.

Tumors are highly complex systems. Understanding the compositions and functions of the tumor immune microenvironment is a prerequisite for effective tumor immunotherapy. Single-cell RNA sequencing can detect the transcriptome of a cell at the resolution of single-cell level,describe its functional status,and thus deepen the understanding of the composition and function of different cell clusters in tumor immune microenvironment. This article reviews the application of single-cell RNA sequencing in research on tumor immune microenvironment.

ABSTRACT

While immune checkpoint inhibitors(ICIs)are effective and promising treatments for a variety of malignancies,they also have safety concerns,especially the immune-related adverse events(irAEs).Unlike the side effects of traditional chemotherapy and targeted therapy,irAEs are adverse events caused by immune activation after ICIs treatment and thus may involve almost every system of the body.Therefore,biomarkers for predicting irAEs after ICIs treatment are urgently needed.Here we review the currently available predictive biomarkers of irAEs.

ABSTRACT

Amine-basedcarbon-capture technologies have been shown to be energetically expensive and to cause significant environmental and epidemiological impacts due to their volatility. Bicarbonate formation from carbon dioxide's reaction with water has been suggested as an effective alternative for capturing CO2; however, the thermodynamics of this reaction are not well understood. This study experimentally determined the equilibrium constant of sodium bicarbonate (NaHCO3) decomposition to sodium, water, and carbon dioxide; the study also compared the equilibrium constant to theoretical calculations. Using a combination of experimentation and thermodynamic relationships, the unitless equilibrium constants of the forward and reverse reactions were calculated accurately (error <±9% and <±4%, respectively). Equilibrium data were calculated using enthalpy and entropy values of each component of NaHCO3 decomposition at temperatures ranging from 25 to 155°C respectively. These results offer more data essential to optimizing NaHCO3 use in environmentally friendly next-generation CO2-capture technologies.

ABSTRACT

Background: Diagnostic performance of PET/CT using 18F-fluciclovine (18F-FACBC) in patients with prostate cancer (PCa) has been evaluated in only a few studies. There is no consensus on the diagnostic value of 18F-FACBC PET/CT in PCa recurrence or metastasis (except for bone metastasis), the primary diagnosis of the lesion. Hence, a meta-analysis was conducted to evaluate the performance of 18F-FACBC PET/CT. Methods: The literature published from June 2015 to June 2019 on using 18F-FACBC PET/CT for the diagnosis of PCa was retrieved from PubMed and EMBASE. Pooled sensitivity (Sen), specificity (Spe), positive and negative likelihood ratios (LR+ and LR-), area under the curve (AUC), and diagnostic odds ratio (DOR) of 18F-FACBC PET/CT in patients with PCa were calculated. An SROC map was made, and a meta-regression analysis was carried out. A Fagan plot and likelihood ratio dot plot were drawn. Sensitivity and funnel plot analysis were made. Meta-disc, Review Manager 5.3, and STATA 13 were used for the meta-analysis. Results: A total of nine articles met the strict criteria for diagnostic meta-analysis, which included 363 patients and 345 lesions. Pooled Sen, Spe, LR+, LR-, DOR were 0.88, 0.73, 3.3, 0.17, and 20, respectively. Lesions detected on the PET/CT image included primary lesions and metastases. For the lesion, the doctors considered the abnormal part as a lesion on the PET/CT image by their own experience and expertise, including primary lesions and metastases. For the patient, patients who participated in the trial can be diagnosed as PCa through 18F-FACBC. Conclusion: This study comprehensively evaluated the diagnostic value of 18F-FACBC PET/CT on PCa. Our analysis suggests that 18F-FACBC PET/CT is a valuable agent in diagnosing PCa. More studies are needed for further validation.

