ABSTRACT

Experience in treatment of 168 patients with burns of the esophagus including 42 (25%) with consequences of combined chemical burns of the esophagus and the stomach is presented. In combined burn strictures of the esophagus and a pyloroantral part of the stomach with a large area of the lesion the method of repair of enteral alimentation was successfully applied. This method lies in creation of distal ended gastrostoma on the anterior wall of the stomach with gastroenterostomy by Roux. Low traumaticity, simplicity and reliability of gastrostoma healing are the advantages of this method. There were no complications after ended gastrostoma creation.

ABSTRACT

The examination of the greater omentum was performed in 102 cadavers. Most frequently the quadrangular shape of omentum was found, less frequently--triangular and appendicular shape. The anterior duplicature of the omentum receives blood supply from the right gastro-omental artery, that often has 5 omental branches. The posterior duplicature is blood supplied by spleno-omental artery, that creates the inferior arterial arch. The methods of lengthening of omentum are worked out. Satisfactory filling of the created omental flaps was proved by arteriography.

ABSTRACT

The information concerning the shape and sizes of the hepatogastric and gastrocolic ligaments which is necessary for choice of a method for their restoration after performance of the operation on the stomach is presented. In the experiment on dogs, the methods for restoration of the mentioned ligaments have been developed. After restoration of the ligaments, rapid vascularization of a scar between them and gastric walls was revealed. After restoration of the hepatogastric and gastrocolic ligaments, the favourable immediate and long-term results were noted in 15 patients.

ABSTRACT

The article describes the results of a laparoscopic transparietal puncture of the gall-bladder used for diagnosis and treatment of acute cholecystitis in 31 patients older than 60. The acute inflammatory process was found to subside due to the procedure in 30 patients, 26 of them were operated upon in the cold stage of the disease. The authors recommend a wider use of the method of urgent laparoscopy for diagnosis and treatment of acute cholecystitis in elderly and senile patients.
