ABSTRACT

The results were presented of the retrospective analysis of the treatment of 116 patients' with colostomies following war injuries. Reconstructive operations were performed on these patients between November 1 1991 and November 1 1994. Operations were performed on 8 patients with caecostomy, 47 with transversocolostomy, 45 with colostomy on the descendant and sigmoid colon and on 16 patients with Hartmann procedure. The average age of patients was 33.77 years. The occlusion of colostomy was performed in 81 patients, and in 19 of them the resection with T-T anastomosis was done with difficulties due to technical problems caused by adhesions, ventral hernias, defects of mesenterium and abdominal wall. Occlusion was also performed in 16 patients with Hartmann procedure. The complications encountered were: ileus in three patients, stercoral fistula in 3 (healed with conservative therapy), dehiscence of anastomosis in 1 patient (cured after reoperation, exteriorization of both ends of colon), and wound infections in 21 patients. There was no lethal outcomes.

ABSTRACT

Chronic selenium (Se) toxicosis was diagnosed in two groups of growing pigs. Emaciation, loss of hair, necrotic areas in the skin, lesions of the coronary band and hooves, postnecrotic atrophic cirrhosis of liver, and lumbal poliomyelomalacia were the principal findings. High Se concentrations were detected in blood plasma. Addition of the calculated amounts of sodium selenite directly to the feedstuff instead to mineral premix was the cause of this intoxication.

The authors describe 8 cases of carcinoma of the male breast treated at the Clinic of Surgery, Clinical Medical Center Banja Luka in the period 1968-1988. In their discussion, the authors review contemporary findings concerning the genesis, evolution and treatment of this carcinoma.

ABSTRACT

These injuries are becoming more and more frequent and the prevalent etiological factor is the traffic accident. The isolated ruptures of the diaphragm are rare, while combined injuries of the organs (craniocerebral, the injuries of spine, pelvis, thorax and extremities) are more frequent. Left hemidiaphragm occurs more often than the right rupture. Clinical feature is a result of the prolapse of abdominal organs into the thorax. Apart from anamnesis and physical examination, radiological findings are the most important for diagnosing the rupture. Gastric tube and contrast medium are of considerable help. Pleural punction should be avoided because of possible complications, abdominal punction is helpful, whereas pneumoperitoneum is dangerous in the present respiratory insufficiency. If abdominal symptoms prevail, laparotomy is indicated: while in damages of thoracis organs and in neglected cases with large adhesions, thoracotomy is carried out. The prognosis depends on the injuries of other organs while in isolated diaphragmal ruptures death-rate is minimal (if diagnosis was made early). The authors present their experiences in the treatment of 30 patients during 16 years period (1972-1988).

ABSTRACT

Contemporary way of living accompanied by everday frustrations, a frequent use of medicaments, an easy application of corticosteroids, complexity of an opeation, so as an increased incidence of polytraumas --those are the factors responsible for an increasing rate of stress ulcus. The rate of 54 verified stress ulcus in the period from 1969-1979 in Regional Medical Centre of Banja Luka, at Surgical Department, are very often indicative of this disease. Thirty eight operatively treated patients are indicative of severity of this disease. The authors are reporting their experiences in resolving this disease by method of vagotomy with partial gastrectomy in comparison to simple sutures of acutely developed ulcus in addition to PS vagotomy. Perforation, which has, usually, been bigger than the chronic ulcus, we had in 31 cases. If the reports of the world authors are reliable, perforation occurs in 5% of cases only, therefore it is clear how many such conditions has remained undetected or treated under various other "working" diagnoses: (DIC, consumer coagulopathy, fibrinolysis, etc.". The stress ulcus is the disease which is a danger to any patient at Surgical Department.

ABSTRACT

The authors are analizing the mechanism of initiation, frequency, surgical characteristics, the way of treatment and the factors affecting the prognosis of close injuries of abdomen in peace and war conditions. The discussion is about the problems of uniform surgical war-doctrine (army sanitary service, public-health service) in conditions of general civil-war defence. Own experiences are presented in surgical treatment of 179 patients with close injuries of abdomen in 10-year research on the Regional Surgical Department of Banja Luka's Medical Center.