ABSTRACT

PURPOSE: Small bowel adenocarcinoma (SBA) is an uncommon malignancy with poor prognosis and therefore difficult to study. The purpose of this study was to evaluate the characteristics, treatments and prognostic factors in patients with metastatic or locally unresectable SBA. METHODS: Epidemiological and treatment data from metastatic or locally unresectable SBA patients who were admitted to Peking Union Medical College Hospital for first-line chemotherapy between December 2003 and November 2016 were retrospectively analyzed. RESULTS: Of the 34 enrolled patients, 22 (64.7%) were male and 12 (35.3%) female, with a median age of 52 years. Tumors originated in the duodenum in 24 (70.6%) patients. All patients received one of the following regimens as first-line therapy: FOLFOX or XELOX (n = 27), FOLFIRI or CAPIRI (n = 5), GEMOX (n = 1), and TP (n = 1). The response rate and disease control rate were 11.8 and 61.8%, respectively. The median progression-free survival (PFS) and overall survival (OS) were 4.5 and 13.8 months, respectively. Multivariate analysis revealed that liver metastasis was independently associated with poor PFS, and both unresected primary tumor and males were significantly associated with poor OS. The survival of three metastatic patients was 52-96 months after combination treatment of chemotherapy, resection of primary tumor and metastasis. CONCLUSIONS: The prognosis of metastatic or locally unresectable SBA was poor, and unresected primary tumor and males were significantly associated with poor OS. Combined modality therapy of systemic chemotherapy combined with local treatment of the primary tumor and oligometastasis might improve prognosis in selected patients.

ABSTRACT

Objective To investigate the expression of α-smooth muscle actin(α-SMA)in advanced colorectal cancer tissue and its association with chemotherapy response and prognosis.Methods The expression of α-SMA was detected by immunohistochemistry in tissues from 52 advanced colorectal cancer patients who received oxaliplatin plus 5-fluorouracil regimen as first-line chemotherapy. Its relationship with clinical characteristics,chemotherapy response,and survival were analyzed.Results Of these 52 patients,29(55.8%)were α-SMA overexpression,and the expression of α-SMA protein was not significantly associated with the age(χ =0.113,P=0.730),gender(χ =0.515,P=0.332),tumor location(χ =3.675,P=0.159),and tissue differentiation(χ =1.852,P=0.604). The chemotherapy resistance rate was 65.5%(19/29)in patients with high α-SMA expression,which was significantly higher than that (13.0%,3/23)in patients with low α-SMA expression(χ =14.470,P=0.000). Patients with high α-SMA expression exhibited a significantly shorter progression-free survival(PFS)compared with those with low α-SMA expression [(6.4±1.0)months vs.(16.0±3.5)months; χ=5.985,P=0.018]. Conclusion High α-SMA expression is associated with resistance to first-line chemotherapy and poor prognosis in advanced colorectal cancer patients.

