ABSTRACT

The staff of the Medical Center "MEDGEO" have surveyed the working conditions and everyday life determined by the specific features of their watch-and-expeditionary work for their impact on the health status of geological workers. Since 1975, more than 13,000 specialists of the industry and members of their families have been examined in different climatic and geographical regions of the country, including 5,689 persons directly in their working places. More than 3,000 specialists have been studied in the clinical setting by using comprehensive diagnostic techniques. The authors' findings indicate that in the study cohort, there is a paradoxical epidemiological situation with a high risk of death due to arterial hypertension whereas the prevalence of this disease in the watch-and-expeditionary workers do not differ from that in the country's population. According to the authors' data, the reasons for the poor situation associated with circulatory diseases are associated with both the high spread of total risk factors for cardiovascular diseases and the adverse impact of some occupational conditions. The situation is also aggravated by inadequate medical control and by the psychological peculiarities of specialists working in the watch-and-expeditionary manner.

ABSTRACT

A prospective epidemiological study of women at textile mills (a study group) working under the conditions of intense industrial noise (85-105 dBA) and at bakeries without such noise (a control group) was conducted. In the study group 353 (80%) women and 450 (71%) controls were repeatedly examined after 4.9 and 4.2 yrs, respectively. In the presence of such factors as borderline AH, smoking, aggravated heredity with respect to AH, MI and cerebral stroke, noise increased 3-fold, on an average, the risk of AH development as compared to its absence. Therefore noise can be defined as a "decisive" factor for the action of other AH risk factors.

ABSTRACT

The mean values of blood plasma lipids were studied in two age groups (20 to 29 years and 60 to 69 years) from a randomized sample of 1238 men and 1247 women. The levels of the total cholesterol, alpha-cholesterol and Tg were studied and compared according to the mean values in each age group. The cut-off points of percentile distribution of the upper 10% for the total cholesterol and the magnitude of the lower 10% decile for alpha-cholesterol were determined.

ABSTRACT

A random sample of males aged 20-69 years and living in one of the administrative districts of Moscow was studied. The total number of those enrolled was 1225, and they were studied for the incidence of dyslipoproteinemias and coronary heart disease (CHD) in relation to the level of glycemia, basal insulinemia, diabetes mellitus or impaired tolerance to glucose. It was shown that the effect of disorders of carbohydrate metabolism is largely determined by the level of insulinemia and is mediated through the development of dyslipoproteinemias. The prevalence of CHD without dyslipoproteinemias as well as in combination with arterial hypertension or an excessive body weight did not depend on the level of glycemia and baseline insulinemia or the presence of diabetes mellitus. Hyperinsulinemia was found to be associated with types of dyslipoproteinemias most unfavourable in terms of CHD development.

ABSTRACT

A single epidemiologic screening of schoolchildren at 11-17 years of age revealed a higher heart performance index during exercise peaks in individuals with one or several atherosclerosis precursors, as compared to that of children showing no precursors of the disease. It is suggested that high heart performance index in schoolchildren with atherosclerosis precursors reflects cardiovascular overstrain rather than high exercise tolerance.

ABSTRACT

The incidence of arterial hypertension in 1247 women (74% of response rate) has been studied. It accounted for 26.2% in women, aged 20-69, and increased with age. The incidence of systolic hypertension is significantly greater in women 50 years of age or older, diastolic hypertension occurring more often in women 20-49 years of age. The incidence of arterial hypertension in women is associated with lipid metabolism disturbances and excessive body weight. The level of blood pressure in blue-collar women is somewhat higher than in white-collar women.

ABSTRACT

In a cross-sectional epidemiologic study of schoolchildren aged 11-17 years, the cardiac work index at exercise was found to be higher in schoolchildren with arterial hypertension (AH) and excess body weight than in the control group. The duration of an exercise test was similar in the two groups. The higher cardiac work index at exercise may be explained by considerably higher systolic blood pressure in schoolchildren with AH and by higher heart rates in those with excess body weight.

ABSTRACT

A prospective study of 40-59 year-old males from one of the Moscow districts has shown the correlation between newly developed cases of arterial hypertension and blood pressure level, blood lipids, body weight, smoking. Complex informative index of the above parameters for possible hypertension development is being analyzed. The most informative criteria have been stated and the prognostic value of the index for possible hypertension development has been established.

ABSTRACT

The analysis of data of the prospective epidemiologic survey of the male population aged 40-59 of one of the Moscow districts determined relative limits of normal values of the BP reaction during the exercise test on the treadmill. The incidence was found of different BP reaction types to exercise in the examined population. There was determined prognostic significance of the exercise test on the treadmill for detection of persons at risk of the development of arterial hypertension.
