ABSTRACT

An adolescent presented with a 5 hour of history of unilateral testicular pain and examination findings in keeping with a unilateral testicular torsion. On scrotal exploration, there was evidence of bilateral testicular torsion and bilateral orchidopexy was subsequently performed. Intravaginal bilateral testicular torsion is scarcely reported in adolescents and adults. Both unilateral and bilateral testicular torsions in adolescents are commonly associated with anatomical anomalies, which were not evident in this case. Despite warming the symptomatic testis for 40 minutes, viability was indeterminate. Guidelines remain equivocal about adequate reperfusion period before considering orchiectomy. Interval sonographic follow-up is recommended to assess testicular atrophy; however, considerable variation exists in practice. This is an atypical case of bilateral synchronous testicular torsion demonstrable by an absence of classical risk-factors, alongside unilateral clinical signs and symptoms. Inconsistency in guidelines for attempted reperfusion could result in orchiectomy where testicular salvage is possible.

ABSTRACT

The management of penile cancer has altered dramatically over the last decade. Confidence to excise lesions safely with smaller margins has led to the adoption of penile-preserving techniques and in turn improved the functional and cosmetic results. Patients undergoing partial penectomy (PP) find that the urethral meatus is located in an abnormal ventral position. In addition, there is a high risk of meatal stenosis. We describe our novel technique that allows the urethral meatus to be centralised after PP and creation of a pseudo-glans and wide meatus and therefore maintain the cosmetic appearance of the penis after split thickness skin grafting. The UCAPP technique allows the restoration of the normal meatal location and creation of a pseudo-glans in case of partial penectomy and therefore can improve the overall cosmetic appearance and reduce the psychological morbidity.

ABSTRACT

OBJECTIVE: It was aimed to investigate the effect of standardized ginseng extract on fertility parameters in diabetic rats. METHODS: Thirty male rats were randomly allocated into three groups of 10 rats each: 1. controls, 2. diabetes (D) and 3. diabetes + ginseng (DG). The latter two groups were rendered diabetic by i.p. injection of streptozotocin (STZ; 50 mg/kg). Standardized ginseng extract (Dansk Droge A/S, Copenhagen, Denmark) was administered per os (100 mg/kg BW) by stomach tube daily for 90 days starting one week after STZ. Ninety days post STZ the rats were sacrificed, and testis, epididymis, prostate, and seminal vesicles were weighed and subjected to histological examination. In addition, spermiogram, testicular enzyme markers, intratesticular steroid hormonal profile and testicular antioxidant status were estimated. RESULTS: The administration of ginseng extract resulted in a significant improvement of fertility parameters and testicular antioxidants together with a decrease in malondialdehyde and testicular pathological signs including degenerative changes of the seminiferous tubules. CONCLUSION: Ginseng extract may be a beneficial adjuvant therapy for diabetics suffering from infertility as a complication.

ABSTRACT

The present study contributes information about the environmental factors controlling the distribution, variation in standing crop phytomass and chemical compositional change of Ludwigia stolonifera. Also, treatment with cattle manure, as source of organic waste, was made to evaluate the response of the plant to the environmental pollution. The sandy textured bottom sediments of Ludwigia habitat at Damira irrigating canal had higher percentages of organic carbon and total soluble salts in spring than in the other seasons while the overlying water was characterized by low total soluble salts. The phytomass and assimilating surface area were increased in spring and summer months and appeared to decline in winter. The phytochemical constituents were highly concentrated during August. As common in hydrophyte, the plant organs of L. stolonifera are characterized by abundance of aerenchyma, absence of cork cells and reduction of vascular tissues. Addition of cattle manure to the aquatic habitat of Ludwigia plant resulted in an increase of organic carbon, salinity, chloronity and in a decrease of pH value of hydrosoil and water. Heavy metals accumulation showed considerable increase due to application of cattle manure and this appeared to be a reflection to the increased concentrations of these metals in the environment.