ABSTRACT

Objective To observe the effects of Siweiyuganzi prescription on anti-peroxidation and blood lipid levels in experimental rats with hyperlipidemia. Methods Sixty male Sprague-Dawley (SD) rats were divided into normal control group, hyperlipidemia model group, Xuezhikang group, Siweiyuganzi prescription large, medium and small dose group according to the random number table method, with 10 rats in each group. The hyperlipidemia rat model was established by intragastric feeding with high fat emulsion everyday 10 mL·kg-1·d-1; normal saline 10 mL/kg was given to the normal control group, twice a day by intragastric feeding; 3 dosages of Siweiyuganzi suspended fluid 12.8, 6.4, 4.3 g·kg-1·d-1 intragastric administrations were given to Siweiyuganzi prescription large, medium and small dose groups respectively; Xuezhikang suspended fluid 0.3 g·kg-1·d-1 was given to Xuezhikang group intragastrically;the same volume of normal saline was given to hyperlipidemia model group. After 4 weeks, the level changes of blood lipid, serum superoxide dismutase (SOD), malonaldehyde (MDA), hydroxymethylglutaryl Coenzyme A (HMG-CoA) were observed. Results Compared to those in the normal control group, the levels of triglyceride (TG), total cholesterol (TC), low density lipoprotein cholesterol (LDL-C), MDA, content and positive expression of HMG-CoA, alanine aminotransferase (ALT) were all higher in hyperlipidemia model group [TG (mmol/L): 6.59±0.72 vs. 4.32±0.36, TC (mmol/L): 7.10±0.25 vs. 5.98±0.40, LDL-C (mmol/L): 4.18±1.30 vs. 2.33±0.35, MDA (μmol/L): 26.05± 5.99 vs. 10.08±1.98, HMG-CoA content (ng/L): 54.60±2.90 vs. 48.73±3.09, HMG-CoA positive expression in liver tissue:(57.80±12.30)% vs. (22.00±4.92)%, ALT (U/L): 106.83±15.75 vs. 81.97±13.18]; SOD and high-density lipoprotein cholesterol (HDL-C) in hyperlipidemia model group were significantly decreased [SOD (kU/L): 295.47±37.51 vs. 345.13±19.76, HDL-C (mmol/L): 2.32±0.49 vs. 4.84±0.45, both P < 0.05]. Compared with the hyperlipidemia model group, the TG, TC, LDL-C, MDA, contents and positive expression of HMG-CoA in each group were significantly reduced, and the SOD and HDL-C were obviously increased, and the changes in the Siweiyuganzi high dose group were more significant than those of the Siweiyuganzi middle-and low-dose groups [TG (mmol/L): 4.70±0.46 vs. 5.40±0.31, 5.70±0.41, TC (mmol/L): 5.80±0.23 vs. 6.14±0.20, 6.56±0.32, LDL-C (mmol/L): 2.56±0.45 vs. 2.93±0.33, 3.28±0.32, HDL-C (mmol/L): 4.58±0.28 vs. 3.89±0.30, 3.59±0.08, SOD (kU/L): 381.45±20.68 vs. 360.60±30.16, 325.49±32.13, MDA (μmol/L): 16.98±5.39 vs. 17.89±5.37, 21.03±6.01, HMG-CoA content (ng/L): 50.58±0.77 vs. 52.16±0.66, 52.90±0.91, HMG-CoA positive expression in liver tissue: (27.90±6.03)% vs. (32.20±7.00)%, (43.00±8.39)%, all P < 0.05]. In the normal control group, there were positive cells scattered in the central vein area and loosely distributed around the portal area in the rat liver; in the hyperlipidemia model group, the positive cells were increased in the central vein area and the cells in relatively great number were seen around the portal area. While the positive cells in Xuezhikang group and in the high, medium and low dose Siweiyuganzi groups were decreased. Conclusion Siweiyuganzi prescription can regulate the levels of blood lipids, prevent and treat the lipid peroxidation caused by hyperlipidemia, and inhibit excessive expression of HMG-CoA in experimental rats with hyperlipidemia.

ABSTRACT

Objective To evaluate the curative effect of Huoxue-Xiaozhi capsule on non-alcoholic fatty liver disease (NAFLD). Methods A total of 100 patients with NAFLD who met the inclusion criteria were divided into 2 groups with 50 patients in each group by random number table method. The treatment group was treated with Huoxue-Xiaozhi capsule, while the control group was treated with Compound Methionine Choline Tablets. Both groups were treated for 8 weeks. The ALT, AST, γ-GT (γ- glutamyltranspeptidase ), ALP (alkaline phosphatase, alkaline phosphate) were detected by automatic blood biochemical analyzer. The serum high sensitivity-C reactive protein (hs-CRP) and Homocysteine (Hcy) were detected by supplementary detection method. The serum IL-18 was detected by ELISA method, and liver imaging was performed before and after treatment. The symptoms and signs were evaluated, and clinical efficacy was evaluated. Results The total effective rate was 90.0% (45/50) in the treatment group and 74.0% (37/50) in the control group. The difference between the two groups was statistically significant (Z=-2.328, P=0.020 ). After treatment, the serum hs-CRP (2.4 ± 2.9 mg/L vs. 3.6 ± 2.8 mg/L, t=2.105), Hcy (11.2 ± 5.5 μmol/L vs. 13.9 ± 6.4 μmol/L, t=2.262), IL-18 (690.6 ± 61.3 ng/L vs. 775.4 ± 60.5 ng/L, t=6.962) in the treatment group were significantly lower than those in the control group (P<0.05 or P<0.01). After treatment, the improvement of hypochondriac lump, sallow complexion, dark complexion and hypochondriac dull pain in the treatment group were superior to those in the control group (Z values were -2.563, -2.788, -2.780, -2.220 respectively, all Ps<0.05 ). The Serum ALT, AST,γ-GT and ALP levels were significantly lower than those of the control group (t values were 18.820, 19.811, 10.221 and 3.248 respectively, all Ps<0.001 ). Conclusions The Huoxue-Xiaozhi capsule can improve the liver function and reduce the levels of serum hs-CRP, HCY and IL-18, and its curative effect of NAFLD.

ABSTRACT

In this paper, DNA containing six cytosines as the formation site for silver nanoclusters (Ag NCs) was adopted as a template for preparing fluorescent DNA-Ag NCs. For the first time, it was found that the fluorescence of DNA-Ag NCs could be quenched after hybridization with their complementary sequence. On the basis of this new phenomenon, we designed a sequence C1 that was completely complementary to human immunodeficiency virus (HIV) DNA, and probe DNA which was partially complementary to C1 for the synthesis of DNA-Ag NCs. The fluorescence of DNA-Ag NCs was quenched after hybridization with C1 and the DNA-Ag NCs/C1 composite was formed, while C1 could be dissociated away from the DNA-Ag NCs by HIV DNA through a strand exchange reaction due to the stronger affinity between HIV DNA and C1, which could switch on the quenched Ag NCs, thus a new "off-on" fluorescence method for HIV DNA detection was developed. In the experiment, the Ag NCs formation site of DNA, the number of base pairs, and the pH and salt concentration of binding buffer were optimized. Under the optimum conditions, the limit of detection for HIV DNA was obtained to be 3.18 nM (3σ/N, n = 7) with the linear range of 15-150 nM for the 150 nM DNA-Ag NCs/C1 probe. Besides, the probe showed excellent specificity to HIV DNA, and even distinguished one nucleotide mismatched HIV DNA.

ABSTRACT

Angiogenesis is a complicated and sequential process that plays an important role in different physiological processes. Mesenchymal stem cells (MSCs), which are pluripotent stem cells, are widely used for the treatment of ischemic and traumatic diseases, and exosomes derived from these cells can also promote angiogenesis. Therefore, we aimed to uncover mechanisms to improve MSC exosome-mediated angiogenesis. For this study, we isolated human adipose-derived MSCs (hAD-MSCs) and assessed differentiation ability and markers. Cells were divided into hypoxia-treated MSCs (H-MSCs) and normoxia-treated MSCs (N-MSC), and exosomes were extracted by ultrafiltration. Exosomes (100 µg/mL) from H-MSCs and N-MSCs were added to human umbilical vein endothelial cells (HUVECs). Exosome uptake and the ability of endothelial cells to form tubes were detected in real time. Protein samples were collected at different time points to detect the expression of inhibitors (Vash1) and enhancers (Angpt1 and Flk1) of angiogenesis; we also assessed their related signaling pathways. We found that exosomes from the hypoxia group were more easily taken up by HUVECs; furthermore, their angiogenesis stimulatory activity was also significantly enhanced compared to that with exosomes from the normoxia group. HUVECs exposed to exosomes from H-MSCs significantly upregulated angiogenesis-stimulating genes and deregulated angiogenesis-inhibitory genes. The expression of vascular endothelial growth factor (VEGF) and activation of the protein kinase A (PKA) signaling pathway in HUVECs were significantly increased by hypoxia-exposed exosomes. Moreover, a PKA inhibitor was shown to significantly suppress angiogenesis. Finally, we concluded that hypoxia-exposed exosomes derived from hAD-MSCs can improve angiogenesis by activating the PKA signaling pathway and promoting the expression of VEGF. These results could be used to uncover safe and effective treatments for traumatic diseases.

ABSTRACT

In this paper, gold nanorods and InP/ZnS quantum dots were encapsulated together in a silica medium, and the targeting molecular peptide c(RGDfC) was further connected after surface modification with PEG and PEG derivatives to prepare a multifunctional Au@QD@SiO2/PEG-c(RGDfC) probe. Dynamic Light Scattering showed that the probe size was about 215.01 ± 2.72 nm, and its dispersibility was good. In in vitro experiments when the concentration was as high as 200 µg mL-1, the activity of the cells was still 85% due to low toxicity. In vivo experiments showed that the probe had excellent tumor targeting, X-ray computed tomography (CT) imaging and fluorescence imaging capabilities. The experiments revealed that the probe had a long blood circulation time (T1/2 = 7.78 h) in mice. Biochemical analysis, liver enzyme analysis and histomorphological analysis after probe injection showed that the probe had no obvious side effects on the normal functions of the main organs, indicating good biosafety. In vivo imaging experiments showed that 6 d after intravenous injection, the tumor sites of a HeLa tumor-bearing nude mice positive group presented obvious fluorescence and CT signals, indicating that the prepared nanoprobe had good tumor targeting dual-mode imaging capabilities and therefore showed great potential in biomedical imaging applications, especially the diagnosis of cancer.

This paper aimed at evaluating the therapeutic effects of acupuncture and moxibustion in treatment of heart-spleen deficiency and observing the improvement effect for insomnia patients′ sleep quality. A total of 70 cases of heart-spleen deficiency were selected as the research data, to the all cases were divided into acupuncture group and controlgroup. Acupuncture group were treated with acupuncture and moxibustion in many acupoints, the control group weretreated with estazolampo. After 3 courses of treatment, the PSQI and TESS of the two groups were observed. The totaleffective rate of the acupuncture group was 94.3%, the total effective rate of the control group was 77.1% (P < 0.05), andthe acupuncture group was more ideal than the control group (P < 0.05) . After treatment the PSQI score and TESS scorein acupuncture group were decreased compared with control group (P < 0.05) . Acupuncture and moxibustion in thetreatment of heart-spleen deficiency insomnia curative effect is ideal and reliable, can effectively decrease the PSQI andTESS score, and has lower auxiliary work compared with taken estazolam.

ABSTRACT

Objective: To observe the clinical efficacy of acupuncture plus bloodletting therapy in treating insomnia in people with blood stasis constitution. Methods: Seventy-two patients were randomized into a treatment group and a control group by using the random number table, 36 cases each. The treatment group received acupuncture plus bloodletting therapy, while the control group was intervened by oral administration of estazolam. One-week treatment was taken as a course, for 3 successive courses in total. A follow-up study was conducted 3 months later. The Pittsburgh sleep quality index (PSQI) was evaluated before and after the intervention, as well as in the follow-up. The clinical efficacies were also compared. Results: The total effective rate was 88.9% in the treatment group versus 83.3% in the control group, and the between-group difference was statistically significant (P<0.05). After the intervention, the PSQI scores dropped significantly in both groups (both P<0.05); the between-group difference in PSQI score was statistically significant (P<0.05). The follow-up study showed that the between-group difference in the global score of PSQI was statistically significant (P<0.05). Conclusion: Acupuncture plus bloodletting therapy can produce a more significant efficacy than oral administration of estazolam in treating insomnia in people with blood stasis constitution.

ABSTRACT

Objective To investigate the effect of human hepatocellular carcinoma HepG2 cell-derived Exosome on the differentiation of mesenchymal stem cells(MSC)into cancer-associated myofibroblasts(CAF)and the impacts of CAF on liver cancer cell proliferation,migration,and invasion. Methods The protein expression of HepG2 cell-derived Exosome was detected by Western blotting. MSCs were separated from human adipose tissue and cultured with HepG2 cell-derived Exosome(100 ng/nl)to initiate differentiation. The expressions of mesenchymal markers and several interleukins were also detected by Western blotting. HepG2 cells were co-cultured with the conditioned media(CM),in which HepG2 Exosome induced the differentiation of MSC into CAF. The expressions of epithelial and mesenchymal markers were detected by real-time polymerase chain reaction(PCR)and Western blotting. Cell proliferation was assessed using MTS assay. Transwell chambers were used in the in vitro migration and invasion assay. Results HepG2 cell-derived particles expressed CD63,70 kilodalton heat shock proteins,and 90 kilodalton heat shock proteins. With the treatment of HepG2 cell-derived Exosome,the expressions of mesenchymal marker α-smooth muscle actin,fibroblast activation protein α,interleukin(IL)-6,IL-8,and IL-1β were up-regulated,while vascular endothelial growth factor had no significant change. The conditioned media which HepG2 Exosome induced MSC differentiation CAF(CAF-CM)could significantly promote HepG2 cells proliferation(1.075±0.104),compared to BSA control(0.874±0.066,P=0.023)and MSC-CM(0.649±0.034,P=0.0005). CAF-CM could significantly enhance cell migration [(42.5±9.1) cells vs.(18.5±3.1) cells,P=0.001] and invasion [(29.0±3.5) cells vs.(13.1±3.7) cells,P=0.009] compared to its control group. Moreover the conditioned medium which HepG2 Exosome induced MSC to differentiate into CAF could also promote the expressions of mesenchyme-related genes Smad interacting protein 1(P=0.040),β-catenin(P=0.038),fibronectin(P=0.029),and Vimentin(P=0.013)and inhibit the expression of epithelial related genes zonula ocdudens-1(P=0.010).Conclusions Exosome extracted from HepG2 cells can induce human adipose-derived MSC to differentiate into cancer-associated myofibroblasts. CAF-like cells can promote the migration of the liver cancer cell line HepG2.

ABSTRACT

Objective To study the single nucleotide polymorphisms (SNPs)that predict a patient's risk of grade 2-3 paclitaxel-induced peripheral sensory neuropathy (PSN) in Chinese Han populations.Methods Totally 216 patients received paclitaxel in Peking Union Medical College Hospital from May 2014 to December 2016 were enrolled.DNA was isolated from peripheral blood.Genotyping for eight candidate SNPs was performed on Sequenom-MassARRARYiPLEX platform.Patients were followed up and PSN was assessed by trained physicians according to National Cancer Institute-Common Terminology Criteria for Adverse Events v4.03.Results A total of 209 patients entered the final analysis.Among the candidate SNPs,only rs4141404:A>C(LIMK2) was significantly associated with grade 2/3 PSN (OR:4.32,95%CI:2.37-7.89,P<0.0001).In multivariate logistic regression analysis,both rs4141404:A>C(LIMK2) and history of receiving platinum compound (OR:2.70,95%CI:1.32-5.51,P=0.007) were associated with grade 2/3 PSN.Conclusion rs4141404:A>C(LIMK2) may be the markers of risk of grade 2/3 PSN.

ABSTRACT

Objective:To explore influence of intracoronary tirofiban application during percutaneous coronary intervention (PCI)on coronary blood flow,endothelial function and inflammatory factor levels in patients with acute myocardial infarc-tion (AMI).Methods:A total of 90 AMI patients undergoing PCI in our hospital were selected.According to random num-ber table,patients were randomly and equally divided into routine PCI group and combined treatment group (received intra-coronary tirofiban treatment during PCI).Coronary blood flow,serum levels of soluble intercellular adhesion molecule-1 (sICAM-1),soluble vascular cell adhesion molecule-1 (sVCAM-1),von Willebrand factor (vWF),interleukin (IL)-6, IL-10 and IL-17 after PCI were compared between two groups.Results:After PCI,TIMI grade 3 rate in combined treat-ment group was significantly higher than that of routine PCI group (77.78% vs.57.78%),P =0.042. Compared with be-fore PCI,after PCI,there were significant rise in serum levels of sICAM-1,sVCAM-1,vWF and IL-10,significant reduc-tions in serum levels of IL-6 and IL-17 in two groups,P =0.001 all;compared with routine PCI group after PCI,there were significant reductions in serum levels of sICAM-1 [(28.1±4.7)μg/L vs.(22.9±3.4)μg/L],sVCAM-1 [(34.1 ±5.5)μg/L vs.(24.0±3.7)μg/L],vWF [(661.5±94.2)U/L vs.(497.0±85.8)U/L],IL-6 [(5.01±1.16)ng/L vs.(3.30±0.82)ng/L]and IL-17 [(19.74±4.96)ng/L vs.(15.98±3.20)ng/L],and significant rise in serum IL-10 level [(2.16±0.45)ng/L vs.(2.89±0.48)ng/L]in combined treatment group,P =0.001 all.Conclusion:Intra-coronary tirofiban application during PCI can further improve coronary blood flow,relieve inflammatory response and en-dothelial function injury in AMI patients,which is worth extending.

ABSTRACT

Objective: To analyze gender differences for the long-term prognosis in patients with chronic heart failure (CHF). Methods: A total of 925 consecutive CHF patients admitted in our hospital were enrolled those including 460 (49.7%) female. The mean follow-up time was 3.4 years and the end point event was all-cause mortality. Gender differences for the long-term prognosis in CHF patients were studied by Cox proportional-hazards regression analysis. Results: The overall all-cause mortality was 27.9% during follow-up period, mortality in female was 25.2% and in male was 30.5%. Multivariate Cox regression analysis presented that the long-term death risk in male patients was higher than female (HR=1.309, 95% CI 1.002-1.708, P=0.048). Subgroup analysis showed that for patients age≥65 years, LVEF<45%and with NYHA grade III/IV, the long-term prognosis in female was much better than male. Conclusion: Long-term prognosis of CHF in female patients was better than male; female at age≥65 years, LVEF<45%and NYHA grade III/IV had the better prognosis.

ABSTRACT

Objective To observe the clinical therapeutic effect of Xiaozhong Sanjie prescription combined with Huayu powder external application for treatment of patients with nodular goiter (NG) and approach its mechanism. Methods A prospective randomized controlled study was conducted, including 128 patients who were definitely diagnosed as NG and admitted into Affiliated Hospital of Qinghai University from September 2014 to December 2016, and they were divided into a control group and a traditional Chinese medicine (TCM) treatment group by random number table method, 64 cases in each group. The control group received follow up observation; the TCM group was treated by Xiaozhong Sanjie prescription (the ingredients: rhizoma sparganii 15 g, zedoary 10 g, fritillary bulb 15 g, prunella vulgaris 20 g, bupleurum 6 g, dried tangerine 10 g, oyster 20 g, fructus aurantii 10 g, radix glycyrrhizae 5 g) in the mean time combined with Huayu powder (the ingredients: borneol 10 g, prunella vulgaris 20 g, pinellia ternata 15 g, oyster 15 g, radix curcumae 15 g, rhubarb 15 g); the ingredients of the prescription were immersed in water and decocted to form a decoction, a dose 400 mL daily, 200 mL taken orally in the morning and 200 mL in the evening after meal; the powder ingredients were mixed and stirred thoroughly with Chinese rice wine or vinegar (10 mL) to form a paste which was put uniformly flat onto a gauze about 15 cm in length and 1 cm in thickness, roasted by infrared lamp, as the paste temperature was lowered to body temperature, it was attached on the goiter skin of neck, then fixed by bandage before sleep to the next morning, afterwards the paste was removed and nodular skin washed, the length of external application being guaranteed to be over 4 hours a day; in the event when skin red swelling or pruritus occurred, promethazine should be used or the duration of application was shortened. Fifteen days constituted one therapeutic course, and after consecutive 3 courses of treatment, the therapeutic effects in the two groups were observed. Before and after treatment in the two groups, the changes of thyroid nodular size and TCM syndrome score were observed, meanwhile the thyroid functional index levels of serum vascular endothelial growth factor (VEGF), insulin-like growth factor Ⅰ(IGF-Ⅰ) and transforming growth factor-β1 (TGF-β1) were detected by enzyme-linked immunosorbent assay (ELISA). Results After treatment for 45 days, compared with control group, the thyroid nodule in TCM treatment group was significantly shrunken (mm: 8.75±3.41 vs. 15.89±4.51, P < 0.05) , and TCM syndrome score in TCM treatment group was significantly lowered (20.35±4.83 vs. 35.53±6.71, P < 0.05); before and after admission in the two groups, the thyroid function indexes of thyrotropic-stimulating hormone (TSH), free triiodothyronine (FT3), free thyroxine (FT4) were in the normal range, and there were no statistical significant differences between the two groups (all P > 0.05); the levels of serum VEGF, IGF-I were reduced in the TCM treatment group [VEGF (ng/L): 27.52±8.35 vs. 33.08±9.57, IGF-Ⅰ (μg/L): 24.12±6.74 vs. 35.57±11.39], TGF-β1 was increased in the TCM treatment group (ng/L: 363.67±97.83 vs. 225.87±47.71), and the differences were statistically significant (all P < 0.05). Conclusions Xiaozhong Sanjie decoction combined with Huayu powder external application can inhibit the levels of VEGF, IGF-Ⅰ and elevate the TGF-β1 level that possibly the mechanism of the combined therapy to shrink the thyroid nodule size.
